Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.
The Dance Academy has put in place preventative measures to reduce the spread of COVID-19; (See procedure on website) however, the Studio cannot guarantee that you or your children will not become infected with COVID-19
* I, or my child are not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell. __________
* I, or my child have not traveled internationally within the last 14 days.__________
* I, or my child have not traveled to a highly impacted area within the United States of America in the last 14 days.__________
* I, or my child do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.__________
* I, or my child have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.__________
* I, and my child are following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19. ___________
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending the Studio and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at the Studio may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Studio employees, volunteers, and program participants and their families.

I give permission and consent to the directors of The Dance
Academy to use my child's images, photographs, or videos in promotional videos, television spots, or any
other media, including social media and Web Site advertising, new coverage or any other commercial use
of our events.
Studio Policy
GENERAL INFORMATION AND STUDIO POLICY FOR THE 2020-2021 SEASON:
The Dance Academy offers a complete program of dance education for children, teens,
and adults. Classes are available for all levels from the beginner to the advanced. A multiple class
professional program for those interested in intensive study is also provided.
Classes Begin Thursday, September 10, 2020.
Our Season runs from September 2019 through Thursday, June 2021.
CLASS LEVELS AND PLACEMENT:
Our studio uses a system of levels to match students as closely as possible within each
subject. Students are placed based on age and years of experience as well as on size, skill
level, coordination, musicality, concentration and maturity in accepting corrections. Each class
can therefore be paced to meet the needs of the majority. It is important for parents and
students to understand, however, that dance class levels are not the same as grade levels in
school, and that students may not always change levels each year. This does not mean
that they have not made progress or that they are being "held back". In fact, in most cases the
entire class progresses as a unit. Students dancing significantly above their level will be moved
to the next, based on evaluations by their teachers. As we progress through the season, we will
video tap classes weekly so that we are able to move to a virtual scenario should we be forced
to close by state mandate.
REGISTRATION FEE:
A $25.00 registration fee, the first installment, and a completed and signed registration
form are required to reserve class space. A registration is not considered complete and class
space is not reserved until all three steps are completed. Due to state regulations, our class
sizes are currently reduced until further notice. If you are unable to register for a class because
it is closed, please contact me directly so that we can create options for your dancer.
TUITION:
Tuition is posted to each student account at the beginning of each month. Accounts not
paid in full by the 15th of the month will be charged a $15.00 late fee. Reminders are sent via
email, website, text message and social media blast. It is your responsibility to submit your
payment before the 15th of the month. We do not automatically charge your credit card. You
can monitor your account and make credit card payments via the parent portal on the TDA
website. Paying by check or cash? Please place payment into an envelope with your child's
name on it and drop it into the office mail slot.
