I/We hereby give my child permission to dance at Bethesda Conservatory of Dance, LLC. I/we waive the right to any legal action against Bethesda Conservatory of Dance, LLC for any injury sustained while dancing at any class or any Bethesda Conservatory of Dance event. I/we realize that participation in dance classes and activities could involve some possible personal injury. Despite precautions, accidents, and injuries may occur. By signing this release, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Bethesda Conservatory of Dance, LLC.
I/we understand that I/we are enrolling in a program of physical activity and have agreed that I/we are in good physical condition and do not suffer from any condition that would prevent or limit participation in the dance program and to inform the Owner/Director and/or instructor of any condition or injury that may occur while enrolled.
I/we agree to release and hold harmless Bethesda Conservatory of Dance,LLC including its teachers, dancers, staff members, and facilities from any cause of action, claims, or demands, now or in the future. I/we will not hold Bethesda Conservatory of Dance, LLC liable for any personal injury or any personal property damage, which my occur on the promises before, during, or after classes. Furthermore, I/we agree to obey the class and facility verbal and/or written rules/directions and take full responsibility for my behavior in addition to any damage I/we may cause to the facilities utilized by Bethesda Conservatory of Dance, LLC.
In the event that I/we should observe any unsafe conduct or conditions before, during, or after my/our classes, I/we should report the unsafe conduct or conditions to the Owner/Director, Instructor,or Staff Member as soon as possible.
Unless enrolled in an 8 week session or mini-session, I understand that this is a 10 month, school-year based program. Tuition is a yearly fee broken down into 5 equal payments. Charges will be generated on September 1, November 1, January 1, March 1, and May 1. We will not be pro-rating any part of the tuition after the child begins lessons. NO EXCEPTIONS WILL BE MADE. If you plan to miss classes within the billing period, you will be issued make up lessons prior to the missed class. To cancel enrollment, a cancellation request must be submitted in writing by the 15th of the month prior to termination. In order to be issued a make-up lesson, I must inform the staff at office@BethesdaConservatoryofDance.com if you or your child before missing a lesson. Though not guaranteed, every effort will be made to offer you/your child an appropriate make-up lesson. Make-up lessons must be scheduled with a staff member no less than two days prior to the desired make-up class. No credit or refund will be issued for missed lessons. Unless BCoD is notified by Nov. 1, I understand that I will be charged a $70.00 costume deposit per child per class on November 15. I understand that I will be charged a balance of between $5 and $35.00 upon receipt of the costume.
A $35.00 late fee will be applied to charges that extend past the 15th of the month. Those not paid in full by tuition as of May 1 may not participate in recital.
