THE MT. HOOD DANCE ACADEMY AND ITS REPRESENTATIVES ARE NOT RESPONSIBLE FOR INJURIES OR DAMAGES IN CONNECTION WITH THE CLASSES AND/OR ACTIVITIES OFFERED.
By signing below, I acknowledge that dance, like any physical activity, can result in physical or emotional injury and
therefore, I will not hold the Mt. Hood Dance Academy, and/or associated staff liable for injuries
incurred on the studio's premises, during any class(es), or at any outside performance arenas where MHDA is participating.

Registration & Payment Policies
• Annual Registration Fee: New Student registration fee & Returning students renewal fee is $35. Annual Registration is due at the time of enrollment.
• We do offer a Trial Class Fee of $25 which can be used towards your annual registration fee if the student chooses to enroll in classes after their trial class.
• Tuition is due on the 1st of each month. A Late Fee of $10 will be assessed after the 10th of the month.
• We accept Cash, Check, Debit, Credit and monthly auto-draft options.
• Tuition does not vary due to holidays or the number of weeks in a month. We do not pro-rate tuition, with the exception of mid-month Registration.
• Tuition is non-refundable. However, we do offer make up classes whenever possible and we will take consideration based on the situation as we do understand that circumstances can arise. Please see our Office Staff or your Instructor with specific questions.
• A minimum TWO-Week WRITTEN drop notice is required for a student to discontinue in their class(es). Sending an email or delivering a written notice to a MHDA Office Staff member are both acceptable.

General Studio Rules
• Classes run September through June, on a month to month basis.
• We also offer Summer Classes and Camps during July & August. Special pricing is available for summer term.
• Please arrive to class on time. Students participating in class warm-ups are important for their physical preparedness and progression.
• Please do not leave children unattended in our studio lobby.
• Our Instructors/Staff cannot be responsible for students left in the lobby or for the dancers once their scheduled classes are done.
• If your child is ill, injured, or going to miss their class please call or email our studio.
• Missed classes will not be pro-rated or refunded. However, we do offer make-up classes whenever possible. Please speak with our Office Staff or your child's Instructor for more details.
• If you child is injured for an extended period of time, please provide our office with a doctor's note stating when your dancer is cleared to return to dance.
• We do not allow animals in our studio lobby. We have students who have allergies and/or are afraid of dogs.

Use of Photographs
I understand and agree to my child's photograph being taken and used by Mt. Hood Dance Academy for their website, studio bulletin boards, and/or other promotional uses.
General Attire Information
Ballet Classes
Girls: Black leotard, pink tights, pink ballet shoes. Hair is required to be in a Ballet Bun.
Boys: White t-shirt, black tights or shorts, black ballet shoes.
Girls: Dance clothing of your choice and the required shoes (i.e., tap shoes, jazz boots, footpaws, or tennis shoe for hip-hop).
Boys: Comfortable athletic clothing and the required class shoes.

**All Hair must be pulled back out of the dancers face.
Performance Information
Performances:
•We host 2 performances a year; a Winter Show & our annual Spring Recital. There are additional fees that come with performances; costume and stage fees. Participation is optional for students (mandatory for competition dancers) but strongly encouraged for all.
•The Winter Showcase is scheduled in December, time & location TBA.
•The Spring Recital is scheduled in June, times & location TBA.
•Confirmation of your participation is due with your Costume Fee.
•Prior to our Spring Recital, we have a mandatory Dress Rehearsal for all students, at the Performance location.
***More detailed information will be given prior to each performance and prior to your commitment in participating in the show(s).
