Covid-19 Assumption of Risk & Liability
The novel Coronavirus, COVID-19, has been declared a worldwide Pandemic by the World Health Organization
(WHO). COVID-19 is extremely contagious and is believed to spread mainly from person to person contact. Studio 3 Entertainment, LLC has put in place measures to reduce the spread of COVID-19; however, Studio 3
Entertainment, LLC cannot guarantee that you or your child(ren) will not become infected with COVID-19.
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk
that my child(ren) and I may be exposed to or infected by COVID-19 by attending Studio 3 Entertainment, LLC.
The exposure or infection may result in personal injury, illness, permanent disability, and death. I understand
the risk of becoming exposed to or infected by COVID-19 may result from the activities, omissions, or
negligence of myself and others, including, but not limited to Studio 3 Entertainment, LLC employees, staff, volunteers,
participants, and their families.
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my
child(ren), or myself (included, but not limited to, personal injury, disability, and death) for illness, damage,
loss, claim, liability, or expense of any kind. On my behalf, and on behalf of my child(ren), I hereby release,
covenant not to hold Studio 3 Entertainment, LLC, including all liabilities, claims, actions, damages, costs, or
expense of any kind arising out of or relating thereto. I understand and agree that this release includes and
Claims based on the actions, omissions, or negligence of Studio 3 Entertainment, LLC and its employees
whether a COVID-19 infection occurs before, during, or after participation in my dance class/program.
Assumption of Risk & Release of Liability
In consideration of being permitted to have my child participate at Studio 3 Performing Arts or in online "virtual" classes, I do hereby agree to release and indemnify Studio 3 Performing Arts, its owners, and instructors from any liability, claim, injury, or damage of any kind resulting from my child's participation in classes, activities, and performances, and for the rendering of any necessary medical treatment whatsoever may have to be rendered due to his/her participation. I also give permission for my child's photograph, or recorded performance or practice to be used for marketing, advertising, or other purposes, regardless of whether or not my child is a current student. I release Studio 3, their photographer, employees, agents and designees from any liability for any violation of any personal or proprietary right I may have in connection with such use.
Enrollment & Cancellation Policy
I AM AWARE OF AND UNDERSTAND STUDIO 3'S CANCELLATION/ENROLLMENT POLICY AS DETAILED BELOW
Studio 3 Enrollment is for the entire "performing year": August 3, 2020 to June 19, 2021.
Cancellation Policy for ongoing students: To cancel/drop classes or private lessons, you must provide 30 days written notice via EMAIL ONLY. As of the date of your cancellation letter, you will be billed for the remainder of the 30-days regardless of attendance. Cancellation of company classes (S3TV & PrePro) will result in a $200 fine. There will be no refunds for costumes or other fees paid.
Makeup Classes: Students may makeup group classes within 30 days of an absence by attending another class in the same level and style. There are no refunds for classes missed or not made up. Students with injuries/medical issues may petition an extension on makeup classes.
Summer Registration and Cancellation Policy: Non-current students will need to set up an account but will not be billed a registration fee for summer. A credit card is required to register online, but you may let us know in the comments of your registration you wish to pay by cash or check. Cancellations are refunded minus a $25 non-refundable deposit based on the following: 100% within 21 days, 75% at 15-20 days before, 50% at 7-14 days before. No refund credits will be issued for camps cancelled less than 7 days..
Should another stay-at-home order be put into place that prevents our in-person classes from happening, students will move to our Studio 3 Virtual Academy for online learning through Zoom until in-person classes resume. In the event of a fully virtual experience, all classes will continue and cancellation policies will still apply.
Registration & Fees
I AM AWARE THAT I MUST PAY REGISTRATION & PRODUCTION FEES ON TOP OF TUITION.
- A Registration Fee is due upon registration each performing year (August to June. There is no registration fee for summer camps.) Fee is $35 per student or $50 per family. Registration fees are non-refundable.
- A Production Fee is also billed for students registered in performing classes (dance, voice, theatre, musical theatre, & Private lessons). Recital is NOT optional. This fee covers recital and performance costs, including auditorium rental, staffing, video productions, virtual recitals, showcase week, and other related expenses.
- The Production Fee includes recital extra rehearsal fees, 2 tickets to the production, and access to the digital video recording of the production. $85 for the first student. $30 per additional sibling.
- Performing Classes will be billed for costumes at $80 +tax each. Sometimes we are able to get costumes at cheaper rates. if that is the case, you will receive a partial refund for the difference once costumes are ordered and received.
- Extension Program, Pre-Professional Program, and company will not be charged additional fees beyond the registration and production fee. These programs are billed at a package price with additional classes added at a discounted rate.
Payment Policies
I AUTHORIZE STUDIO 3 TO BILL MY CARD ON FILE ON THE 25th OF EACH MONTH PRECEDING FOR TUITION & FEES OWED. (Except January tuition, which is billed on December 28th)
- Payment is done by monthly automated billing. Studio 3 requires that every family keep a credit card on file. You may choose to pay by cash or check, but payment must be received by the 25th of the month, or the credit card on file will be billed.
- If you choose not to keep a card on file, you must pay June 2021 tuition up front to be always 30 days ahead of billing.
- Some months have 4 weeks, others 3 or 5. The tuition rates you are paying are based on 11 equal payments for the entire cost of the program, not based on the number of weeks for that month.
- If a credit, debit, or check auto pay is declined for any reason, including expired cards, you will be charged a $10 processing fee. You will be contacted to remit payment, which must be received immediately. If we have not received your payment by the following business day, you will be charged an additional $15 late fee per week that payment is late.
-Returned checks will be charged a $35 returned check fee and will be subject to additional $15 per week late fees.
- If your account is delinquent, your child will not be able to participate in classes or activities until all owed payments are made. Accounts delinquent for more than 30 days will be sent to collections.
- Registration, production, costuming, and other fees are NON-REFUNDABLE.
- All checks should be made out to "Studio 3 Performing Arts Academy"
- Sibling discounts are available to families with multiple students enrolled (excluding private lessons). Student with the lowest tuition cost receives the discount.
- A 10% tuition discount will be given to families who pay for the semester in full by cash or check. (Semester payment must be paid by cash or check to receive the discount). Discount is not applicable to costumes, registration, & other fees. Due by July 25, 2020 for Fall tuition and December 28, 2020 for Spring tuition.
- Please keep receipts for all transactions, especially cash payments. Studio 3 and its employees will not be held responsible for payments made without evidence of a receipt. If a cash payment is in question, unless you have a receipt we will assume payment was not made.
Late Pickup Penalty
I UNDERSTAND THAT students may not hang out unsupervised at the studio for long periods before or after their class(es). Students should be dropped off no more than 5 minutes prior to their class and picked up immediately after. Students who are picked up late will be subject to a $10 per 10 minutes late pickup fine.
Communication
I AUTHORIZE Studio 3 to regularly contact me at my email address(es) on file and understand that this is the primary means of communication.
I AM AWARE that all families must attend a VIRTUAL student orientation in the fall. There are multiple dates/times to choose from.
I AM AWARE that all families (new AND returning) must also have one adult present at the recital information meeting in the spring. There will be multiple dates/times to choose from.
Classroom Expectations
By registering for classes, you are aware of Studio 3's social distancing policies and agree to follow policies and procedures as outlined in Studio 3's Student Handbook. This includes our Behavior Expectations/No Tolerance Bullying Policy: Studio 3 will continue to provide a safe and enjoyable learning environment, and will not tolerate inappropriate behaviors. In the rare case of a parent or student showing disrespect or defamation to any parent, staff member or student, a meeting will be called immediately and dismissal may be considered at the discretion of the staff. Physical, mental, emotional or cyber bullying by parents, staff or students will not be tolerated and may result in dismissal from the studio.
Studio 3's Social Media Policy refers to all social networking sites, video/photo sharing sites, blogs, micro-blogs, wikis, podcasts, forums, instant messaging and geo-spatial tagging (for example, Facebook check-ins). Any comments or posts perceived to be obscene, defamatory, threatening, harassing, discriminatory or hateful towards Studio 3 staff, students or families may subject the owner to disciplinary action.
