In consideration of the opportunity for my child to receive dance lessons provided by Dynamite Dance LLC, I, for my child and myself:
1. Agree that my child is in good physical health and able to take dance lessons without unusual risk of injury.
2. Understand that dance lessons entail a risk of physical injury, which may increase if my child does not follow the instructor's directions.
3. Release and discharge, and covenant not to sue Dynamite Dance LLC, its members, managers, agents, officers, or employees from/for any and all liability claims, demands, losses, or damages allegedly arising from or related to my child's participation in Dynamite Dance lessons. Provided that Dynamite Dance, its members, managers, agents, officers, and employees are not grossly negligent in their duties.
4. Agree that I have read this agreement release, fully understand the terms, and have signed it freely and voluntarily.
5. Agree that if any portion of the Agreement Release is held to be invalid, the balance shall continue in full force and effect not withstanding.
Registration Fee - $25 for first dancer and $10 for each additional dancer in a family. If you cancel your enrollment at a later date the registration fee is non-refundable.
Tuition Payments - All monthly tuition payments will be processed automatically by Dynamite Dance Studio on the 1st of each month. If the 1st falls on a Sunday or holiday it will be processed the next business day. I understand that it is my responsibility to keep my credit card or bank account information up to date.
If you do not want to be charged automatically, or would prefer to pay by check or cash, you can make your payment in advance of the 1st of the month. However, any unpaid tuition will be processed on the 1st.
If you would like to pay your year in full you must make your payment by check, cash, or through the parent portal by September 1st, 2018. We will not process annual tuition payments automatically.
If a payment cannot be processed due to out of date credit card or bank account information you will have until the 5th of the month to update your information and make your payment manually. After the 5th late fees will apply.
Tuition Discounts - Families with one or more dancers enrolled in a total of 5 or more hours of class each week will be eligible for a discount of 5% off their monthly tuition amount. Families with one or more dancers enrolled in a total of 10 or more hours of class each week will be eligible for a discount of 10% off their monthly tuition amount.
Returned Checks - If a check is returned to us for insufficient funds you will be charged a $25 returned check fee.
Costume Fees - Costume payments are due on November 15th, 2018. Late costume payments will have a $10 fee added to them per dancer. Costume fees are non-refundable once they have been paid. If your costume fee is not received by November 30th, 2018 a costume will not be ordered for you and your dancer will not be able to perform in the recital.
Spring Dance Recital - Our annual Spring Recital will be held in May 2018. Participation in the recital is not mandatory but strongly encouraged. Costume fees must be paid if you wish to dance in the recital. Recital tickets will go on sale in April 2019; tickets are required for anyone age 4 or older.
All students enrolled in a Company program are required to participate in the recital.
Absences - We do not give refunds for missed classes regardless of the reason for the absence. If an equivalent class is shown on our schedule you may attend that class for a make-up within one month of your absence. For classes without an equivalent on the schedule please contact us for make-up options. A make-up option is not guaranteed.
Cancellation - There is a $50 cancellation fee per class that you are enrolled in should you choose to withdraw from the program at or after your 5th class of the season. If you cancel before your 5th class all future classes that have already been paid will be refunded. Cancellation is from the date that we are notified in person or in writing via letter or email that you are withdrawing from the program. Any absences prior to formal withdrawal will not be refunded. If you cancel mid-year and your class was at capacity you will be responsible for the full balance of tuition for the year.
If you paid the full year - You will be refunded all future classes from the date of withdrawal minus the $50 cancellation fee per class. Your refund will be calculated in our office and we will mail you a check or refund to your card on file within 10 days.
If you are paying monthly - Your refund or amount due will be calculated in our office based on the amount of tuition paid to date and the number of classes held to date, as well as the $50 cancellation fee. Because each month does not have an equal number of classes and tuition is divided evenly into monthly payments cancelling at the end of the month does not necessarily mean that you are paid in full for the classes you have taken to date. You will be issued a final invoice or credit within 10 days of your formal withdrawal taking into account all of the above.
Choreography Policy - All choreography learned while at Dynamite Dance Studio is the property of the studio and may not be performed outside of the studio without consent from the choreographer and payment of a choreography fee.
Weather/Other Closings Policy - If classes are cancelled due to bad weather or for any other reason we will reschedule classes during our make-up week which is held the week leading up to recital. We cannot guarantee that make-ups will be held at your regular class time.
Appearance for Class - All dancers are expected to strictly adhere to the dress code as posted on our website. This includes proper clothing and shoes in the correct colors. Hair must be securely fixed back according to dress code; some classes require a ponytail while others require a bun. Hair should never be taken down and fixed during class; please secure it before class so that it does not need any attention during class. Dancers who are not dressed appropriately for class or do not have their hair secured correctly may be asked to sit out at the teacher's discretion.
Tardiness - All students are expected to be on time to class. If students are late they may be asked to sit out and warm-up for class independently. They may join in class at the teacher's discretion. If a student is repeatedly tardy we will contact a parent to discuss the issue. All students that require a parent's assistance to use the bathroom should be taken to the bathroom prior to the start of class to limit bathroom breaks during class time.
Expectations for Class - Dancers are expected to show respect for their teacher, fellow students, and the studio space at all times. If a teacher determines that a student is not acting in accordance with expectations they may be dismissed from class for the day.
Photo & Video - The studio reserves the right to use photos or videos from class or any other studio related event for marketing and advertising purposes. Names of dancers will never be associated with photos or videos used outside the studio.
Cell Phone & Other Electronic Device Policy - Students should silence or turn off cell phones or other devices prior to class and store them in our pocket organizers in the studios for the duration of class. Phones should never be used during water breaks as that is still considered part of class time. Phones should never be used to take videos or photos during class time unless it is with the teacher's permission. This policy applies to any device other than a phone that can take photos or video.
Personal Items - Dynamite Dance Studio is not responsible for items that are lost or stolen if you choose to bring them to the studio with you. We recommend leaving valuable items at home.
Dismissal - Failure to adhere to the studio's policies and procedures can result in a student's dismissal from the program without refund at the discretion of the studio directors.
