By clicking the box below, you are consenting to electronic signature in regards to Mint Hill Dance Center registration and participation. Electronic registration also consents to communication electronically by allowing Mint Hill Dance Center to send you notices, forms, and other notifications in an electronic format. Clicking below also consents that your electronic signature is equivalent to written signature.
Covid Acknowledgment and Release
I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.
I further acknowledge that Mint Hill Dance Center has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.
I further acknowledge that Mint Hill Dance Center can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, staff, and other clients and their families.
I voluntarily seek services provided by Mint Hill Dance Center and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment.
I attest that:
* I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
* I have not traveled internationally within the last 14 days.
* I have not traveled to a highly impacted area within the United States of America in the last 14 days.
* I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
* I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.
* I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.
I hereby release and agree to hold Mint Hill Dance Center harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the dance center, or that may otherwise arise in any way in connection with any services received from Mint Hill Dance Center. I understand that this release discharges Mint Hill Dance Center from any liability or claim that I, my heirs, or any personal representatives may have against Mint Hill Dance Center with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Mint Hill Dance Center. This liability waiver and release extends to the dance center together with all owners, partners, and employees.
MHDC Handbook
Mint Hill Dance Center requires every parent, dancer, and guardian to read and understand the rules and policies listed in the Mint Hill Dance Center Handbook. The Mint Hill Dance Center Handbook can be found on the Handbook tab of the website at www.minthilldancecenter.com/handbook.
By clicking below, you agree you have received, read, and understand the rules and policies and were provided a copy of the Handbook. By clicking below, you also acknowledge parents, guardians, and dancer will comply with all rules and policies set forth by Mint Hill Dance Center and any failure to comply will result in suspension or expulsion of the dancer from Mint Hill Dance Center classes and activities. By clicking below, I also agree that the MHDC Handbook can be updated at any time and policies can change at any time at the discretion of MHDC.
Payment Polcies
For 2021 Summer Enrollment: By enrolling online, I agree by providing a credit card/debit card on file in my account will be the main form of payment unless otherwise in written declaration provided by MHDC (form located in office). I agree class tuition or camp tuition and all other fees (including but not limited to class tuition, merchandise, and more) are non-refundable as stated in the MHDC Handbook. I agree to provide my card and to have it automatically used for e-payments for tuition per class or camp, run immediately for e-payment for any merchandise fees that I sign up for or purchase in the MHDC office. I agree to withdraw payment policies as stated in the MHDC Handbook.
A late fee/declined card fee of $15 will be applied to the account if a card is declined payment for any reason.
Low Enrollment Policy
Mint Hill Dance Center holds the right to cancel classes based on low enrollment. If in the event of a canceled class, a dancer may transfer into another class in the corresponding level and age range appropriate, with approval of teacher.
Pick Up/Drop Off/Unattended students or children
Common areas of Mint Hill Dance Center outside of the dance studio classrooms 1&2 are not supervised by staff at all times. A parent or guardian must supervise student and siblings with student at all times. Students should not arrive more than 10 minutes before scheduled class, students also should be picked up no later than 10 minutes following class. Any early arrival or late pick up more than 10 minutes will result in a babysitting fee of $15 for every 10 minutes prior or following the 10 minute window allowed.
Photography and Video Release Policy
By clicking below, permission is given to Mint Hill Dance Center to use and publish for editorial, trade, advertisement, and other promotional purpose all photographic, video, and digital images of my child, including the child's name in conjunction with Studio and School Name. All parents who do not wish their child's image or name to be used in this manner must submit so in writing to Mint Hill Dance Center.
Acknowledgment and Release
By clicking the box below, you have read and understand the following statements:
1- I recognize and understand the risks of physical injury inherent to dance and dance training and I fully assume those risks. I hereby release Mint Hill Dance Center, its affiliates dance studios, event sponsors, employees and dance teachers from all liability for injuries sustained or illnesses contracted while attending or participating in any dance classes, rehearsals, workshops, or performances. I agree to indemnify, defend, and hold harmless Mint Hill Dance Center, its affiliate dance studios, event sponsors, employees and dance teachers for liabilities, costs and judgments arising from acts of omissions committed by me or my child which result in injury or damage to any person or property.
2- I understand and agree that it is my sole responsibility to safeguard my personal property while attending or participating in any classes, rehearsals, workshops, or performances. I hereby release Mint Hill Dance Center, its affiliate dance studios, event sponsors, employees and dance teachers from all liability for loss or damage to my personal property while attending or participating in classes, rehearsals, workshops, or performances. I also agree to abide by any rules, regulations and policies set forth by Mint Hill Dance Center.
3-In case of physical injury or medical emergency, I hereby authorize Mint Hill Dance Center to make necessary arrangements to transport myself or my child to a medical treatment facility as necessary. All such transportation and medical treatment will be at my sole cost and expense. In extreme emergency, or if my child is under 18 years of age, I understand that Mint Hill Dance Center will attempt to notify the person(s) I have named below as my emergency contact(s) of my condition and how to reach me.
4-In signing this Release, I acknowledge and represent that I have fully informed myself of the content of the waiver and hold harmless agreement by reading it before I sign it, and I understand that I sign this document as my own free act and deed; no oral representations, statements, or inducements, apart from the written statement, have been made. I further state that I am at least eighteen (18) years of age and am fully competent to sign this agreement; and that I execute this release for full, adequate, and complete consideration fully intending to be bound by the same. I further state that there are no health-related reasons or problems which preclude or restrict my or my child's participation in this activity, and that I will pay any medical costs that may be attendant as a result of injury to me or my child.
Dress Code
Dress Code is listed on our website and in our Handbook. Summer Session also follows dress code for classes. Dress code is:
Dress code for all classes is a leotard, tights, specified class shoes, and hair in a bun. Only cover up options are dance shorts or ballet skirts. NO EXCEPTIONS! Jewlery is not allowed in class for safety. Shoes listed below:

All dancers and parents must abide by MHDC Handbook in regards to dress code.
Absences and Inclement Weather
Inclement Weather: Incase of inclement weather, Mint Hill Dance Center will generally follow the decisions of Charlotte-Mecklenburg School system. There will also be information regarding inclement weather on the website, on social media (Facebook and Instagram), and by calling the studio.
Absences:
Illness: Parents are advised to keep their student at home for the following conditions: Runny Nose (especially green mucous), persistent cough, or sore throat, difficulty breathing, fever, infected skin or rash., itching of body or scalp, nausea and vomiting, and any other symptoms of illness that has not been mentioned above.
Missing Class: If a student will be missing multiple weeks of classes, please contact the studio and the information will be relayed to the instructor.
Make Up Classes: THERE ARE NO REFUNDS OR CREDITS FOR MISSED CLASSES. CLASS IS HELD WEEKLY. MISSING CLASS DOES NOT DENOTE LACK OF PAYMENT. Missed classes can be made up at any time before May in an age appropriate class and it may be in a different style than what the student is taking.
Withdrawl Policy
To withdraw a student from MHDC classes and programs, one must do so in writing with a 30 days notice. Failure to attend class does not constitute withdrawal from MHDC. Students and parents are required to submit a withdrawal form with a 30 days notice and are responsible for all tuition and fees incurred during the 30 day time frame. If costumes have already been ordered, costumes may be picked up at MHDC office when costumes arrive.
