Release of Liability
I agree to allow my child to participate in all IRENE SCHOOL OF RHYTHMIC GYMNASTICS activities. I, the undersigned, do agree to hold harmless:

All Coaching Staff
Any practice or tournament facility used by IRENE SCHOOL OF RHYTHMIC GYMNASTICS
And any others that are affiliated with IRENE SCHOOL OF RHYTHMIC GYMNASTICS
of any and all claims for injuries, causes of action, or liability related to participation in these activities. I understand that participation in this sport could be a dangerous activity involving many risks of injury and that injuries, possibly serious, may occur. I hereby assume all risks associated with the sport and agree to hold IRENE School of Rhythmic Gymnastics harmless from any and all liability, causes of action, debts, claims or demands of any nature which may arise in connection with participation in this sport. I, as parent or guardian, have been informed of the risks inherent in this sport as outlined above and agree to hold IRENE School of Rhythmic Gymnastics harmless from any liability which may arise out of participation in class training activities, related activities.
In the event that I cannot be reached in an emergency and my child requires medical attention or other treatment, I hereby give permission to the physician selected by IRENE School of Rhythmic Gymnastics administration to hospitalize, secure proper treatment for, and to order injection, anesthesia or surgery for my child, as named in this Release of Liability form
Code of Conduct
Irene School of Rhythmic Gymnastics, have implemented the following Parent Code of Conduct for the important message it holds about the proper role of parents in supporting their child in sports. Parents should read, understand and sign this form.
I will focus on enjoying the sport and reduce emphasis on winning. I will teach my gymnast that an honest effort is as important as victory, so that the result of each competition is accepted without undue disappointment.
I will praise my child for competing fairly and trying hard, and make my child feel like a winner every time.
I will emphasize skill development and practices and how they benefit my child over winning.
I will not criticize my gymnast in front of others.
I will avoid gossiping about coaches, other gymnasts, programs or the club - particularly in front of gymnasts or parents. In the past this has been a source of misinformation and bad feeling within the club.
I will always remember that my gymnast is involved in the sport for their enjoyment, not mine.
I will exercise good sportsmanship and applaud good skills and routines by all team members.
I will accept decisions of all judges as being called to the best of their ability and will not raise issues of disagreement publicly.
My behavior and language will always remain appropriate in the presence of children.
I will support the coaches and their coaching techniques and decisions.
I will inform the head coach or administrative director immediately of any physical injury or problem that may affect the safety of my gymnast during practice or competitions.
I will refrain from coaching my gymnast at home, at practice, or at any competition.
I will not interfere in coaching matters and never give my child or the coach technical advice about rhythmic gymnastics.
I understand that once my child enters the training area they are in the care and supervision of the coaching staff. I will refrain from attracting their attention until they have been dismissed at the end of the class. I should not remove my child during class without first speaking to the coach.
I will share concerns and complaints about any aspect of the club through the appropriate channels. (#1 Administrative Director, #2 Head coach ). If I have a question, concern or complaint, I will follow the correct procedure. I will not discuss the matter in front of the gymnasts or other parents. In the past this has been a source of misinformation and bad feeling within the club.
I may not seek coaching at any gymnastics facility other than ISRG or seek coaching from any coach not currently involved with the ISRG competitive program without written consent from my gymnast's coach.
I understand that failure to adhere to these rules by myself, my spouse, or guests may result in revocation of my membership to Irene School of Rhythmic Gymnastics at any time.
COVID 19 waiver
Participant Requirements & Waiver and Release of Liability Assumption of Risk Relating to Coronavirus/COVID-19
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact, including individuals without disease symptoms. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited or limited the congregation of groups of people.
VOLUNTARY PARTICIPATION: I, agree and acknowledge that I have voluntarily enrolled my child, (herein after referred to as "the Athlete") to participate in a trainings in the sport of Rhythmic Gymnastics run by OCRG LLC (or the "School").
EXISTING RELEASE FORM VALIDITY: I acknowledge that previously signed Release of Liability and Waiver forms signed on my behalf and on behalf of the Athlete continue to hold their full force and effect. This Waiver and Release is specific to Covid-19 and its risks.
AGREEMENT TO ABIDE: All participants in "School" must agree to and abide by a set of specific conditions. I, the undersigned, and the Athlete acknowledge and agree to abide by conditions including, but not limited to the following:
- No one may enter the OCRG/A4 facility if you have COVID-19, you or family members have been exposed to COVID-19, you have been in contact with someone who has contracted COVID-19, you are feeling sick, you have traveled by air or internationally during the last 15 days. - Participants must sanitize hands prior to gym entry and throughout. - No personal items in the gym except keys and water bottle. - Participants must wait outside the gym until 5 minutes prior to the start of their trainings. - Bathrooms are off limits unless in an emergency. - No spectators allowed in the facility - Participants must submit to a quick contactless temperature testing prior to participation. Any temperature over 100 degrees will disallow participation. - Keep social distancing when not training.
OCRG LLC and A4 Volleyball Club, LLC. has put in place numerous preventative measures and enhanced cleaning protocols to reduce the likelihood of spreading Covid-19 in our environment; however, the School cannot guarantee that you, the Athlete, your other children, if applicable, or additional family members will not become infected with COVID-19. Further, attending activities at the School could increase your risk and the risk of additional family members around you in contracting COVID-19. Participation conditions are subject to change.
COVID-19 CONTAGIOUS NATURE: I acknowledge the contagious nature of COVID-19 and voluntarily agree to the terms described above and assume the risk that the Athlete, my children, my family members, and I (hereafter my "Family") may be exposed to or infected by COVID-19 by our participation and that such exposure or infection may result in personal injury, illness, permanent disability, and death.
ASSUMPTION OF COVID-19 RISK: I understand that the risk of becoming exposed to or infected by COVID-19 at the School may result from the actions, omissions, or negligence of myself and others, including, but not limited to, School employees, volunteers, and program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself, the Athlete, and my Family (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or the Athlete or my Family may experience or incur in connection with my Athlete's attendance at the School or participation in School programming ("Claims").
REPORTING OF GOOD HEALTH: I warrant and represent that myself, the Athlete, and my Family are in good health and have no symptoms of sickness or illness, either which have been linked to or have not been linked to COVID-19, and that
myself, the Athlete, and my Family are not aware of any exposure to other individuals who are confirmed or suspected to have COVID-19, have COVID-19 symptoms, or are awaiting test results for COVID-19. I acknowledge and agree to report any change in the good health status of myself, the Athlete, or my Family and that such reporting will terminate the ability of the Athlete to participate until the health status of myself, the Atletes, and my Family is determined again to be good.
HOLD HARMLESS: On my behalf, and on behalf of the Athlete and my Family, I hereby release, covenant not to sue, discharge, and hold harmless the School and A4 Volley LLC, their employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Club, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Club program.
KNOWING AND VOLUNTARY EXECUTION: I have carefully read this Agreement and fully understand its contents. I am aware that this is a release of liability between me, OCRG LLC, A4 Volleyball Club, LLC. and/or their employees, volunteers, agents or contractors, any of its affiliate organizations, and any other contracted facilities it uses, and I acknowledge its contents and agree to the terms, conditions, and contents fully of my own free will.
