Celebrate the craziest holiday of the year at the C.I.T.Y. Club Spooktacular! Flip out in our haunted maze, show your skills at the carnival games, strut your stuff in our costume contests, and be the fastest ghoul through our obstacle courses. Parent participation for children ages 5-7 is required at this event; LIMIT 1 grownup per child. Two time slots to choose, 7-8pm and 8-9pm. Doors open 15 mins. before and close 15 mins. after start times, no exceptions. Bring your email confirmations (digital or paper) as your ticket for entry. Rates: $20 Members. $25 Non-Members. Parent(s)/Guardian free. **There are no sibling discounts for this event.** SPOTS ARE LIMITED! All participants must pre-register. Liability Waiver

1) The undersigned will be present at and participate in activities at C.I.T.Y. CLUB GYMNASTICS ACADEMY. Such activities may include, but without limitation, instruction, training and competition in the sport of gymnastics, social and recreational activities, lodging, meals, transportation, and other services (any and all of the foregoing are referred to in this document as "Activities") whether in person, Skype, Zoom, or other video conferencing tool.

2) In consideration of participating in the C.I.T.Y. CLUB GYMNASTICS ACADEMY I represent that I understand the nature of these Activities and that I am qualified, in good health, and in proper physical condition to participate in such Activities. I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participation in the Activity. I fully understand that these Activities involves risks of serious bodily injury, including permanent disability, paralysis and death, which may be caused by my own actions, or inactions, or those of others participating in the event, the conditions in which the event takes place, or the negligence of the "releasees" named below; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, cost, and damages I incur as a result of my participation in any of these Activities.

3) I acknowledge, understand, appreciate and agree that participation in Activities may result in possible exposure to and illness from infectious diseases, including, but not limited to, MRSA, Influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others, and assume full responsibility for my participation and exposure.

4) I hereby release, discharge, and covenant not to sue C.I.T.Y. CLUB GYMNASTICS ACADEMY, its respective administrators, directors, agents, officers, volunteers, and employees, other participants, any sponsors, advertisers, and, if applicable, owners and lessors of premises on which the Activity takes place, (each considered one of the "RELEASEES" herein) from all liability, claims, demands, losses, or damages, on my account caused or alleged to be caused in whole or in part by the negligence of the " releasees", or otherwise, including negligent rescue operations and future agree that if, despite this release, waiver of liability, and assumption of risk I, or anyone on my behalf, heirs, or assignees, makes a claim against any of releasees, I indemnify, save, and hold harmless each of the Releasees from any loss, liability, damage, or cost, which any may incur as the result of such claim.

5) FOR PARTICIPANTS OF MINORITY AGE. This is to certify that I/we, as parent(s)/legal guardians(s) with legal responsibility for the Participant, do hereby consent and agree not only to his/her assumption of risk, waiver, release and indemnification, but also for myself/ourselves, and my/our heirs, personal representatives, administrators, successors and assigns, to waive, release and indemnify C.I.T.Y. CLUB GYMNASTICS ACADEMY and "releasees" from any and all Claims incident to my/our child's participation in the Activities as stated above, even if arising from the negligence of C.I.T.Y. CLUB GYMNASTICS ACADEMY or one of the "releasees" to the fullest extent permitted by law.

6) I have read the RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT, understand that I have given up substantial rights by signing it and have signed it freely and without any inducement or assurance of any nature and intend it to be a complete and unconditional release of all liability to the greatest extent allowed
