IN CONSIDERATION of being permitted to participate in any way in any activity ("Activity") at Stars Gymnastics LLC and or Elevate SGV, I, for myself, my personal representatives, assigns, heirs, and next of kin:
I ACKNOWLEDGE, agree and represent that I understand the nature of of physical activities and that I and/or my minor child am qualified, in good health, and in proper physical condition to participate in such activity. I further agree and warrant that if at any time I believe the conditions to be unsafe, I will immediately discontinue further participation in the Activity.
I FULLY UNDERSTAND that: (a) GYMNASTICS/TUMBLING, FITNESS, WEIGHT LIFTING AND OR NINJA ACTIVITIES INVOLVE RISKS AND DANGERS OF SERIOUS BODILY INJURY, INCLUDING PERMANENT DISABILITY, PARALYSIS, AND DEATH ("Risks"); (b) these Risks and dangers may be caused by my own or my minor child's actions or inactions, the actions or inactions of others participating in the Activity, the condition in which the Activity takes place, or THE NEGLIGENCE OF STARS GYMNASTICS LLC OR ELEVATE SGV (c) there may be OTHER RISKS AND SOCIAL AND ECONOMIC LOSSES either not known to me or not readily foreseeable at this time; and I FULLY ACCEPT AND ASSUME ALL SUCH RISKS AND ALL RESPONSIBILITY FOR LOSSES, COSTS, AND DAMAGES I incur as a result of my participation or that of the minor in the Activity.
MEDICAL RELEASE & WAIVER
I, the athlete named above or the parent/guardian of the child(ren) named above, know that participation in any sport is a potentially hazardous activity. I realize that I/ he/she should not participate unless I/he/she is medically able and properly trained. I assume all risks associated with my/his/her participation. Having read this waiver and knowing these facts and in consideration of your accepting my child's application to participate, I waive and release Stars Gymnastics LLC and Elevate SGV, all sponsors, affiliated Clubs, event organizers, and officers and members thereof from all claims or liabilities of any kind arising out of my/his/her participation.
PERMISSION TO ADMINISTER EMERGENCY TREATMENT
Should emergency medical treatment be necessary during instruction/events, I hereby grant consent to apply the following medical treatment to myself (or my child in my absence): any examination, anesthetic, medical or surgical diagnosis and/or special supervision of duly licensed physician or surgeon. This consent is given in advance of any specific diagnosis. In my absence, and as parent or legal guardian of the child(ren) named, I hereby grant my permission, in the event of injury or sickness, to have the necessary emergency medical treatment administered to my child by a trained medical professional.
In addition, I also grant my permission to have my child transported to a hospital, doctor's office, or emergency clinic in the event of such injury or sickness.
** It is my responsibility to provide Stars Gymnastics LLC with my child(ren)'s medical conditions, allergies, and insurance information.
PICTURES & SOCIAL NETWORKING
Stars Gymnastics Staff may occasionally take pictures of students participating in the Stars Gymnastics Program. Photos may be published on program website, newsletters, schedules or on social networks such as Facebook, Instagram, etc. Your signature grants permission
Students enrolled in Stars Gymnastics classes have a $35 annual membership fee that is due at the first time of registration and on that anniversary date each year following. REVISED DURING COVID-19 ON 5/22/20: Students whose membership usually expires in the months of March -December: Stars Gymnastics will post QUARTERLY Membership Fees to student accounts on the 1st of July AND October 2020, in the amount of $9.00.
Registration is on a first come first serve basis. Class enrollment is open 15 days prior to the start of the upcoming session. Your space is reserved for the session once your payment is made. Sessions are eight weeks in length. We recommend that you enroll your children as soon as possible, to avoid your favorite class filling up. Tuition is non-refundable once the session has begun. Auto Pay participation provides students with a guaranteed spot in the class of their choice. However, a separate authorization form must be completed and signed to join auto pay.
Each class has a max number of students to coach ratio: Classes Age 1-6: 8:1 Classes Age 7+: 10:1

ASSIGNED CLASS INSTRUCTORS
Each class has an assigned instructor for the full eight-week session. Requests for a class taught by a specific instructor may be submitted at the front office up to two weeks prior to an upcoming session. If available, students may be transferred into another class during the current or for a future session. Substitute instructors will be assigned as needed for a sick instructor.
ADVANCING/CHANGING CLASS LEVELS
Your child will be automatically enrolled in the same class every month unless you notify the office otherwise. If your child advances in levels, it is your responsibility to notify the office as soon as possible to initiate the change in class enrollment and automatic registration for future months.
WHAT TO WEAR
Boys must wear tucked in T-shirts and elastic waist shorts/pants. Girls are required to wear a leotard or unitard. NO chewing gum or dangling jewelry. Stud earrings may be worn. Dangling or hoop earrings may NOT be worn. Bracelets and rings may NOT be worn. Hair should be pulled neatly and securely away from the face so that is stays up for the entire workout, INCLUDING BANGS. Girls should not wear bows or other large hair ornaments that may cause discomfort during activity. All students will be barefoot during class. Personal items should be left in cubby holes or with parents. LEAVE JEWELRY ARTICLES
AT HOME. Stars Gymnastics facility's staff will not be responsible for ANY items that may be lost or stolen. Be sure your student's personal items are marked with their name.
Remember that our students may include young children. Drive slowly and carefully through the parking lot. Walk your child to the line. Do not take a chance on your student running to and from your car. If late pick up happens frequently, Stars Gymnastics will charge $1.00 per minute late pick up fee, which must be paid before future enrollment.
REVISED DURING COVID-19 ON 5/22/20: Arrive 5-10 minutes prior to the scheduled class time. Everyone must line up outside of the facility upon arrival to receive a quick Covid-19 Health screening prior to entry. Everyone will be asked to use hand sanitizer upon entry. If there is not room inside the facility, parents must wait outside. Instructors will take the student out to you right outside of the front doors.
Events such a camp, TGIF, Parents Night Out, Movie night or anything else that is going on outside of normal business and class hours will require a parent to line up with the child outside of the facility to sign the participant in and/or out. These events will usually require an additional registration fee. Non-Members may participate in many of our special/closed events but are required to have student information forms and waiver completed before enrolling or attending. Children without required documents submitted will not be allowed to participate and may not be able to remain in the facility.
Every class begins with a warmup period. This warmup time is important and necessary for every student. It provides physical and mental preparation for class and when performed completely, can help prevent injuries. Therefore, if your child arrives late and misses any portion of warm up period, he/she WILL NOT BE ALLOWED TO JOIN THE CLASS. You may call for a make-up class if you are running late. Each student is allowed ONE make up class per eight week session for classes missed for their own personal reasons. Instructors take attendance at the beginning of every class. Make-Up classes cannot be scheduled prior to the missed class. Students must first be marked absent. Make up classes are offered every 6-7 weeks, on a Sunday. Students may not use another regularly offered weekly class as a make-up. Students must enroll in a make-up class, prior to attending. Make up classes must be scheduled within two weeks and taken within eight weeks of the missed class or will be forfeited. All make up classes will be arranged by the office (not by instructors). Make up credits may NOT be used to discount tuition. A missed scheduled make up class may not be made up.
UPDATED DURING COVID-19 ON 5/22/20: Before reopening for regular classes, Stars Gymnastics will have a soft reopening, offering make up classes for the weeks that we were closed due to Covid-19. Students must register in advance for the makeup classes and may attend multiple days per week until they have made up all classes missed.
OTHER SAFETY RULES:
All students may use our storage pockets and cubbies to store clothing and shoes, and then wait in the designated areas until the instructor calls them for class. Students may not be in the active practice area or play on the apparatus at any time prior to, or after their class.
• Students must arrive for practice already dressed in the appropriate class attire. (see other side for information on attire)
• Parents, guests, and siblings are required to wait in our observation area. Enrolled students ONLY under the direct supervision of an instructor are allowed in the practice area.
• Since our seating area is limited, we ask that no strollers be brought into the gym. Baby carrier seats are acceptable but must be placed in a safe location.
• You may bring toys, books, or other activities to keep your other children busy during class time. The lobby is not to be used as a play area. All spectators must remain in the designated seating area.
AUTO RE-Enrollment (Auto Pay)
UPDATE DURING COVID-19 ON 5/22/20: Auto pay processing will resume June 15, 2020 for those who were enrolled prior to the Covid-19 closure on 3/18/20. Any cancellations must be received in writing prior to 6/13/2020 at Accounts@Stars-Gymnastics.com. No payments were processed in April or May 2020.
Auto Re-Enrollment (Auto Pay) will keep your child enrolled and your tuition paid. No invoicing or payment reminder will be needed. Once enrolled, your child will be automatically enrolled in the same class each eight-week session until you notify the office otherwise. Participants of Auto pay also receive a 5% discount off total tuition due for each 8-week sessions. Auto-Pay transactions are processed on the 15th of each month with 1/2 of each session tuition (after discount), processed each month. Auto Pay cancellation or suspension must be submitted in writing a minimum of 10 days prior to upcoming processing date. Tuition is non-refundable once the session has begun.
• Enrollment into Auto Pay is done by completing a separate Auto Pay Authorization form. You may not join auto pay by online registration, verbal communication, a text, or an email. Once you have completed the form, your payments and guaranteed enrollment will begin on the upcoming date of the 15th with enrollment in the following month. Until that time, enrollment is first come-first served, and you must take care of your registration by using our online enrollment system or getting assistance from our front office staff.
• PARENTS MUST NOTIFY STARS GYMNASTICS TO DROP A STUDENT FROM CLASS. Only a written notice via email, regular postal mail or hand delivered to our front desk will be acceptable. Automatic registration and enrollment will continue until written notice is received.
• 15 day written notice is required to be removed from autopay. No exceptions. Phone calls, voice mail or verbal notification at the front desk are not acceptable forms of cancellation. Suspension/cancellation forms are available at the front office. Emails are also acceptable. Refunds will not be given if written notification is not received by the 1st of the month, prior to processing on the 15th.
60/40 INSTALLMENT AGREEMENT:
Register at the front desk before the start of each eight-week session. Pay 60% of tuition at time of enrollment and the balance of 40% on the 4th Monday of the session. Multiple child or Multiple class discounts will still apply. Participants of the Installment plan must complete an authorization form for the 2nd payment. *There are no installment discounts. Participation in the Installment plan does not include re-enrollment for future sessions. You must continue re-enrolling on a first come-first serve basis each session. You may re-use the installment agreement each month if you choose to.
Tuition is non-refundable once the session has begun, even if the student does not attend the entire session. Once the student takes a spot on the class roster and the session begins, tuition is due in full and will not be refunded for any reason.

2. Missing your scheduled work out time will result in forfeiting that session. No makeups or refunds are available.
3. Autopay is required for Independent Assistant training and Semi Personal Training.
4. Independent Training clients not on Autopay, may schedule training sessions a max of 7 days ahead on a first come, first served basis.
5. All training sessions are a max of one hour and 15 minutes.
What to wear/bring
1. Arrive in your workout attire.
2. Bring a water bottle.
3. Bring a small towel for your sweat.
4. Bring a bag to keep your belongings in.
Independent Assisted Training
1. Mandatory Auto Pay
a. Up to 16 Pre-scheduled training sessions per month.
b. $120 processed on the 15th of every month for the upcoming month.
2. Training sessions with workout plan provided by your trainer.
Semi Personal Training
1. Mandatory Auto Pay
a. Up to 12 pre-scheduled training sessions per month
i. $200 processed on the 15th of every month for the upcoming month.
b. Up to 16 pre-scheduled training sessions per month
i. $250 processed on the 15th of every month for the upcoming month.
