Safety Concerns
As with any sport or physical activity, serious injuries and even death are possible. The studio makes every reasonable effort to protect students and visitors. However in the event of an
accident, parents agree to hold harmless the studio as allowed by law.
Payment
2020-2021 School Year: There is an Annual Registration Fee of $30.00 per family per year, with the year being from September to September. No registration fee is due for Summer Term students.
Tuition charges are added to accounts on 1st of the month, and parents should log in and pay online on or before the 7th of the month. Electronic payment is encouraged, and Electronic payments that fail on the 7th will be automatically attempted again on the 14th. Lessons cannot continue if payment has not been made.
Summer Term: All Tuition is due on or before July 1st and can be paid via debit/credit card or bank account in the Parent Portal. No registration fees are charged for Summer Term, and e-payment registration is not required.
Please note our payment and late fee schedule:
• 7 days before the 1st of the month, tuition charges will be posted and should be paid between then and the 1st. Failed payments are issued an email and text.
• 2nd week of the month (day 7-13) -Failed payments on the 1st will be attempted again- if payment is successful, no fees will be assessed. If tuition is still unpaid, it is now late and the account is past due, but no late fee will be assessed if paid in full by the end of this week. Electronic payments that failed will be attempted again a week after their scheduled date. Fees incurred by the studio as a result of a failed electronic payments will be added to the client's account and deducted from the electronic funds source.
• 3rd week of the month (14-20) -a late fee of $10.00 will be assessed for payment received in this week, and no more classes can be taken until the account is paid.
• 4th week of the month (21-27)-a late fee of $15.00 will be assessed for payment received in this week.
• 5th week (28-31)-a late fee of $20.00 will be assessed for any payments during or after this week, and students will not be allowed to attend classes until full payment has been made. No makeup classes will be offered if a student has missed classes due to unpaid tuition.
Reminders of late payments will no longer be sent out weekly to those whose accounts are past due, and late fees will automatically be assessed according to the above schedule without further notification. It is the responsibility of the client to log in to his/her portal and note past due balances.

Communication
The studio utilizes electronic means to communicate crucial information to parents. Parents have options for receiving these messages- via email, parent portal, Instagram, and Facebook. Studio parents are responsible for knowing and responding to messages received this way, and should be willing to commit to checking at least one of these methods regularly. Parents who opt-out of mass emails from the studio will not receive all the information needed to have a successful school year at DBAD.
Photography Opt-In
I understand that my child's image may be recorded via video or photography while inside the studio or at a studio event. I understand that those images may appear on the studio's website and social media and I agree to allow that usage. I agree that if I cannot allow that usage, I will contact the studio in writing to notify the staff. I understand that although in that case the studio will not use my child's image in any way, video recording and photography by other clients sharing the studio space is beyond the studio's control.
Diagnosis Disclosure
If your child has any sort of diagnosis, be it cancer, ADHD, dwarfism, or seasonal allergies, please disclose that to us if you feel you can. We commit to keeping information of this type private, and can often help your student learn more if we know what they face daily. Use the medical portion of the web registration form or update your child's profile in the Parent Portal to share with us. All of the Dallas Ballet Faculty have training in different pedagogies, learning styles and student challenges- we are a school and we want to help your child learn and feel safely supported!
COVID19 Waiver
Thank you for your continued support and trust in Dallas Ballet & Academy of Dance (DBAD). As with the transmission of any communicable disease, you may be exposed to COVID-19, also known as the "Coronavirus", at any time or in any place. Be assured that we have always followed state and federal regulations to limit transmission of all diseases in our office and studios and continue to do so.
Despite our careful attention to sterilization, disinfection, and use of personal barriers, there is still a chance that you or your children could be exposed to an illness in the studio, just as you might be at your gym, grocery store or favorite restaurant. Social distancing has reduced the transmission of Coronavirus. Although we have taken several measures to provide social distancing and contactless participation, please know that participation in DBAD instructional classes is at your own risk.
Although exposure is unlikely, do you accept the risk and consent to participation?

I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin HEREBY RELEASE AND HOLD HARMLESS Dallas Ballet & Academy of Dance and their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("RELEASEES"), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IF FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
Costumes and Performances
2020-2021 School Year, we do not plan to order or charge for costumes.
In a "usual" school year: Typical total costume cost for a student in Creative Movement, PreK Tap, or any level I or II class is $50 per year per class (Sept. to Sept.). Typical total costume cost for students in level III and above classes is $75. Participation in the performances that costumes are needed for are optional, but class curriculum focuses on learning dances for those performances as they are our recitals. Paying your costume deposits ($25 per student per class due September 1st or at time of registration if registering after September 1st) denotes that you will also pay balances due, and that your student will participate in the annual performances. Our annual performances are held in December and June. A schedule of costume balance and deposit due dates is provided to every family who registers.
No Costumes are required for Summer Term. There is no recital for Summer Term, but we will hold Parent Viewing Days on the last day of each dance camp, and in week 6 of the term, for regular classes. Summer Term students are also invited to join us for the Summerfest Parade on the studio float.
Discontinuing Class
Students may drop class at any time, but parents/guardians must notify the studio directly to avoid further tuition payment. Tuition will be charged until the actual day the studio is notified directly by the parent/ guardian either in person, by phone or in writing that the child will no longer be attending class. Simply not showing up for classes is insufficient as notification, and tuition will be charged until personal notification has been made. Refunds on tuition will not be given for any classes that were missed before the studio received personal notification that the student is discontinuing classes.
Observation Days
2020-2021 School Year: Parent and Family Observation Days are not possible during the COVID19 Pandemic. Instructors will hold one Observation Week per term via Zoom, and families can log in from home to see how their student is progressing.
In a typical school year: Like many other local studios, Dallas Ballet & Academy of Dance has the policy of a once quarterly "Observation Day". On those special days, we invite parents, guardians, grandparents, etc. who would like to stay and watch their students in class to please do so. This gives you a great opportunity to see how much fun they are having in class! For classes during the rest of the term however, parents are required to please leave the studio during the student's class period. This allows for a more focused lesson, where the students are not distracted by having other people in the studio watching them. Not all students are ready for this element of the educational process, and may need to reconsider taking dance classes when they feel more ready to separate. We understand a student might have extenuating circumstances that might make it impossible for a guardian to leave the studio. If this is the case with your student, please see Miss Stephanie to discuss the situation.
The last week of classes for Summer Term and the last day of any Dance Camp are set aside as Observation Days.
