We, the staff of Rising Star Gymnastics recognize our obligation to make our students and their parents aware of the risks and hazards associated with the sport of gymnastics. Students may suffer injuries, possibly minor, serious, or catastrophic in nature. Gymnastics can be dangerous and lead to injury. Parents should make their children aware of the possibility of injury and encourage their children to follow all the safety rules and the coaches' instruction. Rising Star Gymnastics, its coaches and other staff members, will not accept responsibility for injuries sustained by any student during the course of gymnastics instruction, or open workouts or in the case of any exhibition, competition, or clinic in which he or she may participate while traveling to or from the event. With the above in mind, and being fully aware of the risks and possibility of an injury involved, I consent to have my child or children participate in the programs offered by Rising Star Gymnastics. I, my executors, or other representatives, waive and release all rights and claims for damages that my child or I may have against Rising Star Gymnastics and/or it's representatives whether paid or volunteer. I also affirm that I now have and will continue to provide proper hospitalization, health, and accident insurance coverage, which I consider adequate for both my child's protection and my own protection. I also understand that it is the parent's responsibility to warn the child about the dangers of gymnastics and injury. Rising Star Gymnastics will only warn the child through "Safety Messages" and our teaching style and progression.
I represent and warrant that if I am purchasing something or paying for a service from this facility or from other merchants through this facility that (i) any credit card information I supply is true and complete, (ii) charges incurred by me will be honored by my credit card company and (iii) I will pay the charges incurred by me at the posted prices, including any applicable taxes, fees, and penalties.
I hereby authorize this facility to charge my credit card. I understand that a 14 day written notice is required to terminate billing and I am responsible for payment whether or not my student attends classes until I notify this facility in writing to drop my student from class(es).
Should I dispute a charge through my financial institution this will constitute a breach of contract possibly resulting in, but not limited to, penalties, additional fees, collection, legal action, and/or termination of any and/or all current and future services.
Tuition is based on a monthly membership and is due the 1st of each month. Tuition may be prorated during the first month of membership only. Tuition will automatically be charged to the credit or debit card on the 1st day of each month. Withdrawals from program require a two week advance notice or tuition will be billed for the following month through the end of the season or until we receive your written drop notice in the gym office, whichever comes first.
The annual membership fee of $20 is due at the time of registration, and annually thereafter.
I understand that there are no refunds offered for tuition, competition fees, apparel, or any other associated expense for classes or team participation.
