In consideration of allowing the previously-declared participant(s) to begin participation in CSA Kids activities, while on the premises and property of said Campus, the undersigned, for themselves, and/or being the legal and acting guardian of participant, acting for themselves and on behalf of the participant, release and hold harmless Creative School of Arts, Inc., d/b/a CSA Kids "“ Sports & Arts Campus its owners, officers, employees, and agents of and from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises upon which CSA Kids is conducted, or any premises under the control and supervision of CSA Kids, its owners, officers, employees, or agents or in route to or from any of said premises, or while at any premises or place when activities sponsored by or participated in by CSA Kids, its owners, officers, agents, or employees.
Participation in physical activities can involve motion, rotation, and height in a unique environment and as such carries with it a certain assumption of risk. The undersigned and the participant(s) choose to voluntarily enter upon said premises under the control of said corporation, knowing their present condition and knowing that said condition may become more hazardous and dangerous during the time the participant or the undersigned is upon said premises. The undersigned and the participant(s) voluntarily assume any and all risks of loss, damage, or injury that may be sustained by the participant(s) and/or the undersigned or any property owner by them while on or upon said premises described above. The corporation may but shall not be obliged to carry insurance on the participant(s), and the existence of insurance shall not change, alter, or increase the liability of the corporation to the participant and the undersigned or affect the terms of this Release. In signing this Release, the undersigned acknowledges:
a) That he/she has read thoroughly and understands completely, the terms of Registration and Release and signs it voluntarily.
b) That the undersigned, signing either for themselves or as Legal Guardian, is in fact, the true and legal guardian and has the consent of the participant.
There is an annual Registration fee due at the time of registration and annually thereafter. Monthly tuition is paid in full upon enrollment. Student is assumed to continue enrollment until at which time CSA Kids receives a written 30-day termination request (written form available at our front desk). The termination must be received on/before the first day of the month PRIOR to the month the termination is effective. CSA Kids will obtain your credit card, debit card, or checking information at the time of registration. Auto payments will be recovered by Automatic Check Withdrawal (ACW), Automatic Debit Card Withdrawal (ADC), or Automatic Credit Card (ACC). There is a $35.00 returned check charge for any checks returned by the bank for any reason. Missing, Transferring & Withdrawal from Class: You are liable for the full tuition from your date of enrollment, regardless of class type or payment method; there are no make-ups for missed classes. Transferring classes within the same program is possible as long as there is a vacancy in the class you desire; transfers will only be allowed at the beginning of a month. There are no refunds or credits for dropping a class or for changing from one program to another mid-month. ONCE ENROLLED, YOU ARE IN CLASS FOR THE ENTIRE MONTH regardless of attendance, except for Medical Reasons. Medical Reasons for Dropping a Class: For medical situations validated by a written acknowledgment from a licensed medical practitioner, you will be granted a tuition credit prorated from THE DATE WE RECEIVE THE WRITTEN ACKNOWLEDGEMENT. A retroactive request for medical credit cannot be granted, as it eliminates our opportunity to fill your vacated class spot. GYMNASTICS TEAM FAMILIES: If you choose to pay your Competition Fee's with cash, check or ACW you will receive a 3% discount. Your Competition Fee payment will be automatically deducted from your checking account or billed to your credit card account if the payment is not made by the 14th of the month. You may of course pay for the Competition Fee's in full at any time. Gymnasts will not be entered in meets if your competition fees are not current.
