Participation Waiver
I, in my legal capacity as the parent/guardian of the minor(s) named in this registration ("Minor"), acknowledge and agree that any use of Algonquin Academy of Dance and Music, LLC and Revolution Dance Company facilities, services, equipment and premises ("Facilities") and any participation in AADM and RDC programs and activities ("Programs") comes with inherent risks including, but in no way limited to: (1) moderate and severe personal injury, (2) property damage, (3) disability, (4) death, and (5) sickness or disease. I voluntarily, for myself and Minor, accept and assume full responsibility for these risks as well as any and all other risks of the use of Facilities and participation in Programs. I agree that I have full knowledge of the nature and extent of all such risks and am not relying on all such risks being described in this document.
In consideration of Minor's use of Facilities and participation in Programs, I, in my legal capacity as parent/guardian of Minor, agree on behalf of myself and Minor that AADM/RDC, it's officers, directors, agents, employees, volunteers, insurers, and representatives ("Releasees") with not be liable for any personal injury, property damage, disability, death, sickness or disease incurred by Minor, however occurring including, but not limited to, the negligence of Releasees. I understand that Minor and I will be solely responsible for any loss or damage, including personal injury, property damage, disability, death, sickness or death sustained from the use of Facilities and participation in Programs.
I further agree, in my legal capacity as the parent/guardian of Minor,on behalf of Minor, myself, and any and all legal successors and pixies, to release and HERBY DO RELEASE, WAIVE AND COVENANT NOT TO SUE Releases from any causes of action, claims, suits, liabilities or demands of any nature whatsoever including , but in no way limited to , claims of negligence, which Minor, myself, and any and all legal successors and proxies may have, now or the future, against Releasees on account of personal injury, property damage, disability, death, sickness, disease or accident of any kind, arising out of or in any way related to the use of Facilities or participation in Programs, whether that participation is supervised or unsupervised, however the injury or damage occurs, including, but not limited to , the negligence or Releasees.
In further consideration of the use of Facilities and participation in Programs, I, in my legal capacity as parent/guardian of Minor, agree on behalf of myself and Minor to INDEMNIFY AND HOLD HARMLESS Releasees from any and all causes of action, claims, demands, losses, suits, liabilities, or costs of any nature whatsoever,including claims of negligence, arising out of or in any way related to the use of Facilities and participation in Programs.
By accepting the agreement, you acknowledge that you are the party financially responsible for this family's account and are legally authorized to accept the responsibility pertaining to the students of this family. You are stating that you have read, understand and agree to all statements in this Waiver for Dance Students.
Financial Agreement
DURING FALL SESSIONS, a yearly $50 non-refundable family fee will be charged (The fee covers 1 dancer registration and recital video fee). An additional $20 per dancer will be charged after the first dancer. Tuition in the fall is based off of 36 weeks, with 9 equal monthly payments being run on the 25th of each month from August to April. If tuition is not collected by the 1st of each month, a $25 late fee will be added to past due tuition. After the 15th of the month, a dancer will not be able to participate unless the account is brought up to date. At any time if you decide to stop dance classes, you must notify us in writing. Withdrawals require a 4 week notice, and any outstanding amounts due will be collected when the withdrawal is communicated. Tuition will continue to be charged regardless of attendance if we do not receive written notification. No withdrawals will be processed after March 31st during the fall/spring session.
DURING SUMMER SESSIONS, At the time of registration a one time session payment will be due. Due to the short length of the session (6 weeks), no refunds are given for summer sessions.
Registration fees (Fall) or summer tuition fees will be charged once this form is submitted.
