Discipline policy re: violence between students
In After School we define violence as physical, verbal, social or intimidation. The first incident of violence will require all students involved to have a meeting with the After School director and parent conferences with the After School Director. More than one act of violence or intimidation or an egregious act of violence or intimidation or harm to another child or adult will result in a suspension of up to 1 month without a refund, and or a meeting with the director of the program and the director of the school, probation upon returning to the program and or an expulsion from the program.
General Release
My child has permission to engage in all After school and CACS SUMMER CAMP activities unless otherwise noted. In consideration of acceptance of this authorization, intending to be legally bound, hereby, for ourselves, our heirs, executors and administrators, waive and release all rights and claims that may arise against CACS, and persons affiliated with After School or CACS SUMMER CAMP. I give my permission for my child to go on field trips within walking distance from CACS. My child will be with staff at all times. I have provided After School and CACS SUMMER CAMP with emergency contact information, which will be taken on all off-campus activities.
Emergency Medical Care
I give my consent for any CACS staff to take appropriate action for the safety and welfare of my child, including transporting my child to an emergency facility if necessary. In the case of an emergency, he/she will be taken to the nearest hospital. I agree to keep my child's health information and our family contact information up-to-date so school staff can reach me at any time.
Financial information and Emergency contacts
I understand that it is my responsibility to update my financial information prior to being billed. I understand that failure to do so results in a $25. fee if my card is declined.
I understand that it is my responsibility to update my child's emergency contact. Failure to do so will be noted when emergency information is needed. Failure to update will result in a $50. fine.
I understand that it is my responsibility to update my child's grade every year.
CELL PHONE POLICY IN AFTER SCHOOL
I understand that my child will not have access to their cell phone during After School. I accept that my child will turn in their phone to After School staff at the beginning of After School, that it will be kept in a locked box and will be available for me or my child to pick up when signed out of After School. I understand and accept that if my child refuses to give up their phone to After School staff that they may not attend After School and will have to leave campus. If my child is under 12 yrs of age I understand that I will have to arrange for them to be picked up within 15 minutes of After School starting ( 3:30pm/ 1:30pm Tuesdays). I understand that if this happens repeatedly my child can no longer attend After School until the problem is resolved. I am still responsible for any fees incurred during this resolution time.
Discipline Policy
Participation in the AFTER SCHOOL PROGRAM and or CACS SUMMER CAMP is a privilege. Students are expected to follow school rules and expectations and show respect for one another. After three (3) warnings, a child may be sent home for the rest of the day and/or may be suspended for the next day of attendance. If a student is sent home for a second occurrence, a mandatory conference with the After School or SUMMER CAMP coordinator and/or school administration and the parent is required for readmission to the program. Upon a third occurrence, the child will be dismissed from after school for the remainder of the week or month. There are no refunded fees for time sent home or suspension from the After School Program.
Student Sign-Out
Every student needs to be signed out each day they attend our AFTER SCHOOL PROGRAM or CACS SUMMER CAMP by an authorized adult. Failure to sign out results in a $25. fee. A text from a parent or guardian to After School (415-542-8502) and or the person picking up your child is required from a parent/guardian to release a child to an adult other then themselves. This text to the pick up person must be shown to an After School staff person. Only middle school students can sign themselves out provided their parents have given consent ( by emailing zcumings@creativeartscharter.org giving permission). Only children in the sports program can come back to campus after leaving at the end of the day. Otherwise once a child leaves campus they cannot return unless they are an older sibling picking up a younger sibling.
Late Pickup
Please call (415-542-8502) if you expect a late pick-up of your child.
AFTER SCHOOL PROGRAM:
All students still on campus 20 minutes after their dismissal will have a $25. late fee applied. If they are still on campus at 4pm parents will be charged a full day drop-in rate of $50.
All Grades/6pm Pickup: Call (415-542-8502) if you are going to be late to pick-up your child. A $25 late pick-up fee applies for all late pickups beginning at 6:05p.m. An additional late fee of $25 will be charged at 6:30pm and for every 15 minutes following. Habitual lateness will result in dismissal from the program. Late fees will be charged to the credit card or bank account on file.
CACS SUMMER CAMP late pick-up fee is $1.00 per minute after 5pm.
Schedule Changes
I understand that all AFTER SCHOOL PROGRAM changes in schedule must be requested through the Jackrabbit care system by the parent within the deadline stated. For Monthly and club changes that deadline is the first Friday of the month. I also understand that the changes will be effective on the next deadline date, depending on the date of my request. MONTHLY REGISTERED requests made after the deadline date will result in an additional administration fee of $35.
I understand that SUMMER CAMP Schedule changes must be requested prior to camp starting in order to receive a credit.
Payments
AFTER SCHOOL PROGRAM fees will be charged in equal monthly payments September through June. Fees are payable for the student's confirmed schedule and are not discounted for absences. All changes in schedule must be requested through the Jackrabbit care system by the parent within the deadline stated. For Monthly changes that deadline is the first Friday of the month. Fees will be increased for time used outside of the student's confirmed schedule at the current DAY drop-in rate. I understand that I am responsible for and will be charged for registered days of AFTER SCHOOL until I change my child's schedule via the jackrabbit parent portal.
I understand that all CACS SUMMER CAMP FEES WILL BE CHARGED PRIOR TO THE FIRST WEEK OF CAMP.
Declined/ Late Payments
In the event a payment is declined for any reason, I know I will receive a notice from Jackrabbit stating that my payment was declined. I authorize CACS to assess and collect a Decline Fee of $25 for each occurrence, UP TO 2X A MONTH if I fail to contact CACS to resolve the payment. I understand if any of my scheduled payments are declined, it is my responsibility to keep my payment method up-to-date. Until the problem is resolved and the payment plan brought current, I understand my child may be transferred out of the AFTER SCHOOL PROGRAM or lose their space in CACS SUMMER CAMP.
