Withdrawal Policies
Summer Programs:
1. Withdrawal requests must be received in writing two weeks prior to the start of the program. A $30.00 Deposit Non-Refundable Deposit will be retained.
2. In the unfortunate case that a family member should become sick and a student is unable to attend the camp in person, the camp will be accessible through a virtual platform and a craft kit and prop kit will be delivered to your home to make camp accessible for you.
3. To ensure the health and safety of our students and faculty, Crossings reserves the right to move classes to a virtual platform in the event of a global pandemic such as COVID-19 without adjusting or refunding tuition.
Sessional Programs:

2. Only Sessions that have not yet begun will be refundable for Full Year Bundles. Requests must be received in writing two weeks prior to the start of the new session. A $25.00 administration fee will be retained.
3. Registration Fee + gst ($30 student or $50 per family) is required upon registration and is non-refundable.
4. Costume Fees(s) + Performance Fee + gst are non-refundable (Youth Program Only). *Students returning from the 2019/20 Dance Season may apply the full amount of 2019/20 Costume Fees as credit towards 2020/21 Costume Fees.
Full Year Programs:
1. Registration Fee + gst ($30 student or $50 per family) is required upon registration and is non-refundable.
2. Costume Fees(s) + Performance Fee + gst are non-refundable (Session 3 and Full Year Bundle). *Students returning from the 2019/20 Dance Season may apply the full amount of 2019/20 Costume Fees as credit towards 2020/21 Costume Fees.
3. The first month's tuition is also non-refundable.
4. Withdrawals require one calendar month's written notice and must be received prior to the first day of the month. Your final balance will be calculated by pro-rating classes (based on full, non-discounted rates) to the end of the one-month notice period. Any fees submitted to cover classes after the one-month notice period will be returned.
5. There are no refunds after January 15 , 2021.
General Policies
1. Crossings Dance reserves the right to reschedule, cancel, or combine classes should registration be insufficient.

Release of Liability
1. For purposes such as education and promotion of Crossings Dance Ministries Inc. and its programs, photographs and video may be taken of students and used in school publications, on our website, on our blog, on Facebook, in occasional news releases, and in other promotional mediums. Crossings Dance Ministries Inc., as applicable, is not required to notify students or their parents/guardians, solicit their approval, or compensate them in any way prior to using such photographs or video.
2. In order to teach and correct dance movement and technique, physical contact between the student and teacher is necessary. Crossings Dance Ministries Inc. undertakes to ensure that such contact is applied in a professional manner and is required for dance instruction. This contact, as deemed necessary by the teacher, is consented to by the student and/or parent/guardian. *Crossings staff will adhere to the recommendations of AHS and government regulations concerning physical contact, in case of a global pandemic or similar circumstance. Alternative teaching methods will be used in such instances.
3. Crossings Dance Ministries Inc. and its Directors and Employees will not assume liability for any lost or stolen property, for any bodily or personal injury consisting of or arising out of a student participating in any physical training (dance or fitness), athletic activity or contest. Crossings Dance Ministries Inc. is not responsible for loss, damage or injury to any person or property while on the site of Crossings Dance Ministries Inc., traveling to or from the site of Crossings Dance Ministries Inc., or while participating in an off-site affiliated performance, activity, or contest.
4. Consent and Assumption of Risk: Being fully aware of these dancers, I hereby give consent for my child(ren) to participate in any and all CDM programs and activities for which they are registered and I ACCEPT ALL RISKS associated with participation.
Privacy Policy
Your personally identifiable information is kept secure. Only authorized employees have access to this information. Crossings will never share your personal information with any other person, company or organization.
Medical Emergency
The undersigned gives permission to Crossings Dance Ministries Inc., its owners, and staff members to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or conditions and/or declare the participant to be in good physical and mental health.
Communicable Diseases
WAIVER/RELEASE FOR COMMUNICABLE DISEASES INCLUDING COVID-19
Crossings Dance Ministries Inc. (CDM) has put in place preventative measures to reduce the spread of COVID-19; however, CDM cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending CDM could increase your risk and your child(ren)'s risk of contracting COVID-19.
ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENT
In consideration of being allowed to participate on behalf of Crossings Dance Ministries Inc. (CDM) dance or fitness program and related events and activities, the undersigned acknowledges, appreciates, and agrees that:
1. Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,
2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,
3. I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,
4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS (Crossings Dance Ministries Inc.) their officers, officials, agents, and/or employees, contractors, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("RELEASEES"), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IF FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
FOR PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT THE TIME OF REGISTRATION)
This is to certify that I, as parent/guardian, with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of presence and participation and his/her personal responsibilities for adhering to the rules and regulations for protection against communicable diseases. Furthermore, my child/ward understands and accepts these risks and responsibilities. I for myself, my spouse, and child/ward do consent and agree to his/her release provided above for all the Releasees and myself, my spouse, and child/ward do release and agree to indemnify and hold harmless the Releasees for any and all liabilities incident to my minor child's/ward's presence or participation in these activities as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent provided by law.
Payment Policy
1. Registration Fee + gst ($30 student or $50 per family) is required upon registration and is non-refundable.
2. Registration Fee payments are to be made within 48 hours of registration.
3. Please send Registration Fee payments via E-Transfer to info@crossingsdance.com and use the password "swanlake"
4. Costume Fees(s) + Performance Fee + gst are non-refundable (Full Year Program). *Students returning from the 2019/20 Dance Season may apply the full amount of 2019/20 Costume Fees as credit towards 2020/21 Costume Fees.
5. The first month's tuition is also non-refundable.
6. Tuition fees can be paid in full or paid in 5 billing periods spanning September to January on either the 1st or the 15th of the month. Payments can be made by direct debit (preferred) or credit card, which are to be submitted PRIOR TO THE START OF THE 2020/21 DANCE SEASON.
7. Accounts are to be paid in full by January 15, 2021.
Direct Debit Payments:
1. Please refer to, complete and return PAD form to Crossings Dance.
2. Declined payments will be assessed a service charge of $25.00.
3. I agree to notify Crossings of any changes to my banking information and deliver a new PAD form if necessary by emailing info@crossingsdance.com. If new information is not provided to Crossings, and my payment is declined as such, the above policy regarding a $25.00 service charge will be applied.
Credit Card Payments:
1. The above-detailed fees will be processed to my credit card by Crossings Dance Ministries Inc.
2. Declined credit cards will be assessed a service charge of $25.00.
3. I agree to notify Crossings of any changes to my credit card information including a new credit card number or expiry date by emailing info@crossingsdance.com. If new information is not provided to Crossings, and my credit card is declined as such, the above policy regarding a $25.00 service charge will be applied.
