Release of Liability
As the legal parent or guardian, I release and hold harmless Bourne 2 Dance, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Bourne 2 Dance, its owners and operators or in route to or from any of said premises.
Medical Emergency
The undersigned gives permission to Bourne 2 Dance, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health. Payment Information
Tuition fees is due in the first 2 weeks of each term. If accounts are paid after week 2 there will be a $15.00 late fee applied to the account balance. All fees are payable in advance and are non-refundable. No credit is given for missed lessons however missed lessons can be made up if a similar class has vacancies available. In the event where your overdue account is referred to a collection agency and/or law firm, you will be liable for all costs which would be incurred as if the debt is collected in full, including legal demand costs. Lessons will be terminated for those students that have fees past overdue.
Correspondence
I understand that Bourne 2 Dance sends out all correspondence via email, updates on facebook as well as the Bourne 2 Dance website and it is my responsibility to read and take note of any important dates and events.
Annual Concert & Costumes
I understand that all Bourne 2 Dance students who register before the start of term 3 are assumed to be participating in the end of year concert unless notified otherwise in writing. I understand that a costume fee will be added to my term 3 invoice for the end of year concert and that any student not wishing to participate in the end of year concert needs to inform Bourne 2 Dance in writing before the due date of term 3 fees. I understand that if I fail to notify Bourne 2 Dance in writing before the due date of term 3 fees I am still liable to pay the costume fee for my child's costume. I understand that my account needs to be paid off in full with no amount outstanding before I can take my child's costume home.
Photography
I understand that my child may be in photos or video footage taken at Bourne 2 Dance and they may be used on social media & other advertising avenues. I understand that I need to give written notice to Bourne 2 Dance if I do not want my child to be in any photos or video footage.
Pick up and Drop Off Policy
I understand that it is my responsibility to make sure the child participant is picked up and dropped off from inside the venue to ensure the safety of the child participant. I hereby agree that my child participant/s must wait inside the venue for myself and/or a authorised person to arrive to pick them up. I agree that no child participant will be allowed to exit without the presence of their Parents or authorised persons.
Right of Admission
I understand that Bourne 2 Dance reserves the right of admission.
Overall
I understand that by attending class & paying my fees I agree to all of the above.
