Columbus Gymnastics reserves the right to use any video or photographic material that may contain images of your child(ren) for any lawful purpose.
COVID-19
I understand that the coaches, instructors, and staff at Columbus Gymnastics Academy will make a strong effort to maintain physical distance, but there will be times where incidental contact and less than prescribed physical distancing will occur. I am aware and agree that spotting is an essential aspect of training my athlete in order to keep him/her safe and prevent injury. I will allow my child to be spotted when spotting is necessary. I further understand that I am voluntarily allowing my child to participate in programs and activities offered by Columbus Gymnastics Academy, knowing that it is impossible to keep him/her, myself or anyone else who enters the gym completely safe from exposure to the COVID-19 virus. I accept that risk.
Video Release
I understand that I have enrolled my child or children at Columbus Gymnastics Academy, Inc. located at 6810 Thrush Dr. Canal Winchester, OH, also referred to herein as the "CGA". CGA has a program whereby webcams are in use and my children are under streaming video surveillance that is used for training and management purpose, in addition to, secure accessibility from the web in accordance with the terms and conditions associated with the Spot TV website (also referred to herein as "Spot TV"). By my signature below, I hereby consent to the photographing of myself and/or my minor child(ren) and the recording of my voice and or that of my child(ren) named above. I agree that I have no rights to any of the video footage or photographs for any reason at any time. I also agree that I will not screenshot, copy, reproduce, alter, modify, or create derivative works from the Content and Service. I understand that unauthorized recording, duplication, or distribution of this copyrighted work is illegal. I assume full liability of all the terms outlined in this release for any family member's account activity associated with my child or children. Copyrighted work includes all web streaming and video/audio recordings. I understand that legal action can be taken against me by Spot TV or Children's Activity Center for such copy right infringement. I understand that the term "photograph" as used herein encompasses still photographs, audio, and motion picture footage.

Assumption of Risk
As the parent or legal guardian of the registered child, I hereby consent to his/her participation in the programs and classes offered at Columbus Gymnastics Academy, Inc.
By the very nature of the activity, gymnastics, and tumbling all carry a risk of physical injury. No matter how careful the student and coach, no matter how many spotters are used, no matter how skilled the student or coach, no matter how many mats are provided, and no matter how many times the skill may has been performed successfully, the risk cannot be eliminated. Reduced yes. But never eliminated.
I UNDERSTAND AND ACCEPT THAT RISK.
In consideration of my child(ren)'s participation, I hereby release and covenant not-to-sue Columbus Gymnastics Academy, Inc., or it's Board of Directors, the officers, employees, teachers, coaches, or agents, from any and all present and future claims resulting from accidents or ordinary negligence on the part of Columbus Gymnastics Academy, Inc., or others listed for property damage or personal injury arising as a result of my child's participation in gymnastics, tumbling or any other activities or any activities incidental thereto, while at Columbus Gymnastics Academy, wherever, whenever, or however the same may occur.
As the parent or guardian of the named child(ren), I hereby agree to individually protect for the possible future medical expenses which may be incurred by my child as a result of any injury sustained while participating in any program of instruction, recreation, or competition at, for, or under the direction or control of Columbus Gymnastics Academy, Inc.
This Acknowledgement of Risk, Waiver of Liability, and Indemnification Agreement shall be ongoing and shall apply during all the years that the above named student participates in activities under the direction or control of Columbus Gymnastics Inc. This Acknowledgement of Risk, Waiver of Liability, and Indemnification Agreement, having been read thoroughly and understood completely is signed voluntarily as to its content and intent.
Medical Authorization
In the event of an accident or emergency, I hereby authorize my child(ren) to be transported to a hospital for medical treatment and I hold Columbus Gymnastics Academy and it's representatives harmless in the execution of such. Additionally, I hereby agree to individually provide for all medical expenses which may be incurred by myself or my child(ren) as a result of any injury sustained while participating at or for Columbus Gymnastics Academy. I have read and understand this MEDICAL AUTHORIZATION and I VOLUNTARILY affix my name in agreement.
Payment Policies
Session fees are due prior to the first class. A $20 late fee will be attached to all unpaid balances beginning week two of the current session. You are responsible for the entire session fee even if your child does not finish the session. All returned checks are subject to a $25 fee. WE DO NOT GIVE CREDIT OR REFUNDS FOR MISSED CLASSES.
