In consideration of Rock Solid Gymnastics allowing the participant to participate sports activity, class, competition, team, including non-gymnastic activities such as dance, cheerleading, martial arts and open-gym activities (hereinafter referred to as the "Activity"), I, and if I am not yet 18 years old my parents or legal guardians, agree to be bound as follows:
Representation of Ability to Participate: I understand the nature of the Activity, and I represent that the participant is qualified, in good health, and in proper physical condition to participate in the Activity. Should I ever believe that any of the above representations have become untrue, or if I should ever believe that the Activity is not safe or is no longer safe for the participant, then it will be my responsibility immediately to discontinue the participant's participation in the Activity.
Acknowledgment and Assumption of Risks: I understand that the Activity involves risks of serious bodily injury, including permanent disability, paralysis and death, which may be caused by the participant's actions or inactions, those of others participating in the Activity, the conditions of which the Activity takes place, the negligence of the "Released Parties" named below, or other causes. I further understand that there may be other risks either not known to me or not readily foreseeable at this time. I fully accept and assume all such risks and all responsibility for losses, cost and damages that may result from the Activity. I hereby give my approval of and consent to the participant's participation in the Activity. I assume all risks and hazards incidental to the activity and to transportation to and from the Activity.
I hereby release, acquit, covenant not to sue, and forever discharge Rock Solid Gymnastics, it's owners, officers, administrators, employees, agents, volunteers, sponsors, advertisers, coaches and supervisors, and all other persons providing facilities or assisting in the conduct of the Activity and in the transportation of participants to and from the Activity (collectively the "Released Parties") of and from any and all actions, causes of action, claims, demands, liability, losses of damages of whatever name or nature, including but not limited to those arising from or in any way related to the negligence of any of the Released Parties, that arise out of or are connected in any way to the participant's participation in the Activity (collectively the "Released Claims").
Indemnification: I will defend, indemnify and hold harmless the Released Parties from (that is, to reimburse and be responsible for) any loss or damage, including but not limited to costs and reasonable attorney's fees (including the cost of any claim I might make or that might be made on my behalf or the participant's behalf that is released in this document), arising out of or connected in any way with any of the Released Claims.
The annual membership fee covers participant awards, equipment maintenance and/or replacement, continuing education and certifications of staff, supplemental insurance, etc. Registration fees are due at initial registration and then annually thereafter at fall registration. Membership is active August 1 - July 31 no matter the time of year when you register. This fee is required and non-refundable. Membership fee is prorated throughout the year based on enrollment date.
Class and Team tuition are due on the 1st of each month unless otherwise noted in the class/event registration information. Tuition will be posted 10 days prior and charged on the 1st. Cash or check are accepted forms of payment, but must be received prior to the 1st of the month to avoid credit card charge. Our tuition is monthly and is billed as such. Should you enroll after the 2nd week of the month tuition will be prorated.
Should payment be declined for any reason, a reprocessing fee of $5 will automatically be added to your amount due. A late fee of $10 will be incurred after the 3rd of the month and your child will be unable to attend classes until full balance has been settled. If payment has not been received by the 7th of the month your child will automatically be dropped from the class .
Returned check fee: $25.00
Refunds will not be given for missed classes. Although we will try our best to fit you into a comparable class for a make-up if requested, we cannot guarantee a make-up class will be available. Make-up classes must be done in the same month as the missed class. Make-up classes are not available for summer classes, camps and team sessions. Refunds and make-ups will not be given for cancellations due to inclement weather or other extraordinary circumstances beyond our control.
Please be aware when registering online, the yearly $50.00 registration fee ($75.00 for family) will be charged to your account in addition to your 1st month's tuition as noted in the class/event information. Once you register, you will receive an online portal registration for updating your personal information and enrolling in future class/events. Please do not register via our website after your Parent Portal account is generated or you will be charged another yearly registration fee.
Girls must wear a one-piece leotard without an attached skirt (spandex, booty, or jazz style shorts with leotard are acceptable, skirts are not)
Boys must wear a fitted T-shirt (surf shirt or under armor style) or fitted tank and athletic shorts or pants.
Hair should be pulled back and away from the face. Hard headbands are not permitted.
Jewelry of any type on neck, wrists or ankles is not permitted. Earrings must be studs smaller than tip of fingernail.
Cubbyholes for loose articles are provided for your convenience. Rock Solid Gymnastics is not responsible for lost, stolen, or misplaced items.
A water fountain is not available at our Coleman location. We highly recommend your child bring a water bottle to class. Water bottles will be stored in gym during class. Hydration is extremely important and your child will be allowed to get water as needed. Water bottles must be able to close completely (open straw cups or tumblers with lids will not be allowed in the gym). Sports drinks are not permitted in the gym.
Prior to Class
Participants should wait in the observation areas for a coach or instructor to escort the athletes onto the gym floor. Only registered participants will be allowed onto the gym floor and equipment and ONLY under the direction of staff. Parents, siblings and other visitors must remain in the observation areas and not enter the gym area. Absolutely no one is permitted on the gym floor and equipment without a coach or instructor present. Please advise your child to use the restroom before class begins.
*Special Note: 3yr olds are welcome to enroll in our 3yr old Preschool classes, however if they are not 100% potty trained, the parents must stay at Rock Solid for the duration of the class in case the child needs restroom assistance.
Please refrain from using flash photography and talking to or assisting your child during class. This could be distracting to both the coach and athletes and could cause serious injury. Please leave the coaching to us!
Please be prompt in picking up your child from class. Supervision after class is not available. Unattended children must wait in the cubby area for pick up.
Classes begin promptly at the time scheduled; please make every effort to arrive 3-5 minutes before class begins to allow time to undress, put hair up, use restroom, etc. Should you arrive 15 minutes after class has started the child may not be admitted to class and will be able to schedule a makeup class if available. Arriving 10 minutes late to 3 classes in a row may result in being asked to enroll in a different class to better accommodate your schedule.
I, do hereby consent and agree that Rock Solid Gymnastics, its employees, or agents have the right to take photographs, videotape, or digital recordings of my child and/or me for the duration of their involvement with Rock Solid Gymnastics; it's activities, classes and competitive teams.
I further consent that my child's name and/or my name and identity may be revealed therein or by descriptive text or commentary and their name may also be used to associate them with their specific program.
I understand this will be limited to their first name and may include the first letter of their last name to distinguish the difference between those that have the same first name. I do hereby release to Rock Solid Gymnastics, its agents, and employees all rights to exhibit this work in print and electronic form publicly or privately and to market copies.
I waive any rights, claims, or interest I may have to control the use of my identity or likeness in whatever media used. I also understand that Rock Solid Gymnastics is not responsible for any expense or liability incurred as a result of my participation in this recording, including medical expenses due to any sickness or injury incurred as a result.
I represent that I am at least 18 years of age, have read and understand the foregoing statement, and am competent to execute this agreement.
