Getting started
Welcome to the School Year Session (August 31st, 2020 - May 29th, 2021). Once you enroll, we will make a one (1) time charge to the credit card you provided for the NON-REFUNDABLE Registration Fee of $50 once you enroll (all families) and the First Month's Tuition on September 1st 2020. NOTE: Online registration will not give the 10% discount for the second class/ child's tuition. PSGA staff will adjust the account as it comes through the portal. You may do the math yourself and make the payment or we will do it for you using the credit card on file. If you have any questions about your balance, please contact the Front Desk (478-333-3930). Only enroll in the classes you wish to be charged for. All participants should come dressed in a leotard or shorts and t-shirt (no buttons or zippers), no jewelry, hair up and barefoot.
Dancers must wear a leotard and tights, and the appropriate shoes IE..Ballet, Tap or Jazz shoes. If you have any other questions, please do not hesitate to contact us and thank you for choosing PSGA.

DISCLAIMER: PRECISION SOUTH GYMNASTICS ACADEMY IS NOT RESPONSIBLE FOR ANY INJURY(OR LOSS OF PROPERTY) TO ANY PERSON WHILE PRACTICING, TAKING CLASS, COMPETING, PARTICIPATING IN PARENT'S NIGHT OUT, OPEN GYM, BIRTHDAY PARTIES OR ANY OTHER WAY INVOLVED IN GYMNASTICS CHEERLEADING OR DANCE AT PRECISION SOUTH GYMNASTICS ACADEMY FOR ANY REASON WHATSOEVER INCLUDING ORDINARY NEGLIGENCE ON THE PART OF PRECISION SOUTH GYMNASTICS ACADEMY, ITS OWNERS, OFFICERS, AGENTS OR EMPLOYEES.
In Consideration of participating in the Precision South Gymnastics Academy, I represent that I understand the nature of this Activity and that I am qualified, in good health and proper physical condition to participate in such Activity. I acknowledge that if I believe conditions are unsafe, I will immediately discontinue participation in the Activity. I fully understand that this Activity involves risks of serious bodily injury, including permanent disability, paralysis and death, which may be caused by my own actions, or inactions, those of others participating in the event, the conditions in which the event takes place, or the negligence of the "RELEASEES" named below; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume such risks and all responsibility for losses, cost, and damages I incur as a result of my participation in this Activity.
I hereby release,discharge, and covenant not to sue Precision South Gymnastics Academy, it's respective administrators, directors, agents, officers, volunteers and employees, other participants, any sponsors, advertisers, and, if applicable, owners and lessors of premises on which the Activity takes place, ( each considered one of the "RELEASEES" herein) from all liability, claims, demands, losses, or damages, on my account caused or alleged to be caused in whole or in part by the negligence of the "RELEASEES" or otherwise, including negligent rescue operations and future agree that if despite this release, waiver of liability, and assumption of risk, or anyone on my behalf, makes a claim against any releases, I will indemnify, save, and hold harmless each of the releases from any loss, liability, damage, or cost, which any may incur as the result of such claim.
I have read the RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT, understand that I have given up substantial rights by signing it and have signed it freely and without any inducement or assurance of any nature and intend it to be a complete and unconditional release of liability to the greatest extent allowed by law and agree that if any portion of this agreement is held to be invalid the balance, notwithstanding, shall continue in full force and effect.
Parental Consent
AND I, the minor's parent and/or legal guardian, understand the nature of the above referenced activities and the minor's experience and capabilities and believe the minor to be qualified to participate in such activity. I hereby release, discharge, covenant not to sue and AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS each of the Releasees from all liability, claims, demands, losses or damages on the minor's account caused or alleged to have been caused in whole or in part by the negligence of the Releasees or otherwise, including negligent rescue operations, and further agree that if, despite this release, I, the minor, or anyone on the minor's behalf makes a claim against any of the above Releasees, I WILL NOT INDEMNIFY, SAVE AND HOLD HARMLESS each of the Releasees from any litigation expenses, attorney fees, loss liability, damage, or cost any Releasee may incur as the result of any such claim.

Medical Emergency
The undersigned gives permission to Precision South Gymnastics Academy, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian.
Payment Information
School Year Session~Registration will require a NON-REFUNDABLE $50 family registration fee. We will run the registration fee and any balances on the account once you register. We will run tuition monthly beginning September 1st, 2020 to the card on file. If your card is declined, you will forfeit your spot(s).
Payments not paid by the 15th will result in an automatic payment to the card on file.
Make Up Policy
I understand that the make-up policy allows for (1) makeup per month. Please call (478)333-3930 to schedule your makeup, the child(ren) have to miss the class first. Makeups will need to be made within two weeks of the missed class. There are no makeups allowed for a missed makeup class.
Photo Release
Occasionally pictures are taken of the students for the PSGA website, wall displays and other potential media. I hereby grant PSGA permission to use my daughter's/son's photograph(s)/video(s) in any and all of its publications, such as news releases, newsletters, brochures, Facebook and the PSGA web site.
If you do not agree to this, please check the box and call (478)333-3930 so that we can place this in your file.
Spectator Guidelines
Absolutely no spectators allowed in the gym.
COVID-19-----In order to be in compliance with State Laws, only one adult per student will be allowed to enter the facility. No siblings or additional family members/spectators will be admitted. Spectators in the viewing area will be responsible for sanitizing their seating area before and after use. Maintaining social distance will be at the discretion of the individual. Cleaning products will be available at the sanitation table for the convenience of all spectators. A thorough cleaning of the viewing area will be done by staff at the end of each evening. PSGA Staff will also be available to assist your child if you wish to drop off and pick up at the door. This is highly recommended.
All spectators must view from the designated viewing areas located on the second floor balcony. Spectators must refrain from communicating, disrupting or interfering with gymnasts and coaches. Please be respectful to all other spectators by sitting in the designated seats, do not lean on or over the railing or stand at the railing as you will be blocking the view of others. Please watch your children (siblings). No children should be left unattended. Gymnastics should be done in the gym under the supervision of PSGA instructors, not in the lobby or viewing areas. If you need assistance getting your child into their class if you've arrived late, or need to pick-up early, please see the front desk staff, they will help make sure your child gets where they need to go safely. No parking in the drop-off/ pick-up line. Management does not take responsibility and is not liable for any injuries and/or lost and damaged personal property occurring in the viewing area and/or other common areas of PSGA. PSGA is not responsible for fire, theft damage to, or loss of vehicle or any articles left therein while parked on PSGA property. Please be considerate of others while at PSGA. Management reserves the right to ask those not respecting these guidelines to leave.
Withdraw Notice
If you wish to withdraw your child(ren), we require you to sign a 30 day withdrawal notice at the front desk. You have to sign the withdrawal form in order to stop the fees from being applied monthly.
30 day withdrawal does NOT APPLY to competitive students under contract.
Risk Management
I/We, the above participant(s), and parent(s), guardian(s), do hereby consent to participation in the above program including all activities to the program. I/We assume all responsibilities for, and risks and hazards of, participation in the named programs, even though all will be done to keep all children safe, risk or injury may occur. I/We understand that NO REFUNDS will be issued. I have been informed and understand that this program is not a licensed childcare facility. I acknowledge and understand that this program is not required to be licensed by the Georgia Department of Early Care and Learning. I acknowledge and understand that this program is exempt from state license requirements.

Covid-19
Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.
Precision South Gymnastics Academy LLC ("the Club") has put in place preventative measures to reduce the spread of COVID-19; however, the Club cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending the Club could increase your risk and your child(ren)'s risk of contracting COVID-19.
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending the Club and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at the Club may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Club employees, volunteers, and program participants and their families.
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)'s attendance at the Club or participation in Club programming ("Claims"). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless the Club, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Club, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Club program.
COVID precautions
TEMPERATURE CHECK AND HAND SANITIZATION
Upon entering the facility, you will be greeted by our Wellness Staff. Each person will be scanned with an infrared toughness thermometer and given a squirt of hand sanitizer. Anyone with a temperature measuring 100.1 or greater will not be allowed to enter the facility. The Wellness Staff will also monitor for any flu like symptoms. anyone exhibiting symptoms will be asked to leave and reschedule their class for a later date.
