Summer Registration Terms
Summer Only: By submitting your registration for summer dance classes, you agree to pay tuition in full with your registration.
There is no registration fee. Registrations are considered final upon submission and you accept responsibility for the payment of that submission. If you wish to drop the class, you must contact the studio within one week of your registration and submit a drop fee of $10.00 per student to complete the drop. You will be held responsible for the full amount of tuition, regardless of class attendance, if you do not notify us within one week of your registration.
Select automatic payments to have your tuition automatically drawn on July 6.
Lake Shore Dance reserves the right to cancel, condense, or divide classes at anytime for any reason.
Summer tuition payments are not refundable.
Choosing Classes for Your Child
The program at Lake Shore Dance is unique and exceptional because of our graded curriculum that tracks your dancer's progression. Each children's class covers material that is appropriate for your child's age and development (based on early education standards). Therefore, you can confidently select your child's class by age.
Summer: Ages are determined as of July 1 for new students.
Fall: Ages are determined as of September 15 for new students.
If you will be registering a student that is age 8 or older, and is new to Lake Shore Dance, please contact the assistant director to set up a trial class for placement recommendations.
If you are already a member of our Lake Shore Dance Family, please choose classes based on your child's progress and placement report. It is important to follow the progression of the program because the program is progressive and important technical lessons will be missed if a level is skipped.
Regardless of the classes you choose, you are guaranteed only the best from Lake Shore Dance!
Our Classroom Experience
Lake Shore Dance has a long-standing, solid reputation in the local and regional dance community because of our commitment to excellence in artistry and well-rounded training system grounded in correct technique. All of our children start dancing because it is a wonderful activity for small children - but someday, they may decide that dance is the activity they want to focus on. With that in mind, we are committed to giving all of our students the best dance foundation that progresses as they grow.
Dance is a discipline with a rich history that we are excited to share with your family. Like most disciplines, success and progression takes time, patience, and practice. You will see your child grow so much this season, not only as a dancer, but also in their confidence! Help them to be their best by making sure your child is in class regularly and encourage your child to practice at home and show you what they have learned.
Ballet is the foundation for all dance training. Ballet teaches alignment, muscular control, and fundamental dance vocabulary and skills. For your child's success, we highly recommend that every child takes ballet class. Ballet for Level V and higher meets twice per week.
Pointe class is an extension of Ballet and can only be taken with the successful completion of Pre-Pointe, doctor's authorization, and two days of ballet each week. Girls on pointe will take minimally one ballet class on pointe per week during full curricular sessions.
Tap class is included in many of our children's classes. As one of the core dance genres, tap dancing is essential to developing rhythmic and musical skills. Tap also helps your child learn and recognize patterning while developing logic and memory skills that carry into their school-work.
We may make alternate placement recommendations for your child once we have had the opportunity to work with your dancer for a couple of weeks. We will have a conversation about our recommendations if we feel that the child should be in a different class.
Please allow time for your small child to adjust to their new class. Our faculty is skilled in working with small children and acclimating them into the new classroom experience. If at any time you have a concern, please feel free to schedule a time to talk with the instructor. We work hard to help your child build confidence and independence. Most children take class well by themselves. We encourage you to assist your child in taking class independently and help them to feel pride in their accomplishments!
We reserve the right to change instructors, utilize substitute teachers, cancel or combine classes, and change class times. Many of our instructors are also performers and parents, so we appreciate your understanding if, from time to time, your child's class has a substitute teacher.
We are in Wisconsin and as we all know; winter can be unpredictable around here! In the event of Winter Storm Warnings, Blizzard Warnings, or other extreme weather conditions, we may be forced to cancel classes for the safety of our students and our staff. Please remember that many of our older dancers drive and several faculty members live outside of our communities. While situations may not appear to be too severe, we do consider the conditions of the surrounding areas and the probability of the intensification of the storm through the evening when make the decision to cancel. We will post on our Facebook Page and announce in email and on our group pages if we must cancel.

Lake Shore Dance proudly offers a full range of classes in many different dance genres. Several children's classes have more than one offering each week. Take your child's training further by selecting multiple classes! Exceptional dancers are built through repetition and training - your dancer will excel exponentially the more time they spend in dance class at Lake Shore Dance.
We also offer additional training opportunities to build technical and performing skills. In addition to regular group classes, dancers can elect to enroll in private instruction, competitive dance (with an audition), and performance groups. To take advantage of these opportunities, stop by the front desk and ask for more information.

• Family Discounts: Lake Shore Dance has something for everyone in your family! Family members must be from the same household and be registered on the same account to qualify. The family discount is removed if enrollment drops down to one student. *Family discounts do not apply to private lessons, master classes, special events, costume or boutique purchases, company, or competition activities.
2 enrolled family members: 10%
3 enrolled family members: 15%
4 enrolled family members: 20%
Multiple Class Discounts:
Your student progresses faster with consistent training in multiple techniques. We encourage dancers to take advantage of our wide range of classes in our curriculum by offering discounting on multiple classes. These rates are per student.
2-3 classes per student: 5% off the full tuition pricing
4-5 classes per student: 10% off the full tuition pricing
Unlimited Annual Membership Programs:
Due to the Pandemic, these programs will not be offered this season.
Communications
Communication is key to a successful experience at Lake Shore Dance. Your active participation in receiving and reading communications will enhance your experience and greatly diminish uncertainties in your program participation. Please review the following points regarding communication:
1. Email is used to communicate frequently at Lake Shore Dance. Please ensure that lakeshorereception@gmail.com is in your address book. This email is not monitored and is an unresponsive account. Do not reply to these emails. It is imperative that you open and read emails as you receive them.
2. TeamApp is a web based and mobile based application that provides constant access to studio documents, calendars, and dates
3. Please respect our employees privacy. It is not acceptable to message our faculty, staff, guest artists, or receptionists via their private social media accounts, nor to request or use any Lake Shore Dance's employees personal phone numbers. You may contact our team through TeamApp via the chat feature and by their studio emails which will be communicated to you in the welcome emails.
4. Online newsletters and announcements are posted on the Parent Portal. The Parent Portal is your one stop resource to access your account, read the newsletter, pay your tuition, view your child's schedule and more. In your welcome emails, a link to the portal is provided.
Payment Terms
• Tuition is due with your registration.
Lake Shore Dance offers and prefers electronic payments. You can manage your payment account securely through our online portal.
• Checks and cash payments are accepted during staffed office hours only. We are not responsible for payments left on an unattended desk.
• Select "Auto-payment" as your payment when you register - no more worries about missing a payment and getting a late fee! By selecting "Auto-payment", you agree to allow Lake Shore Dance to pull your tuition installments each month on the first or next business day. You agree to keep your method of payment up to date via the customer portal.
o If you select auto-payment, all billed items on your account will be pulled on the due date, including tuition, private lessons, costumes (except competition costumes), and company/competition tuition.
o If you pay your bill manually via the portal or with a receptionist and/or your account has a zero balance, the system will not pull a payment.
o If your auto-payment is not successful for any reason, you will receive an email to inform you that your payment was not successful. You must then submit your payment manually and update your information within 5 days to avoid a late fee.
*Late fees of $20 will be assessed to all accounts that are 5 or more days past due. This fee cannot be reversed. 30 Day delinquent accounts will be sent to collections and your child will be automatically dropped from their class.
FALL/FULL TERM ONLY: Your first installment is due with your registration. A $10.00 service fee will be charged if the tuition payment is not made with your registration. If you choose automatic payments, this fee will not be charged . Automatic payments will be drawn between the 1st and 5th of each month.
Cancelling your registration
You may cancel your registration anytime by emailing accountmanagement@lakeshoredance.net. You will be assessed a $10.00 cancellation fee per class and any paid tuition is non-refundable. All tuition amounts unpaid at the time of the cancellation will be due with the cancellation fee. Please note that non-attendance does not constitute dropping the class and you will be held responsible for the tuition for the class as we are holding your spot in that class.
You are responsible for payment of pro-rated tuition of the total number of classes from the date of registration through the date of the cancellation, regardless of your child's attendance.
Cancellations are not considered final without notification to accountmanagement@lakeshoredance.net and payment of the cancellation fee.
We reserve the right to cancel registrations at any time.
All classes are subject to a minimum number of students. If the minimum number of students is not met by the 6th week of classes (full term) or as determined by the studio, management will work with the enrolled students to find a new class time.
Fall/Spring Registration Terms
Full Term Registration (September - May): Completing and submitting this registration form confirms and finalizes your child's registration in the class(es) you have selected. The tuition rate listed with each class is due per month for a total of 8 installments. The first tuition installment is due with your registration. The next 7 installments are due October 1, November 1, January 1, February 1, March 1, April 1, and May 1. A payment is not due in December.
As an installment plan, the monthly fee does not necessarily cover the classes taken in a particular month. Some months have more classes than others, but you don't pay more those months.
• All registrations are final. "Registration" is defined as pressing the "Submit Registration Information" when you register online or by submitting your registration either by mail or in person.
• Upon submitting your registration, you are bound to the terms of this registration agreement for the full session including the responsibility for every tuition installment for the session you have selected. An electronic copy with a time stamp and your electronic signature are saved on your account with the studio for your reference.
• All payments are non-refundable, including but not exclusively: tuition, costume fees, and studio purchases.
Mini-Session: Throughout the year, Lake Shore Dance offers mini-sessions - 6 to 8 week specialty sessions or introductory sessions of classes. In selecting a mini-session, you register for the full term of the mini-session and tuition for that mini-session is due in full with registration. This tuition is non-refundable.
Lake Shore Dance reserves the right to cancel, condense, or divide classes at anytime for any reason.
Spring Show (Fall Registration Only)
THIS SECTION PERTAINS TO FALL DANCE FULL TERM REGISTRATIONS ONLY
Dance is a performing art. Learning how to prepare choreography and present that choreography on stage brings the lessons in regular class full circle. We offer a spring concert open to all of our dancers at the conclusion of each season. This experience is exhilarating and essential to learning to dance. You will be amazed by the confidence and grace in your child when you see them onstage!
COSTUMING FOR THE SPRING SHOW
To make the performance experience truly special for our dancers, many of our classes are professionally costumed. All costume purchases include a free pair of tights to wear with All children's classes and all ballet and jazz classes will be assessed a costume fee for the spring show. The costumes will be ordered on October 15. Costumes will only be ordered if the costume deposit fee is paid. We will measure for costumes during the first few classes sessions and you will receive a form to sign with our suggested costume size. Non-refundable deposits (50%) for the costumes are due October 15 and balances are due January 15, 2020. A second order will be placed on December 21. These costumes will arrive later than costumes that were ordered with the first order on October 15. All costumes must be paid in full and accounts current for costumes to be distributed.
Children's costumes are $60.00 plus tax and include a brand new pair of tights for the show.
Dancers in ballet and jazz will be assessed a costume fee ranging from $65.00 to $85.00 per technique. The exact amount will be disclosed shortly and each costume also receives a free pair of tights.
The costumes belong to you. Costumes will only be ordered once the deposit is received and only if your account is current. Costumes will only be delivered to your student if your account is current.
Costume payments are non-refundable.
Virtual and E-learning Class Agreement
Safe Participation Guidelines
In participating in virtual classes offered by Lake Shore Dance and our faculty, whether live interaction or pre-recorded, please observe the following guidelines:
1. No pointe shoes at home unless directed by faculty.
2. Create a safe space to dance in - move breakable objects and furniture with sharp corners out of the dancing space. Be sure that your floor is level and free of debris prior to starting.
3. Dress comfortably - unless otherwise directed, Leos and tights are not required, but you should dress in exercise clothes that allow for free movement.
4. Footwear - footwear is optional for all classes - please choose footwear that will keep you from slipping.
5. Warm-up and cool-down. Many of our instructional videos will take you through the warm-up process and some may have a cool-down. As with participation in any physical activity, pre-warming up by walking or other simple cardio movement is recommended.
6. Level Awareness: Select classes that fall within your skill level whenever possible. Consider your limitations when learning new material and adapt and adjust your execution based on those limitations.
7. Take breaks when you feel it necessary.
8. Hydrate! Just like at the studio, drink clear water before, during and after your class.
Getting the Most out of Your Class
1. Stay open minded! Every moment of dancing is valuable, precious and builds your skills as a dancer, whether the class is easier than your current level or harder. Absorb the words of wisdom from every teacher and apply those words to your dancing.
2. Keep trying! Struggling and persevering is part of the process and makes you better, building both physical skills and intellectual skills.
3. Have everything you need prepared before the class begins (if it is live) and pre-register for the format (IE Zoom, Google Hangout) when required so you don't miss class.
4. Just like at the studio, arrive early for class!
5. Take classes outside your "favorite" technique - We all have our "favorite " technique. But your skills in that favorite technique will only be enhanced and broadened through exploring different techniques. Challenge yourself to classes outside your comfort zone and watch yourself grow!
Permissions and Ownership
Choreography, class material, and all related content is property of Lake Shore Dance and its faculty and staff. Viewing and participation is granted to enrolled clients only . Public viewing or distribution is not granted with access to this media.
Waivers and Registration Agreement
By selecting and opting to participate in this virtual learning format, I release Lake Shore Dance, its owner, operators, faculty, and staff, from loss or injury resulting from participation. I accept the permissions and ownership guidelines stated in this contract and maintain the exclusivity of the product to myself and immediate family. I understand that participation in any physical activities include a certain degree of risk and agree to take necessary safety precautions while taking this class in my own space.
Health and Safety
All clients are encouraged to maintain good personal hygiene habits, to clean up after themselves while containing their own personal effects while on the premises, and to make decisions regarding involvement in live interactions with their own health considerations and best interests in mind. Any one that is ill or battling a fever may not be at the studio during that illness and a minimum of 48 hours after the illness is resolved.
By participating and accepting the option of returning to the studio with an instructor, you accept the following terms and release Lake Shore Dance, LLC, its faculty, its staff, and the owners of the buildings, of loss, of injury, and of illness resulting from any activity or interaction at the facilities or events.
This list is not a full list of disclosures, rather a broad framing of the terms you accept by participating:
-Upon entering and exiting the studio, please use the provided hand sanitizer at the doors.
-We are responsible for our own health and decisions regarding our health and will take precautions within the studio to protect ourselves and those around us from infection or transmission.
-We may choose to use PPE or choose to forgo PPE measures, and agree that transmission, infections, or other side effects from choosing to use PPE or choosing not to use PPE are completely our responsibility and we will not hold Lake Shore Dance, LLC, its employees, owners, or other clientele responsible.
-We believe in your child's and your family's privacy and will not request that you disclose any conditions barring your child from wearing a face covering and expect the same regarding our faculty and staff.
Our classes are very physical and we need you to help your child understand that if they are feeling light headed, struggling to breathe, or experiencing other unusual feelings that could lead to passing out or other urgent reactions, that they should take off their masks and hydrate themselves.
We are all aware that our communities and peers have different personal opinions about this order and regarding facial coverings in general. It is imperative that for the emotional health of our dance family that the facility remains a "conflict free" zone. Our students and faculty deserve a peaceful place where they can focus on doing what they love without the added stress of conflict. At no time should any person be approached, questioned, or demeaned for their face covering or lack thereof. We do not know what another person's situation is and every person deserves respect and dignity. We appreciate that you stand with us to maintain a conflict free and therefore stress free atmosphere for our children.
-We will not be at the studio or send our child to the studio with a fever or illness, nor will we attempt to cover up signs of illness with medication. We will make sure our child or ourselves are no longer ill for a period of minimum 48 hours prior to returning to the studio.
-We will not congregate unnecessarily in or around the studios, particularly during the height of the Pandemic, but additionally during cold and flu season to protect ourselves and the general studio population.
-I agree that this waiver is in addition to all previous agreements that I have electronically and physically signed with Lake Shore Dance, LLC for this and all previous dance sessions.
- I agree to the additional terms outlined in this disclosure and assume any and all risk and possible outcomes of returning to the classes, releasing Lake Shore Dance, LLC, its owners, employees, and clientele, of loss, injury or other effects. I agree to abide by the terms outlined by Lake Shore Dance, LLC to return to class and understand that we will make decisions regarding our return in the best interest of our own health and the health of others.
Social Media and Usage of Personal Devices
Many students now have access to social media and the web via their personal devices. With this access comes responsibility both on the part of the child with the device and the parents. Students that choose to bring a device to the studio are responsible for their behaviors and their devices. Lake Shore Dance is not liable for loss or liability to devices brought to the studio, nor does Lake Shore Dance accept responsibility for the actions taken or postings on Social Media while at the studio nor interactions beteween students or parents at the studio or outside the studio.
Cyber bullying will not be tolerated. Talk with your child about appropriate interactions and managing relationships.
Students may not take video or photos of classmates or other studio members without their consent and may not post this material without all parties' consent. Posting videos of classes or choreography is prohibited without permission from Lake Shore Dance, LLC to protect our educators intellectual property and the privacy of our clientele.
To protect our children, students must turn off their locations on apps such as "Snapchat" and "Instagram" while at the studio or studio events.
While at the studio and particularly while in class, students must have their phone on silent and refrain from checking their devices until they are completely done with classes. We reserve the right to restrict usage of devices at the studio based on activity.
We appreciate your help in protecting the children at Lake Shore Dance and assisting us in creating a respectful and safe environment for all.
Release and Waivers
As the legal parent or guardian of the registrant, I release and hold harmless Lake Shore Dance, its owners, employees, and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of the owners, staff, faculty, and operators or in route to or from any of said premises. I further acknowledge that physical activity has inherent risks and accept responsibility for my child's participation in lessons at Lake Shore Dance and will not hold Lake Shore Dance, its' faculty, staff, and owners, as well as other associated parties responsible for injury or loss incurred through participation in dance lessons. I have noted any medical or special needs that my child has that could potentially affect their performance in dance lessons. I give permission to Lake Shore Dance to contact medical professionals on behalf of my child in the case of emergency if I cannot be reached.
If I sign up for a membership program, I agree to the terms outlined in the membership program document and authorize Lake Shore Dance to pull my payments automatically.
I agree to join the studio Teamapp and take personal responsibility to stay informed on studio events. By signing below, I agree to terms in the registration agreement. I understand that tuition and costume fees are non-refundable and that by submitting my registration, I accept the responsibility for tuition payment. I accept and understand the cancellation policy. I understand that cash refunds will not be issued at any time. I understand that I am responsible for payment of tuition for the entire dance session, regardless of attendance, absences, or cancellations. I further acknowledge that all information is subject to change and I will be informed of said changes via Lake Shore Dance communication portals.
I agree to allow my child to be photographed and/or video taped for promotional purposes. If I wish to revoke this permission, I agree to sign a slip at the studio.
As dance is a physical activity, our faculty will need to use tactile corrections to build your dancer. If you have concerns about this specific method of teaching, please contact your studio assistant director.
I agree to bring my dancer to class regularly and I acknowledge and accept that the dancer's progression is dependent on regular class attendance. Furthermore, I agree to inform the studio via email or phone call if my child will be absent, agree to keep my child out of class if they are ill (highly contagious illnesses ie strep throat, vomiting, diarrhea, lice, hand/foot/mouth, high fever etc), and accept that tuition credits or refunds will not be issued. Dancers may make up class within two weeks of an absence if scheduled through the studio and space is available. I take full responsibility for my child's presence at the studio; agreeing to drop off my child no more than 10 minutes to the start of their class, agree to arrange pick up for my child within 5 minutes of the end of class, will instruct my child to wait inside until I pick them, and will instruct my child never to leave the facility without supervision.
This agreement is signed electronically and time stamped for authenticity. A copy of the agreement is available on your portal and is saved internally for reference.
