LEGALLY BINDING AGREEMENT
I understand that this agreement is legally binding whether my use of the facilities and its services is determined and paid on a yearly, monthly, weekly or individual basis.
I acknowledge that increases in periodic payments may occur and patrons will be given a minimum 30 days notice if this is to occur.
PAYMENT POLICY
When enrolling, we require an Enrolment fee of $60 and a pro - rata of the monthly fees determined by the commencement date. We require payment each month before the start of the month to book your child's lessons.
Fees are due on the 25th of each month for the following month.
DIRECT DEBIT POLICY
Payments are charged monthly on the 25th of the month for the following month.
Any debits that fall on public holidays will be initiated the previous business day.
Please allow three (3) business days from the scheduled debit date for your payment to clear.
*Should there be a failed direct debit payment from your account a $10 Administration Fee & a Merchant Fee will be charged.
*A late fee of $10 will be charged for any account not paid before the 1st of the month.
*If fees are outstanding past 14th of the Month your account will be sent to a debt collector and 25% of the total amount+GST will be charged to your account.
* If your account continues to be unpaid after 30 days your swimmer will be dropped from the program.
WITHDRAWAL
Cancellations require 4 weeks notice and must be given BEFORE the 15th of the month.
A Signed Cancellation Form our centre must be submitted. No Verbal withdrawals or messages on social media are accepted. Should you withdraw without written notice, you will be charged 4 weeks lesson fees according to your swimmers level. Management reserves the right to cancel any enrolment.
MISSED LESSON POLICY
If PDSA deems it necessary to cancel a lesson due to extenuating circumstances we will contact you giving as much notice as possible. A make up will be applied to cancelled lessons that PDSA cancels.
Our missed lesson policy is as follows:
In the event you suspend lessons with 14 days notice, PDSA will only charge a $10 suspension fee per lesson missed to a maximum of 2 consecutive lessons within a 4 week period, not the full lesson fee. PDSA allows up to 4 suspensions per year.
Cancellations due to illness will be charged and a make up lesson (up to 8 per year) will be provided if PDSA are advised via email or phone call before 9:30am weekdays on the day of the lesson or 8:00am if lesson is on a weekend and a medical certificate is provided dated during the period the child falls sick.
Even in the case of no show, if there has been no or insufficient notification of a cancellation your lesson will be charged and no make up lesson will be offered.
Make up classes are a courtesy and are not guaranteed.
We will not re-schedule any makeup classes i.e. no make ups for your make ups.
If you no show for your makeup you will be charged a $10 fee.
Makes ups are NOT valid if you have outstanding fees.
**As per NSW health guidelines, cases of gastro require 2 weeks exclusion from swimming. Please provide a medical certificate and 2 make up lessons will be issued.
Our missed lesson policy is as follows:

• Cancellations due to illness will be charged and a make up lesson will be provided if a medical certificate is provided and we are advised via email or phone call by 9am weekdays on the day of the lesson, 8am on Saturdays.
• Even in the case of no show, if there has been no or insufficient notification of a cancellation your lesson will be charged and no make up lesson will be offered.
• Make up lessons must be used within 2 weeks of the missed lesson.
Make up lessons do not roll over from one year to the next.

SWIM NAPPY POLICY
Neoprene swim nappies are compulsory for ALL participants under the age of 3 or any child who is not fully toilet trained.
Participants arriving to class without a swim nappy will be required to purchase one before entering the water. In the event that your child poo's in the pool when not wearing the required swim nappy, you will be required to pay the pool cleaning fee of $400.00 at the time of the accident.
A $200.00 cleaning fee is required in the event if vomit contamination.
SWIMWEAR
Rash vests & board shorts are not appropriate swimwear for lessons as they weigh children down in the water.
Swim caps are compulsory for all swimmers. We do not lend goggles, swim caps or swimwear due to NSW health regulations. All swimmers must wear goggles for their lesson. We prefer that children do not wear jewellery, watches, make up, coloured hair spray or face paint in the pool. All swimmers in our stroke correction program require Flippers for every lesson.
COMMUNICATION
We do our best to communicate with our parents. Please return our calls when possible. Download our app, read our emails, Facebook updates and newsletters. Teacher changes will be displayed at reception.
POOL DECK SAFETY
Strollers are not permitted in the centre due to occupational health and safety regulations deeming fire evacuation directions not be blocked. Food is not permitted in the Swim Centre.
PARKING POLICY
Ample off street parking is available at our location. We take no responsibility for your car when you park either on the street or parking lot.
LOST PROPERTY
Puddle Ducks Swim Academy are not responsible for lost property. Lost property is not held at the centre and is given to Charity. Drink bottles are thrown away each day.
PHOTOGRAPHY/PROMOTION
I give permission for myself/my child to be photographed/videos being used for publicity purposes while in a controlled environment. This includes social media advertising.
All personal information complies with the Puddle Ducks Privacy Policy.
Please advise PDSA if you do not give permission to video or photograph your child.
COMMITMENT TO CHILD PROTECTION
Puddle Ducks Swim Academy will uphold practices that promote the safety, welfare and wellbeing of children and young people. All patrons and guests must behave accordingly.
RELEASE
I acknowledge that during all such times as I am on the premises of or included in any activity for me or my child to take part in, both my property and person shall be at risk and I will not hold Puddle Ducks Swim Academy liable for any personal injury or loss of property which may arise from negligence of Puddle Ducks Swim Academy, its servants, agents, contractors, voluntary workers, other users of the facility or participants in the activities or spectators or other parties providing services through on in the facilities of Puddle Ducks Swim Academy.
I also warrant that I/my child is physically fit and able to engage in exercise and fitness programs at the facility.
I give permission for medical/ambulance assistance in the case of an emergency and agree to pay such costs incurred.
MUTUAL RESPECT
I acknowledge mutual respect is practised in all forms of communication at Puddle Ducks Swim Academy.
By enrolling my swimmer at Puddle Ducks Swim Academy I will respect the staff and swimmers at the Centre.
If I yell, swear or disrespect any staff or swimmer at Puddle Ducks I understand my childs enrolment will be taken into consideration and could be terminated immediately.
