Policies
I understand it is the parent's responsibility to be aware of all dance studio activities, such as showcases, openings or closings, and class photo days.
I will monitor the studio website, social medias, and the monthly emails ("Dance Notes").
I will make sure to notify the studio in writing if my current address, e-mail, or phone number changes.
Tuition is due the 1st of every month.
There is a $15 registration fee for all dancers and is due at registration. This fee is non-refundable.
A $15 late fee will be added to all tuition not received by the 15th of the month.
Tuition at Dance Dimensions is based on a 9 month schedule (regardless of the number of classes).
There will be a $25 NSF fee for all returned checks. After the 3rd returned check, we will require that all fees be paid in cash or by credit card.
Accounts that are on automatic payment with a credit card will be charged a $10 NSF fee if payment is declined.
If tuition is not kept current, that dancer will be dropped from that class until the unpaid tuition is brought up to date.
Family discounts are available.
We accept cash, check, Visa, MasterCard, and Discover.
To keep with social distancing and limit direct contact, we recommend that everyone keep a credit card on file for their monthly tuition.
Please make checks payable to Dance Dimensions and lace the student's name in the memo box.
There are NO REFUNDS, credits, transfers from one dancer to another, or carry-overs month to month or season to season. There are no refunds for missed classes or vacations.
At this time we are unable to offer any make-up classes due to mandatory class sizes. Nor are we able to refund any tuition or offer class credit if the state closes down due to Covid-19.
I understand that any changes in classes MUST be made in writing. Please email us with your request.
I understand that these changes must be approved by the director.
I understand that if I am dropping a class, I must email the studio prior to the start of the new month or I will be charged for that following month's tuition.
I am aware that class additions can be made up until January, as long as there is room in the class and all current tuition and due balances are paid.
Dancers must arrive a minimum of 10 minutes prior to their first class to allow time for check in. Each studio has its own entrance & exit.
Dancers must come dressed and ready for class in proper attire. And are not allowed to change clothes at the studio.
Dancers must wait outside their studio door until they are asked to come into class.
Dancers who are late will not be allowed to enter the studio.
Shoes must be taken off before entering and all outside items must be placed in a bag. This includes Cell Phones & Water Bottles.
Instructors will be at the entrance to each studio to check dancers into class.
Hand sanitizer will be available at each studio door and at the front desk. Each dancer will use the hand sanitizer upon entering and exiting the studio.
Dancers will be assigned a space once they enter the studio and this will be their space to dance.
Only dancers, faculty, and staff are permitted to enter the building. Parents will drop-off and return for pick up.
Dancers must be picked up on time. We do not have the staff to watch dancers at all three studios when parents are late.
I understand that my student must inform the office if they need to be released early from a class for pick up.
The studio will be open 30 minutes prior to the first class of the day.
I understand that Dance Dimensions does NOT have any covered parking spaces and that I cannot park in front of any buildings/on the curb. Dance Dimensions is not responsible for any conflicts with other businesses that may occur if I park in reserved or covered parking spaces.
I WILL NOT PARK ON THE CURB IN FRONT OF ANY PART OF THE BUILDING OR UNDER ANY COVERED PARKING.
I understand that attendance is the key to maintaining level placement and being considered for advancement to the next level. Frequent absences may inhibit the promotion of a student to the next level.
I understand that students who are ill are not allowed to come to the studio.
For periods of extended absence, I understand that my dancer will remain a registered student but that a position will not be held in any particular class unless the monthly tuition is kept current. To halt tuition charges, I will be sure to drop my dancer(s) from any classes.
In the case of medical leave, please contact the front desk with a doctor's note.
I understand that students are expected to know the rules, comply, and maintain the standards of their class level, which includes good attendance and a positive, courteous attitude in class towards their instructors and fellow dancers, and follow the new studio health guidelines. Unnecessary talking and improper behavior will not be permitted.
I am aware that my dancer is only allowed to have water inside the studio. NO GUM, FOOD, OR OTHER DRINK is allowed.
I understand that dancers must have their cell phones turned off or to silent and in their bags during class. If I need to contact my student, I will call the front desk and let a faculty member know.
I understand that ongoing disruptive behavior may result in temporary loss of class participation privileges.
There may be certain cases where we reserve the right not to offer future services. We have a ZERO TOLERANCE policy when it comes to threats, bullying, or acts of violence.
I understand that conferences with instructors must be made in advance. Instructors and/or the director are not available before, after, or between classes without a scheduled appointment. I will not attempt impromptu meetings between or during classes. I understand this interrupts class schedules.
Instructors and/or the director will gladly meet at an appointed time.
I understand that Dance Dimensions has the right to refuse service to anyone.
I acknowledge that dance instruction is a strenuous physical activity involving the risk of physical injury and have taken all steps necessary to learn of any physical impairment(s) that would limit or affect my safe participation. I voluntarily assume all risks arising from my participation in dance instruction and hereby release and waive all claims against Hartin Dance Dimensions Inc., Kathleen Hartin (Owner/Director), any instructor contracted by Hartin Dance Dimensions Inc to provide instruction and any and all other participants in class for any damages arising out of my instruction, demonstrations, or performances. I further agree to hold harmless and indemnify Hartin Dance Dimensions Inc., Kathleen Hartin (Owner/Director), any instructor contracted by Hartin Dane Dimensions Inc. to provide instruction and all of the other participants in class from any loss, cost or expense, damage or injury arising from my participation in dance instruction, demonstrations, or performances. Each students may decline to participate in any activity. Please inform the instruction or director of any physical limitations you may have. If you are in doubt as to your physical abilities, please consult your physician before participating. Hartin Dance Dimensions Inc. can not dispense aspirin or other medication.
Due to the 2020 outbreak of the novel coronavirus, Dance Dimensions is taking extra precautions with the care of every student to include: 6' social distancing, enhanced sanitation procedures, and face coverings (see health guidelines).
I will not bring my dancer to the studio if there has been any known exposure to a person who has been diagnosed or under quarantine for COVID-19.
I will not bring my dancer to the studio if they or anyone in close contact to the them recently travelled out of the USA in the last 4 weeks.
I will not bring my dancer to the studio if they or anyone in close contact to them has any symptoms of COVID-19.
I understand that Hartin Dance Dimensions Inc, its owner, representatives, and/or employees cannot be held liable for any exposures to the coronavirus caused by misinformation on this form or the healthy history provided by each client.
I have read and agree to the Health Guidelines and Policies set forth by Dance Dimensions.
