Self-Wellness Check Agreement
Please ask yourself these questions and answer NO to all of them before coming to the studio. Have you had a cough? Have you had a fever? Have you been around anyone exhibiting these symptoms within the past 14 days? Are you living with anyone who is sick or quarantined?
Release of Liability Policies
As the legal parent or guardian, I release and hold harmless CK DanceWorks, Inc, its owners and operators, from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or in route to or from any of said premises. I understand that appropriate physical contact is required during the instruction of dance & acro and I give permission for instructors to make appropriate physical contact with me or my child for such instruction.
Risk & Waiver of Liability Relating to COVID-19
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending classes at CK DanceWorks and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed or infected by COVID-19 at CKDW may result from the actions, omissions, or negligence of myself and others, including but not limited to CK DanceWorks, Inc, employees, volunteers, students, and their families.
Tuition & Fees Policies
An annual non-refundable $30.00 registration fee per returning students ($50.00 family) is automatically charged to all 2020-21 fall registrations. Excluding current active students from the 2019-2020 season. An annual $40.00 registration fee per new student ($60.00 families). This fee is only charged once in a calendar year and is good through our summer session. This fee is non-refundable. CKDW reserves the right to place all students according to age, ability level, and dance/acro experience. Tuition fees for the season will be automatically charged to the card on file from August 1 and go through May 1.
We do offer a trial class before tuition is charged for new students who are unsure. In order to attend, you must complete the registration process by selecting a trial student, and pay the non-refundable registration fee. Tuition will not process until after the first class has been completed and the student remains in the class. The trial option period begins after August 3 and will not show as an option until after that date.
The dance season will consist of 10 months of classes with an avg. of (35-36) thirty-six weekly class sessions from August 3, 2020- May 29, 2021.
Tuition rates are $60.00 for a 45 minute classes, $67.50 for 1 hr classes, $75.00 for Combo (1.15 hr) class. All add-on classes are an additional $40.00 per class monthly. Monthly tuition is based on an average of 3.6 classes per month and is the same price for 10 months. Some months have as few as (3) three weeks others have as many as (5) five depending on our scheduled holiday closings.
We go into each account and manually fix the tuition when there are add-ons once we receive the registration.
Up to 3 make-up classes are allowed per student per season. These can now be scheduled in your parent portal or by emailing the studio.
There is an automatic class change fee of $5.00 per class change/transfer.
We consider Winter Break as the makeup week for inclement weather closings throughout the year. If any particular weekly class has more than one cancelation due to CKDW being closed we will schedule either a virtual or pre-recorded class as the makeup.
Payments are due on the (1st) first of each month, auto-billing is required, monthly. Unless you pre-pay BEFORE DUE DATE at the front office.
We gladly accept the following methods of payment:

Payments for tuition & other various items may be made online, at the office, or by mail. You can also pre-pay on account through the parent portal.
When making a payment by cash or check, please include the dancer's first and last name/s in the memo of your check so we may credit the proper account. We can accept cash & check payments as long as they are paid prior to the due date or our system will automatically charge your credit card on file on the date due if there is a balance.
Returned check fee is $25.00
You WILL NOT be receiving a statement from our office for your monthly tuition. You may check your account online at any time by logging into our parent portal.
Any account unpaid after the (15) fifteenth of the month, will have late charges assessed for $10.00 of the total monthly tuition.
If at any time you would like a printed statement of your account you may request one or login to the portal to view & print.
Overdue Balances Only: A statement will be mailed and/or email for the amount due with late charges assessed.
Any student with an overdue balance of more than 60 days will not be allowed into class until payment in full is received or payment arrangements have been made.
We hate to see you go but we realize that everyone will eventually grow up and/or move on. As a courtesy to our studio, we require parents to give written notice either by email or the withdrawal form on the website before the end of the month if their child is going to discontinue classes at CKDW. This is so we can invite the next child on the waiting list to join the class the following month. Please Note: If the proper notice is not given, you will be billed for the additional tuition that your child is enrolled in the class/classes. AGAIN, You must cancel/withdraw in writing by email or by the withdraw form found on the website BEFORE the 1st of the month or you are responsible to PAY for that month and any other month that passes until we receive written notice. We do not accept VERBAL withdrawals.
Refunds/Credits Policies
CKDW reserves the right to cancel any camp or class with insufficient registration and a full refund will be given.
If a very young student is not ready to participate in a class after several visits, a refund of tuition only (not used) will be given.
Camp fees are NON-REFUNDABLE but are transferable.
When a student sustains an injury that does not permit them to participate over an extended period of time a credit or refund will be issued for that point forward.
Camp Fees, Registration Fees & Recital Fees are non-refundable!
Credits will not be given for missed classes due to illness, snow days, or personal commitments. If your child misses class due to illness or other personal reasons, they will be able to make up in a similar class up to 3 allowed per dance season.
Year End Recital Policies
We produce year-end recitals & showcases at the end of each season with all dancers participating. This is an exciting end to the dance season! The 2021 Recitals are held at the end of each season. REC Recital Week is May 17-22, 2021 at The Grand Theatre in Cartersville. This is an exciting and rewarding part of the season that everyone looks forward to and we encourage all dancers to participate.
Recital Costume Fee: Costumes must be paid for in full by December 20 of each season. We divide the total over 3 months due October 20, November 20, and December 20. Costume payments are non-refundable. If your costume fees are not paid for in full before we close for Christmas break, they will not be ordered. Any alterations needed will be the responsibility of the parent.
Recital fees for all classes are $152.00 and include a costume, tights, accessories, a keepsake trophy, a recital USB video & the participation fee (per dancer). All combo classes will have only one costume to help keep it more affordable. Additional class costumes are $80 per class.
Recital Tickets: All tickets are for purchase and go on sale on our website. Dancers do not need a ticket.
Medical Emergency Policies
As the legal parent or guardian, I give permission to CKDW, its owners, and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or conditions and/or declare the participant to be in good physical and mental health.
Photo Release & Agreement
I give permission for CKDW to take photos and or videos of CKDW classes, rehearsals and/or performances, including dancers of CKDW in costumes of me or my child while participating in CKDW activities for promotional purposes. Names of students will not be used or disclosed. I understand that I am giving permission to use these photos and/or videos for publishing use, in printed or electronic format.
Season Calendar & Holiday Closings
CK's season begins Saturday, August 3, 2020, and ends Saturday, May 29, 2021. CKDW follows the Paulding County School Systems closings for the following closings: Labor Day Weekend, Fall Break, Thanksgiving Break, Christmas Break, and Spring Break.
We DO NOT CLOSE for MLK Day, other teacher workdays, or Winter Break (in February).

General Studio Rules
*Absolutely NO food or drinks in the dance rooms. Water only, please.
*No cell phones allowed in the dance rooms at any time. If they are in your bag, they should be turned on silent.
*CKDW is a large facility, so please keep an eye on your children, no running or climbing on furniture/benches, please. Young children should always be supervised outside the building and on the CK property.
*All children must be accompanied by a parent/guardian at all times unless they are in class.
*All dancers personal belongings may be kept in the locker room or in the dance rooms with the dancer if needed. We are not responsible for items left in the building. Please label ALL of your belongings esp. dancer shoes.
*Pre-School & Tot Rock moms-please take your little dancer to the restroom before class begins to avoid accidents and distractions.
* Please make an effort to be prompt as this causes distractions to the other students in the class. We cannot be responsible for your child after class. Please call the office if you are running late and need us to keep an eye on your child.
*No parents, friends, or siblings in the classroom at any time. This is for the benefit of your dancer. They will learn more if we have their complete attention.
*Do not for any reason tap or knock on the viewing windows. Please see the office staff for any problems you may have.
*We generally follow Paulding County's holiday schedule. However, we DO NOT close on teacher workdays, MLK, Pres. Day. Check the calendar for specifics.
*We follow Paulding County's decision on inclement weather school closings but in some cases, we remain open if road conditions are suitable.
*Our staff is always available during office hours to answer any questions you might have. If you need to speak with your child's instructor, please call or email us to set up an appointment as we rarely have time between classes.
*Studio COVID-19 RULES
Our lobby and outdoor common areas including picnic tables are closed at this time! Only students and staff are allowed inside the building. All students and staff will be screened upon entrance. All family members will need to remain in their cars or social distancing outside. Students are not allowed to bring any belongings except their dance shoes, dance bags. in their hands and a water -bottle. Students must wear shoes at all times. Hand sanitizer is available at the front desk to use when entering and exiting the building. Our water fountains will be closed so please remember to bring a water bottle.
