I would like my dancer to participate in PERFORMANCE COMPANY. This includes a class jazz dance and being a part of our Performance Company Production number. *
I would like my dancer to participate in HIP HOP CREW. I understand the additional rehearsal time, rehearsal fee, costume fee and competition entry fees involved.*
POLICIES - I have read, understand, and will comply with all of the policies of the Dance Factory and the requirements for the 2019-2020 Performance Company.*
REQUIREMENTS - As listed in the Performance Company Details my child plans to participate in the required CORE classes, rehearsals, competitions, etc.*
BIRTH CERTIFICATE - I will submit a copy of my child's birth certificate. PERTAINS TO NEW PERFORMANCE COMPANY STUDENTS ONLY.*
FEES - I understand that all fees are non refundable, will be automatically debited when due unless prior payment is received, and that I must have an active credit card on file at all times.*
ATTENDANCE - As listed in the Performance Company Details my child plans to participate in the required classes, rehearsals, etc.*
I would like for my dancer to be enrolled in the optional PERFORMANCE COMPANY BALLET/TECHNIQUE class to enhance their technique skills. I understand that this may be on an additional night. *

I UNDERSTAND THAT IN ORDER TO AUDITION FOR THE 2020-2021 PERFORMANCE COMPANY MY ACCOUNT MUST BE CURRENT. NO EXCEPTIONS.*
CHARGES DUE ON ENROLLMENT - Upon registration, I understand that my credit card will be charged $70 for the following one time fees; $25 audition fee and $45 enrollment registration fee.*
