There is an annual, non-refundable, Family Membership due upon initial registration and renewed annually. If family enrolled in gymnastics & dance only the gymnastics membership fee will be charged.
Payment Policies - Recreation, Preschool, & Dance
Recreational, preschool and dance payments are due the first week of each session. Payments not received by the end of the first week of each session will automatically be charged a $15 fee, unless prior arrangements are made with the office. If tuition is still not paid by the end of the second week of the session, student(s) will not be allowed in the gym for practice, and may be dropped from the program. Returned Check: A $35 fee is charged on all returned checks.
Enrollment for recreational, preschool and dance classes is ongoing in 4-week sessions, throughout the school year. There are 10 sessions which run from September through the middle of June. Enrollment for summer months is not automatic, you will need to re-enroll.
Payment Policies - Team, PreTeam & Xcel
Enrollment for PreTeam, Xcel, Girls Compulsory and Optional Team, and Boys Team is month to month year around. Payment is due the first of each month. Payments not received by the 10th of each month will automatically be charged a $25 fee, unless prior arrangements are made with the office. If tuition is still not paid by the 15th of the month, student(s) will not be allowed in the gym for practice and may be dropped from the program. Attendance & Make Ups
Attendance: Please make sure that gymnasts arrive on time for their classes. Warm-ups are important to avoid injury and late students disrupt the flow of the class. We only have your children for a short time and we like to make the most of it. Since coaches go from one class to the next, it is important that your child is picked up promptly. In case of emergency please contact the office to assure your child is supervised until your arrival.
To maintain our gymnast:coach ratio and avoid overcrowding of classes all make-ups must be scheduled in advance. Only one absence per session qualifies for a make-up. You will have 8-weeks to make up your absence.
It is very important to us that your children receive consistent coaching and this policy will be enforced to protect the integrity of the program. We do not pro-rate for missed classes or substitute make-ups for tuition payments. Summer Session make-ups must be made up during the Summer Sessions, School year make-ups can not be made up in Summer.
All make-ups are forfeited if the student drops from the program.
Adds, Drops & Transfers
All adds, drops or request for transfers must be processed via the parent portal or the front office. If a class is dropped after the next session is in progress, you remain responsible for the full 4-weeks of classes. No refund will be given.
RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT AND PARENTAL CONSENT
By signing this form and in consideration of participating in gymnastics events, I, for myself and as the minor's parent and/or legal guardian understand the nature of this activity and participation involves risks of serious bodily injury, including permanent injury, paralysis and death which may be caused by my or the participant(s) actions or by the negligence of the "releasees" named below; and that there may be other risks not known to me or foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, cost, and damages that I or other listed individuals incur as a result of participating in the activity. If we believe the event conditions to be unsafe, we will immediately discontinue participation in the activity.
I hereby release, discharge and covenant not to sue PRECISION ELITE GYMNASTICS ACADEMY, INC., its administrators, directors, officers, employees, volunteers, other participants, and if applicable, owners and lessors of premises on which the activity takes place, (each considered one of the " releases" herein) form all liability, claims, demands, losses, or damages on my account or alleged to be cause in whole or in part by the negligence of the "releasees" or otherwise, including negligent rescue and first aid and future agree that if, despite this release, waiver of liability, and assumption of risk I, or anyone on the minor's behalf or on my behalf, makes a claim against any of the releasees, I will indemnify, save, and hold harmless each of the releasees from any loss, liability , litigation expenses, attorney fees, damage, or cost, which any may incur as the result of such claim.
I have read the RELEASE AND WAIVER OF LIABILTY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT, and understand that the parent(s)/ legal guardian on behalf of themselves and anyone listed on this waiver have given up substantial rights by signing it and have signed it freely and without any inducement or assurance of any nature and intend it to be complete and unconditional release of all liability to the greatest extent allowed by law and agree that if any portion of this agreement is held to be invalid the balance, not withstanding, shall continue in full force and effect.
AND I, the minor's parent and/or legal guardian, understand the nature of the above referenced activities and the minor's experience and capabilities and believe to be qualified to participate in such activity. I hereby release, discharge, covenant not to sue and AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS each of the releasees or otherwise, including negligent rescue operations, and further agree that if, despite this release, I, the minor, or anyone on the minor's behalf or on my behalf makes a claim against any of the releases, I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS each of the releasees from any loss, liability, damage, or cost, which may incur as the result of such claim.
Additional Policies: Photographs and Videos: I also give permission for photographs and videos of my child to be used in print or broadcast media or posted on the internet as deemed appropriate for the promotion of any Precision Elite Gymnastics Academy Inc. activities.
All children, including siblings & guests: must remain inside the Precision Elite building unless supervised by an adult. We cannot be responsible for children once they leave our building. Children are not allowed to wear jewelry into the gym Precision Elite will not be responsible for any lost jewelry or other personal items.
Dress Code: For the safety of the gymnasts and dancers, Precision Elite requires that all participants are dressed appropriately in leotards or shorts and t-shirts, hair pulled back and without socks. Clothing with zippers, buttons, or snaps should be avoided, as well as jewelry, as it can lead to injury.
I represent and warrant that if I am purchasing something or paying for a service from this facility or from other merchants through this facility that (i) any credit card or bank account draft (ACH Draft) information I supply is true and complete, (ii) charges incurred by me will be honored by my credit card company or financial institution, and (iii) I will pay the charges incurred by me at the posted prices, including any applicable taxes, fees, and penalties.
I hereby authorize (if online payment is made or autopay information is provided) this facility to charge my ACH draft, or credit card account. I understand that a 30 day written notice is required to terminate billing and I am responsible for payment whether or not my student attends classes until I notify this facility in writing to drop my student from class(es).
Should I dispute a charge through my financial institution this will constitute a breach of contract possibly resulting in, but not limited to, penalties, additional fees, collection, legal action, and/or termination of any and/or all current and future services.
