Release of Liability
I release Youth Link Southern Indiana (YLSI) and individuals from liability in case of accident during activities related to YLSI, as long as normal safety procedures have been taken.
I give my child permission to go on field trips and release YLSI and individuals from any liability in case of accident during field trips, as long as normal safety procedures have been taken.
Permission
I give YLSI permission to have access to information regarding my child's report card, test scores, demographic information, student ID #, IEP's, and any other information needed for YLSI report forms.
Medical Emergencies
In the event that I cannot be reached in an emergency. I hereby give permission to the director of the program or the designee to secure emergency medical services, including transportation and a physician. I also give permission to the attending physician to order injection, anesthesia or surgery for my child as named above. Medical and accident insurance is the responsibility of the parent or guardian.
CISCC Discipline/Guidance Policy
By checking this box, I confirm that I have read the YLSI Discipline/Guidance Policy stated below and understand that my student may be removed from the program due to behavior as outlined in the policy:
To maintain a safe and fun environment for all students, each EDL site has a positive-reinforcement behavior management system in place. Hitting, kicking, spitting, hostile verbal behavior and other disruptive behaviors are not permitted. In response to these behaviors, Youth Link SI will NOT use threats or bribes, physical punishment, deprivation of food or other basic needs, humiliation, or isolation. Youth Link SI will respect your child, establish clear rules, be consistent in enforcing rules, use positive language to explain desired behavior, speak calmly, give clear choices, redirect your child to a new activity, or move your child to a time-out chair for no longer than one minute per year of your child's age. If your child's behavior is very disruptive or harmful to themselves or other children, Youth Link SI will discuss the issue with you privately. If we are unable to resolve the issue, you will be asked to make other childcare arrangements.
Policy Pick Up
By checking this box, I understand that the EDL program closes at 6:00 p.m. I am aware that at 6:15 p.m. parents/legal guardians will be charged $1 per minute until the child is picked up. At 6:45 p.m. the police will be called to pick up the child.
Photo Release
I give permission for YLSI to use my child's photograph and/or work in any media format. If I DO NOT give permission, I understand that I must send in writing via email to edl@youthlinksi.org or by mail to 1740 Williamsburg Dr., Suite G, Jeffersonville IN 47130 that YLSI is not to use my child's photograph and/or work in any media format.
Payment Policies
By entering my checking account/credit card information, I am authorizing YLSI to charge my account for tuition owed.
I understand that I am paying for my child's spot in the EDL program. I will be billed according to which component of the program my child is enrolled in (i.e.: before care, after care, full-time, or part-time) and NOT the actual number of days attended.
I understand that I must submit in writing via email using the "Contact Us" tab on the Parent Portal or (edl@youthlinksi.org) or via mail (1740 Williamsburg Dr., Suite E, Jeffersonville IN 47130) 7 days prior to unenrolling my student. Failure to do so will result in a charge of one week's tuition.
I understand that the parent portal reflects the current balance owed and is due Monday of each week.
I further understand that failure to make the proper weekly tuition payments on time can result in my child losing their spot in the EDL Program.
