GENERAL POLICIES: Students are expected to be ready for their lesson by the start of class time. This means that they are dressed in the appropriate attire with their hair secured back, and that they have used the restroom. No gum, food or drinks are permitted in the studio. Water bottles are permitted. Students may not sit down unless told to do so by their teacher. No excessive conversation should take place during class. This can be disruptive to fellow classmates on the floor. Students can learn by observing others. Please be respectful of yourself and others. Kindness goes far. No foul language will be tolerated. Students may not leave the classroom without a teacher's permission. This is for their safety. Parents/Students should call or email the studio to let their teacher know if they will be missing class. Parents are not allowed into ongoing classes. If you have a concern, please call or email the studio. If you need to remove your student from class early, please inform the teacher prior to class. If an emergency arises, please knock on the studio door and a teacher or assistant will answer. Please be prompt to pick up your child at the end of class. Most classes are scheduled back-to-back,and we cannot babysit students. If they must wait, have them prepare ahead of time with some work to keep them occupied. The studio will not stay open more than 10 minutes after the last class of the evening. Students who did not attend school due to illness should not attend dance class. If your dancer shows signs of contagious illness fever, nausea, chills,etc please keep them home to get well, and keep from spreading the illness to the other dancers. Attendance is mandatory, and students are responsible for tuition, regardless of how many classes they attend. Excessive absences may result in removal from some or all recital choreography. It is the responsibility of the parent/guardian to be aware of all studio activities, such as observation days, recitals, extra classes, important days, and dates the studio is open or closed. Performing Hearts Dance Studio will post notices and information at the online website, Facebook fan page, through emails, and inside the facility, as well as, on occasion, sending home notes with the students. It is the parent's responsibility to check with the studio or Performing Hearts Dance Studio website/FB page to ensure they are informed. Make-up dance days will only be rescheduled once if a class needs to be made up (non-weather related), if an instance arises in which that make-up class is canceled it will not be made up again. It is also the parent's responsibility to inform Performing Hearts Dance Studio of any email address, home address or telephone number change, as well as using the parent portal to update the information. Also if you are not receiving emails you must contact PHDS.
IN STUDIO POLICIES: No eating is permitted in the studio. Please eat in the waiting area and clean up after yourself. Water is permitted for sipping while in class. No unattended children are permitted in the dance studios/warm up room without a teacher. Remember, no street shoes are permitted on the dance floor/warm up room floors as dirt, rainwater, grime and winter salt can cause the flooring to erode and will break down dance shoes. Please turn off cell phones prior to entering class, they are a distraction, they should be housed in your dance bag throughout your time in the studio space.
WAITING ROOM POLICIES: Please do not arrive more than 10 minutes before your dancer's class. Students with too much time before class tend to get restless in the waiting area and then tired in class. Horseplay is not tolerated in the waiting area or warm up room. It is distracting to the classes currently being held and has caused damage to the waiting area. The benches and chairs are for sitting on, please no standing on benches or chairs is permitted. Do not bring valuables to dance class. Performing Hearts Dance Studio is not responsible for items lost or stolen in the waiting area. No food, street shoes, jackets or dance bags/backpacks are allowed in the warm up room because of its small size.
PARKING LOT: DO NOT EVER LET YOUR CHILD PLAY IN THE PARKING LOT!! We often see parents chatting in the lobby after class while their child walks out the door without them. We also see parents chatting outside the studio while their child plays between cars or in the neighboring business parking. This is absolutely against studio policy. Children are not to be left outside without supervision or leave the building without an adult. Please do not leave your vehicle unattended in the neighboring businesses drive through. Driving/parking on the grass is not permitted. When weather permits, students and parents are welcome to utilize the studio grass and picnic table for outdoor waiting and recreation. Please be aware of vehicles pulling in and out of the parking lot.
Release of Liability: I have enrolled in a program offered by Performing Hearts LLC. I, for myself, my heirs, or my assigns, release Performing Hearts LLC, the owners, Allison Schuebel and Cally Padberg, their families, employees, helpers, and the owner of s248a CO RD A, Wisconsin Dells, WI 53965, from any claims, demands, and causes of action from my or the above named person participating in Performing Hearts LLC. I also release them from any liability now or in the future for any injuries at Performance Hearts LLC or at any time on the premises or in any activity sponsored, represented, or organized by Performing Hearts LLC. By signing this agreement, I hereby affirm that I have read and fully agree with the above waiver and studio policies.
I/we fully understand and acknowledge that: (a) There are risks and dangers associated with participation in dance events and activities which could result in bodily injury partial and/or total disability, paralysis and death; (b) the social and economic loses and/or damages, which could result from these risks and dangers described above, could be severe; (c) these risks and dangers may be caused by the action, inaction or negligence of the participant or the action, inaction or negligence of others, including, but not limited to, the Releasees named below; (d) there may be other risks not known to us or are not reasonable foreseeable at this time. I/WE accept and assume such risks and responsibility for the losses and/or damages follow such injury, disability, paralysis or death, however caused and whether caused in whole or in part by negligence of the Releasees named below. I/WE hereby acknowledge the THE ACTIVITIES OF THE EVENT(S) ARE VERY DANGEROUS and involve the risk of serious injury and/or death and/or property damage. Each of the UNDERSIGNED also expressly acknowledges that INJURIES RECEIVED MAY BE COMPOUNDED OR INCREASED BY NEGLIGENT RESCUE OPERATIONS OR PROCEDURES OF THE RELEASEES.
I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTEND MY SIGNATURE TO BE COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.
