Participation in physical activities can involve motion, rotation, and height in a unique environment and as such carries with it a certain assumption of risk. The undersigned and the participant(s) choose to voluntarily enter upon said premises under the control of said corporation, knowing their present condition and knowing that said condition may become more hazardous and dangerous during the time the participant or the undersigned is upon said premises. The undersigned and the participant(s) voluntarily assume any and all risks of loss, damage, or injury that may be sustained by the participant(s) and/or the undersigned or any property owner by them while on or upon said premises described above. The corporation may but shall not be obliged to carry insurance on the participant(s), and the existence of insurance shall not change, alter, or increase the liability of the corporation to the participant and the undersigned or affect the terms of this Release. In signing this Release, the undersigned acknowledges: A) That he/she has read thoroughly and understands completely, the terms of Registration and Release and signs it voluntarily. B) That the undersigned signing either for themselves, or as Legal Guardian is, in fact, the true and legal guardian and has the consent of the participant.
I, parent or legal guardian of the participant, a minor (hereinafter "Minor"), hereby grant the permission necessary to allow Minor to participate in cheerleading instruction to be conducted by Glory Cheerleading Experience, LLC. I, in my own behalf and on behalf of the Minor, further agree to release and to hold harmless Glory Cheerleading Experience, LLC, the Hosting site,(university, hotel, convention center, high school) on whose premises the instruction or competition will occur (hereinafter the "Location")the affiliates of Glory Cheerleading Experience, LLC and the Location, and the respective directors, officers, representatives, members, agents and employees of the Location and their respective affiliates (hereinafter collectively "Releasees") from any and all liability for negligence or any other claim judgment, loss, liability, cost and expenses (including, without limitations, attorney's fees and costs) arising out of or connected with the instruction, including any claim arising out of or connected with any illness or injury (minimal, serious, catastrophic and / or death) that the Minor may incur or sustain, all activities associated with the Glory Cheerleading Experience and while traveling to and from the site for the cheerleading event whether or not event actually occurs. I further expressly agree to indemnify and hold harmless Releasees and Releasees' heirs, successors, assigns, executors and administrators against loss from any further claims, demands or actions that may subsequently be brought by Minor or by any other persons on the account of damages of any character resulting to Minor in any way from the foregoing activities. I further agree to reimburse and to make good to Releasees any loss, or costs Releasees may have to pay as a result of any such action, claim, or demand. I, in my own behalf and on behalf of the Minor, hereby warrant that I have read this Liability Release in its entirety and fully understand its contents. I, in my own behalf and on behalf of the Minor, am aware that this Liability Release releases Releasees from liability and contains an acknowledgement of my voluntary and knowing assumption of the risk of injury or illness. I, in my own behalf and on behalf of the minor, further acknowledge that nothing in this Liability Release constitutes a guarantee that the cheerleading instruction or event will occur. I, in my own behalf and on behalf of the Minor, have signed this document voluntarily and of my own free will.
I, in my own behalf and on behalf of the minor, acknowledge and agree that such participation subjects Minor to possibility of physical illness or injury (minimal, serious, catastrophic and/ or death) and that I, in my own behalf and on behalf of the Minor, acknowledge that the Minor is assuming the risk of such illness or injury by participating in the activity. In the event of such illness or injury, I authorize Glory Cheerleading Experience, LLC to obtain necessary medical treatment of the minor and hereby, in my own behalf and on behalf of the Minor, release and hold harmless Releasees in the exercises of this authority. I further acknowledge and understand that I will be responsible for any and all medical and related bills that may be incurred on behalf of the Minor for any illness or injury that the Minor may sustain during the instruction or event and while traveling to and from the site for the instruction or event whether or not the instruction or event actually occurs.
All active students are required to have a credit or debit card on file. Accounts will be billed on the 1st of each month. For accounts not paid before the 7th of the month, the credit card on file will be automatically billed. If you wish to pay by a different form of payment (cash, check, or a different card than the one that's on file), you will have to do so before the 7th of the month. There is a $25 late fee for accounts not paid before the 7th of each month. There is a $35 fee for returned checks and rejected credit cards payments. A 15 day written/emailed notice is required to terminate billing. You are responsible for payment whether or not your student attends classes until you notify Glory Cheerleading Experience by email or in writing to drop a student from classes/team. Unfortunately, no refund can be made if termination notice is not received prior to 15 days of the next billing cycle.
Tuition is based on a flat monthly rate, and fees are not prorated for any vacation, sick, personal, gym closings, or holiday time off. If the tuition is not paid before the 10th of the month, your child will not be permitted to participate until the account is brought up to date. No more than one month will be allowed outstanding in your child's account. Accounts that become 30 days overdue will be considered for collections action. Guardians will be responsible for any fees incurred for collection of past due fees (i.e. court costs and legal fees).
I understand and agree that if I write a check with insufficient funds to cover it or am late with payments, that Glory Cheerleading Experience has the options to require automatic credit or debit card payments from that point forward.
There is an annual non refundable membership fee of $40 per athlete ($70 family maximum) due at the time of registration. This membership fee covers participants from May to May of the following year.
For Glory All Star participants, if for any given reason your child cannot finish the competition season, Glory Cheerleading Experience will not be able to refund any portion of money paid, and a quit fee of $150 or more will be due to cover fees paid on your child's behalf.
