By completing and signing this form, I authorize Masterpiece Studios to charge my bank account upon submitting this form for electronic debit. I understand that the first withdrawal will include a non-refundable registration fee, the 1st monthly tuition and art supply fees if applicable. Thereafter, I authorize Masterpiece Studios to charge my bank account on the 15th day of each month for tuition payment of dance, tumbling and/or visual art classes, costume & performance fees, and art supply and gallery fees if applicable. I agree that no prior-notification will be provided if the total payment is under the stated monthly tuition. I will receive advance notice of the charge if it exceeds the monthly tuition amount. If the above noted payment date falls on a weekend or holiday, I understand that the payment may be executed on the next business day. I understand that because these are electronic transactions, these funds may be withdrawn from my account as soon as the above noted periodic transaction dates. WITHDRAWAL NOTICE: I understand that this authorization will remain in effect until I cancel it in writing or until June 15th (whichever comes first), and I agree to notify Masterpiece Studios in writing of any changes in my account information or termination of this authorization at least 30 days prior to the next billing date. NSF FEE: In the case of an ACH Transaction being rejected for Non Sufficient Funds (NSF) I understand that Masterpiece Studios will attempt to process the charge again within 30 days, and agree to an additional $25 charge for each attempt returned NSF which will be initiated as a separate transaction from the authorized recurring payment. I acknowledge that the origination of ACH transactions to my account must comply with the provisions of U.S. law. I agree not to dispute these scheduled payments with my bank so long as the transaction corresponds to the terms indicated in this form.
CELL PHONES: Cell phones are not permitted in class
FOOD & DRINKS: Food and drinks are permitted in the lobby but not in the dance studios (please clean up after yourselves). Water is the only beverage allowed in the dance studios.
NUT-FREE FACILITY: Masterpiece Studios is a NUT-FREE facility. However, Masterpiece can not guarantee it will be free from all foods containing nuts.
RESTROOM: Please make sure your student has used the restroom BEFORE class starts.
CLASS INTERRUPTIONS: Please do not enter the studios during class. Instructors will assist in shoe changes for the little ones.
ATTENDANCE & PUNCTUALITY: Please make every effort to have your child in class promptly each week. Frequent tardiness and absences create problems for the student, the instructor, and the classmates. When unavoidable absences arise please call or email the studio before your child's class time to inform us. Please do not drop your child off early unless you plan to stay with him/her.
COMMITMENT: For each child to learn commitment and perseverance, we believe it is important he/she finish out the studio year. It is in this attitude of respect for our studio, our instructors, and our fellow classmates, we ask that you not "drop out." If there is some problem with your child's class, please contact us and we will make every effort to resolve the issue.
DANCE: Leotard and tights are required during dance class for students to receive correct positioning in dance techniques. No midriffs should be shown at any time. Proper dance shoes are required to participate in class. Required dance shoes are listed on the website, schedule and parent portal and can be ordered on www.curtaincallforclass.com, Passphrase: Masterpiece Studios #1. Jewelry, gum, and street clothes are not permitted during dance class. Ballet skirts are optional. Hair should be worn away from the face. Hair should be worn in a bun (with no hang downs) for Ballet. Please label all shoes, dancewear, dance bags, coats, water bottles, etc. with the student's first and last name.
VISUAL ART: Students should wear old clothes. Some materials used in the studio might stain. All materials used are non toxic and safe, but if you or your child has any known allergies or medical conditions please let our staff know.
WHAT LEVEL TO REGISTER FOR:
Under the age of 10, you register for the corresponding age w/the exception of tumbling.
For tumbling and 10+ classes, you register for the same level you were in the previous year unless you have been instructed otherwise. When we determine placement we look at technique, maturity, attitude, perseverance, and motivation. The number of years you have danced and your age are not the determining factor in determining what level you are in. We take placement very serious and look at it from many different angles. Thank you for trusting us to make the best decision for your student and the entire studio.
Levels II, III, IV, and company are by INVITATION ONLY. Please do not register for those levels unless you were in one of those levels the previous year or you have been invited to join them this year.
Levels III and IV modern and jazz require that you take ballet.
