Covid-19 and General Studio Liability Release Waiver (please type full name below) *
I release Corona Dance Academy, it's instructors, independent contractors, and all other associates from liability for harm, theft, or injury that may be suffered by me and/or members of my family traveling to or from or during participation in activities and programs sponsored by Corona Dance Academy. I hereby acknowledge that I am voluntarily assuming full responsibility for all risks of physical injury arising out of active participation in a dance class or other dance related activities. I acknowledge the contagious nature of COVID-19 and other contagious diseases and viruses and voluntary assume the risk that I and/or my children may be exposed to or infected by COVID-19 by attending and participating and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 and other contagious diseases and viruses may result from the actions, omissions, or negligence of myself and others, including, but not limited to, employees, independent contractors, volunteers, and program participants and their families. I release Corona Dance Academy, its instructors, independent contractors and all associates from liability for harm, injury or death pertaining to COVID-19 and other contagious diseases and viruses. I understand that there are NO refunds for registration fees, costume, performance fees or classes. I understand that if my dancers class gets cancelled due to weather, sickness, or any issues arising from COVID-19 there is no refund for these missed classes. I understand and agree to Corona Dance Academy policies and procedures. I understand that my student must have this release form signed before attending class. I give permission to Corona Dance Academy to use pictures and or video of my child for advertising purposes in print, promotional videos and on the website.
Studio Dress Code
Dancewear is required for ALL classes! There are NO exceptions. Being dressed properly will enable the instructor to correct body placement. It also allows students to feel equally part of a group. The Dress code is as follows and will be enforced:
BALLET- black leotard, pink full-footed tights, hair neatly secured away from face in a bun. Upper level (Ballet 3/4 and up)- any colored leotard, pink full-footed tights and ballet skirts are permitted. Leather or canvas split-sole ballet shoes.
JAZZ/ LYRICAL/ CONTEMPORARY- Any color leotard, tan or pink tights. No baggy pants, t-shirts, leggings or dance shorts are permitted. Hair neatly secured away from face in a bun or pony tail. Tan or black leather jazz shoes or canvas eclipse preferred.
TAP- Any color leotard, tan or pink tights. No baggy pants, t-shirts, leggings or dance shorts are permitted. Hair neatly secured away from face in a bun or pony tail. Tan or black leather lace up tap shoes preferred.
HIP HOP- Knee pads are optional. Hair must be neatly secured away from face in a bun or pony tail. Tied Tennis shoes required. Leggings, or baggy sweat pant that covers the knee preferred. T-shirt, with no distracting logos or slang permitted.
MUSICAL THEATER- Any color leotard, tan or pink tights. No baggy pants, t-shirts, leggings or dance shorts are permitted. Hair neatly secured away from face in a bun or pony tail. Character shoe, Tan or black leather jazz shoe recommended.
Registration & Tuition
A $30.00 fee per student/ family is required per dance season. A registration is not considered complete and class space is not reserved until payment and tuition or pro rated amount is received in full. All tuition installments are due by the 1st of each calendar month of the dance season. Tuition paid after the 10th of each month will be considered late regardless if you have or have not been to class. A 10% discount will be applied to the 2nd sibling taking the lesser amount of classes. Tuition is NON-refundable! Bills are not given unless there is an outstanding balance. All holidays are calculated into the tuition rates.
Attendance
Good attendance is imperative, as absences and tardiness can hold back an entire class, and the studio cannot jeopardize its responsibilities to the rest of the class for one student. Please make every effort to have your child at every class. Make-up classes are valid for up to 60 days from the date of the missed class. Make-up classes are NOT transferable to other students or sessions. Students missing more than two classes per month may lose their place in a class for that enrollment period.
Private Lessons
Private lessons are offered to currently enrolled students and are available at an extra charge of $50.00 per half hour or $100.00 per hour to learn a routine for competition, extra technique help, or extra help if classes have been missed, etc. You must cancel 24 hours before a scheduled lesson or you will be charged for missed lessons. Private lesson payments are due in advance.
Responsibility for Safety
There are certain risks that are inherent in any dance activity. Each Corona Dance Academy member, guest, or nonmember participant agrees to accept personal responsibility for his or her safety. Corona Dance Academy cannot ensure the safety on any participants. In participating at Corona Dance Academy activities, each such person agrees to hold harmless and free or blame the instructors, and the Corona Dance Academy, it's officers and members, for any accident, injury, lost or damaged property, or illness that which might be sustained from their participation. Pictures or video taken during performances and class time may be used for publicity purposes in brochures, emails as well as our website.
