Check-in and Attendance
• At school dismissal, children enrolled in CLUB 158 classes will be escorted to the small cafeteria by their classroom teacher(s).
• Our Club Supervisors will provide a snack and take attendance. Students are welcome to bring their own snack too. If you know your child will be absent, please email CLUB 158 in advance.
• Between 3:05 and 3:15pm, CLUB 158 instructors will escort the children from the small cafeteria to the classroom.
Dismissal and Pick-Up
• CLUB 158 Dismissal is 5:00pm.
• Extended Day Dismissal is by 5:45pm.
All CLUB 158 classes end at 5pm, at which time, the instructors will bring the children back to the cafeterias. If your child is enrolled in Extended Day, he/she will remain in the small cafeteria with the CLUB 158 supervisors.
Parents/Guardians should enter through the school's main entrance and go directly to the cafeteria to meet your child(ren) and sign them out with their instructor. Once you have your child(ren), please vacate the school immediately through the door in the small cafeteria.
We cannot release your child to anyone without parental consent. Please include all acceptable pick-up parties when registering your child. If the person picking your child up is not on that list, we CANNOT release your child to them. If you anticipate that a person that is not on your list of authorized parties will pick up your child, please notify CLUB 158 in writing. Please note, this person will be required to show a picture ID at pick-up.
You must pick your child up from the cafeteria immediately at dismissal. There is a grace period of 5 minutes, after which a late fee is charged. A fee will be charged for all late pick-ups: $15 for the first 6-15 minutes; $20 for each subsequent 15 minutes. Calling DOES NOT eliminate your obligation to pay this fee; although letting us know if you're going to be late is greatly appreciated.
All children must vacate the building by 6:00pm by order of the School Safety Department/City of NY, and any person in violation of these procedures will be suspended from CLUB 158 activities.
Please email CLUB 158 at email@example.com to report your child absent. We cannot refund for missed classes and no make-up classes are offered.

CLUB 158 is closed on the following dates: September- 30 October- 1,9,14 November -5,11,14,27,28,29 December-23,24,25,26,27,31 January-1,20
Please note: Club 158 is not in session on holidays, half days or days when school is closed due to inclement weather. CLUB 158 is unable to offer make-up classes or refunds for classes cancelled due to holidays, half days or inclement weather.
CLUB 158 fosters a harmonious environment where children feel safe.
Please review these guidelines with your child so they may enjoy the after-school program. All children should:
• respect others, their space and their property,
• cooperate with CLUB 158 Supervisors and Instructors,
• remain at their designated table during snack and while waiting for CLUB 158 instructors to take them to class,
• remain with their instructors during classes, until a parent/guardian signs them out at pick-up.
Unacceptable behaviors include using profanity, disrespecting others and their space, failure to comply with directions, excessive horseplay, and inappropriate or violent physical contact. CLUB 158 may report incidents of unacceptable behavior to the parents/guardians of all children involved. CLUB 158 reserves the right to remove a child from the program for unacceptable behavior.
As the parent or legal guardian of the child named below, I hereby give my full consent and approval for my child to participate in the CLUB 158 activity(ies) designated below.
I understand that there are certain risks of injury inherent in the practice and play of this activity(ies), as well as other related activities incidental to my child's participation, and I am willing to assume these risks on behalf of my child.
I hereby certify that my child is fully capable of participating in the designated activity, and that my child is healthy and has no physical or mental disabilities or infirmities that would restrict full participation in these activities, except as noted on the Registration Form.
In addition to giving my full consent for my child's participation, I do hereby waive, release, hold harmless and forever discharge CLUB 158 LLC, P.S. 158, the PS 158 Parents' Association and all its officers, coaches, sponsors, supervisors, employees, contractors, agents, and representatives for any injury or liability arising out of and/or based upon any bodily injury that may be suffered, sustained or caused by my child in the normal course of participation in the designated activities and the activities incidental thereto, whether the result of negligence or any other cause.
The CLUB 158 Program requires my child to meet certain standards of behavior and that if my child fails to behave or demonstrates unsatisfactory conduct, CLUB 158 reserves the right to dismiss my child with said inappropriate behavior without a refund. One written warning will be sent notifying the parent/guardian of an incident of disruptive and/or inappropriate behavior. A second incident will result in the child being removed from the class.
