Disclaimer and Medical Release
Student and Parent/ Legal Guardian understand and expressly assume all risks involved in connection with dance instruction, tumbling, gymnastics, rehearsal, performing, and training at the Studio including but not limited to risk of bodily injury occurring as result of contact with other students, instructors, equipment, floors, structures, poles, and any other object located in or near the dance studio, or the students physical condition or physical limitations.
Student and parent waive claims arising out of dance instruction, rehearsal, training, and performances at the studio and or other venues and or performances spaces, whether caused by negligence, breach of contract, or otherwise and whether for bodily injury, virus, property damage, or loss or otherwise, which student may have against the Studio, its owners, officers, shareholders, director, employees, and agents, and they heirs, executors and administrators.
I/we agree to release and hold harmless Sampul Dance Academy, LLC including its teachers, dancers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold Sampul Dance Academy, LLC liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by Sampul Dance Academy, LLC.
We DO NOT give refunds for missed classes, registration fees, camps, costumes, apparel, classes missed due to natural disasters, or unforeseen events including pandemics and wildfires. We do however allow makeup classes to be made up within one month time period from the missed class in the students particular level. We can not transfer tuition payments or makeups to another student or family.
Safer Studio Policy:
SDA requires all staff and students to stay home when they are ill. In order to reduce community spread Coronavirus, we follow best practices and recommendations from our local health department and from our industry associations, YPAD and More than Just Great Dancing. SDA understands that unlike older children and adults, young children cannot be expected to maintain social distancing at all times. Therefore SDA focuses on a hierarchy of measures beginning with keeping ill people home, followed by frequent hand cleaning and sanitizing, amplified cleaning, and curricular and facility adaptations to minimize contact and mingling where possible. All studio classes are backed up on an online learning platform. In the event a class is unable to be held at the studio classes will convert to online learning platform until in-studio classes can resume.
Photo/ Video Release
The Parent/ Legal Guardian grants the studio the right to use and incorporate the use of photographs and/ or videotapes of the student for publicity, social media, marketing, or display purposes. Studio is granted the right to use said images for any purpose it deems appropriate and will not be held in violation of privacy rights. If you do not wish to have your students photo published you can fill out a form at the desk or send us an email to sampuldanceacademy@gmail.com.
Tuition Policy/ Costumes/ Class Size Information
Tuition is due on the 1st of every month. If payment is not received by the 3rd of the month the card on file will automatically be charged and a late fee of $15 will be added to the tuition amount.
There is a $25 fee for bounced checks.
It is understood that there is an annual non refundable registration fee of $30.00 which is due at the first enrollment of the student each year.
There will be no refunds/ deductions for missed lessons. There will be no refunds due to cancelations from inclement weather or wildfires. There will be no refunds in the event of a pandemic or for classes that are converted to online in the event of governmental advisory. Tuition is non refundable and non transferable unless there is a valid document or documented medical condition or emergency .Tuition is charged monthly on the 1st of each month. There is no pro-rating for months with 3 weeks or those with 5 weeks.
If you choose not to continue at any time, you must fill out an ADD/DROP form by the 20th of the month or send an email to sampuldanceacademy@gmail.com in order to stop the recurring charge from being billed for the next month. Any ADD/DROP forms filled out or emails sent after the 20th will result in the recurring charge being processed as usual.
There are no credits, refunds, or extensions for holidays or missed classes. We do not prorate if you are absent or away on vacation, or for regular studio closures but we do allow makeup classes within 30 days of your missed class. You may make up any class that is at your specific level. Ex: Miss a jazz class in level 1, you can make up that class in a level 1 hip hop class.
A recital may be given at the end of Spring semester. Participation is encouraged but not required. There will be an additional fee for costumes, tickets, video, etc.
This agreement shall be governed by and interpreted under the laws of the State of California. This agreement shall be binding upon and cannot be assigned to another. The failure to attend class does not waive the provisions of this contract.
It is understood that Sampul Dance Academy has the right to cancel a class with less than three students enrolled after a months time period and will notify the parents as well as recommend a class for the student(s) that is/are enrolled. If there are two or less kids in the class the duration of the class will run for 30 minutes.
We do not give refunds on recital fees, recital costumes, competition costumes, competition fees if already submitted, or apparel under any circumstances.
This agreement is understood with respect to the subject matter and must be signed at time of registration.
