REGISTRATION POLICY: Classes at Elite Performing Arts Center (EPAC) begin on September 7, 2019 and end June 20, 2020. At registration, the first tuition installment (for each child), 10th tuition installment (for each child), and a $25 registration fee per family is due. The $25 registration fee is non-refundable. After October 1, 2019 or 30 days after your registration date, the 10th tuition installment is non-refundable for any reason. A valid registration, including full registration payment and all signed forms, is required to guarantee a spot in a class. No class changes will be allowed after December 1, 2019.

TUITION PAYMENT POLICY: Monthly tuition installments are divided into 10 monthly installments. These installments do not vary with short or long months, school recesses, or anticipated vacations since they are calculated on a yearly basis and conveniently divided into 10 monthly installments. It is recommended that a credit card be placed on file and will be charged for any open account charges that come due including but not limited to: tuition, costumes, clothing and shoes, competition fees, private lessons, etc. Tuition is assessed the first of each month for the month and charged to the credit card on file approximately the 1st of each month the tuition is due. To avoid the credit card on file being charged, another form of payment must be received before the 1st of the month tuition is due (or by set due date for other fees and charges). The studio does NOT invoice. Accounts with open balances past the 10th of the month will be charged a $15 late fee per month until balance is paid in full. Payments on an account are applied first to the oldest open charges through current. If you choose to pay the tuition in full (all 10 monthly installments) a 10% discount will be applied. If paying for the year in full, a refund will not be issued under any circumstances if you choose to leave the studio. The 10% discount is only offered for year in full payments if registered before October 1st.
In cases of a declined or expired credit card, we will notify you via email or phone number provided on this form. However, it is your responsibility to contact our office with any credit card changes or your account will be subject to a $15 per month late fee until your balance is paid in full. If payment is made by check, it is to be made payable to ELITE PERFORMING ARTS CENTER. Checks returned to us by our bank for any reason will accrue a $35 service charge. No third-party checks (payable to a payee other than the studio) will be accepted. The Studio may refuse to accept checks from any individual who has caused his/her checks to be returned to the Studio account. In such case, only cash, certified checks, bank checks, or money orders will be accepted. If an account balance is carried over more than 30 days, the studio reserves the right to terminate the student's enrollment in any classes. If termination occurs, the studio will contact you via phone, email, or U.S. postal mail. If no response by you is made within 5 days to the studio regarding student's class termination, student will be prohibited from participating in any and all class(es) until payment is received. If this occurs, the student's spot in the class(es) may then be filled and the student will NOT be able to be reinstated if the class is full and no refunds for the 10th tuition installment, registration fee, or costume(s) will be given. In cases of termination, you will still be responsible for tuition installment charges for months when the student has danced at the studio, as well as the 10th monthly installment. If charges remain unpaid, account may be placed with a collection agency.
I understand and accept EPAC's tuition payment policy, and I permit EPAC to charge my credit card in accordance with the above stated policy. I certify that I have the authority to authorize these transactions with the credit card left on file.
COSTUME & RECITAL POLICY: If a student is not dancing in recital, it must be noted at the time of registration.
Recreational Costumes - Recreational costumes for the 2019-2020 dance season are $89 per costume, which includes 1 pair of tights per costume. For recreational classes, payments will be broken down into 2 installments. The first costume installment is due November 15, 2019, and the final installment is due December 15, 2019.
Company Costumes - Cost of company costumes will vary. A $75 deposit will be collected for each company costume. Remaining balances will be added on to your account based on the final cost of each costume. Company costume deposits will be broken down into 4 payment installments. The first costume installment is due on October 15, 2019, second installment on November 15, 2019, and the third installment is due on December 15, 2019. Once final costs are determined by Studio Directors, the remaining installment will be due by January 15, 2020. Each Performance Team & Stars Team dancer will receive 1 pair of tights. Elite Team dancers will receive 2 pair of tights, regardless of the amount of costumes.
Recital Fee - All families will be charged at $35 media fee for the recital. Each family will receive a digital copy of all recital shows. This fee will be split into two payments and charged at the same time of costumes. The first fee of $17.50 will be charged on November 15, 2019 and the second fee of $17.50 will be charged on December 15, 2019.
All costume installment payments are non-refundable. If full payment is not received by December 15, 2019 (Recreational) or January 15, 2020 (Company) we cannot guarantee a student a costume in time for pictures, recital and/or competitions. Late fees of $15 per costume will be incurred if paying after final installment due date. Additional expenses incurred by EPAC for costumes with payments received after final installment due date will be the responsibility of the parent/guardian of student. Students without a costume cannot participate in competitions, pictures or end of year recital. If you leave the studio, costumes with full payment must be picked up by June 20, 2020 or they will be donated to charity. Once costumes arrive to the studio, if a parent feels that the costume does not fit, you have 2 weeks to exchange the costume or a $35 exchange fee will be implemented. We cannot guarantee the new size will arrive to the studio on time. Costumes will only be distributed once a student's account balance is paid in full. If paying account balance by check, the check must clear before costumes can be distributed or another form of payment will be needed.
If you register after January 1, 2020, a costume must be paid in full at the time of registration for it to be ordered for recital. I understand that I am not guaranteed a costume will arrive in time for Picture Day or Recital. Costumes will not be ordered for any students registered after April 1, 2020.

AGREEMENT: I hereby enroll my child at Elite Performing Arts Center (EPAC) for the 2019 - 2020 Dance Season beginning on September 7, 2019 and ending on June 20, 2020. I have read and agreed to all policies, terms and conditions as stated above. I also acknowledge that excessive absenteeism may result in my child being ineligible to participate in EPAC recitals, performances and competitions. I hereby permit my child to participate in dance and/or dance tricks, jumps, leaps, tumbling, etc. and understand the risks inherent in such activities. I release EPAC, its owners, and its staff from any and all legal liability and medical costs arising due to any injury incurred during the course of instruction and/or participation in any EPAC related activity. I release rights to all photos and videos taken in relation to EPAC for exclusive use in promotion or advertising.
