The registration fee is $40.00 for the first student. Additional student(s), after the first, will incur a registration fee of $15.00 each.
Registration fees and first month's tuition are due at the time of registration. Students cannot be placed on class rolls without payment. These fees are NON-REFUNDABLE. If classes are stopped for any reasons, a new registration fee will be applied to your account. For example, if you attend classes for the fall/spring season, sit out for the summer season, and return for the following fall/spring season, a new registration fee will be charged.
All tuition is due the first week of each month of the dance year. Statements will be emailed for overdue accounts only. The first months' tuition is due at registration. After the 10th of the month, tuition payments are considered late and a $10.00 late fee will be added to your account. All late charges will be carried over each month. Due to the ability to make tuition payments online, studio closures do not extended payment deadlines and late fees will still be applied. All payments received are applied to outstanding charges in the order in which they were billed. No exceptions.
When an account becomes 30 days overdue, the person responsible for that account will be notified by email that the account is overdue and must be paid in full in the next 30 days. If an account becomes more than 60 days overdue the person responsible for that account will be notified by letter that the account has been deactivated and the student is restricted from participating until the account has been paid in full, or a payment plan has been agreed upon with Lakeview Creative Arts. Lakeview Creative Arts reserves the right to send any account which has become 60 or more days overdue to collections.
Any disputes regarding tuition or other charges must be brought to the attention of management in writing within 30 days of the charge in question. Accounts must have and maintain a $0 balance no later than May 30th to participate in the Recital. Dance tuition is based on a 10 month season and is not altered for months with holidays, etc. The extra lessons received during any five-week month account for the days the school is closed for holidays. No refunds or deductions are given if a student misses class for any reason, except in the case of extreme prolonged illness requiring hospitalization with appropriate documentation.
Any cancellations or changes in class schedule will be posted to the Lakeview Creative Arts social media pages first. When possible, we will also send notices via email (using the contact information you have provided to us). However, the primary form of notification will be through our social media accounts. You are responsible to take note of your class changes to mitigate any inconvenience.
Should Lakeview Creative Arts cancel a class for any reason other than weather (very rare) this will result in either a makeup session or class credit.
There will be a $30.00 charge for all checks returned from your bank and late fees will apply. In the event of a check being returned, we will no longer be able to accept personal checks for payment and will require payment to be made via certified funds. Payment methods include Visa, Mastercard, Discover, AMEX, bank draft, cash, and check. We do accept payments over the phone or through email with prior notification. All payments made via cash or check should be placed in an envelope with the student's name on it, and turned in to the tuition box on the wall in the waiting room. Payments should never be left on the desk. For your protection, cash payments will not be accepted without an envelope. When paying with cash always ask for a receipt.
There are no refunds whatsoever once payment has been received except in the case of extreme prolonged illness requiring hospitalization with appropriate documentation. Automatic payment plans are available and encouraged.
In consideration of my child's enrollment in any dance and/or karate instruction program, I understand and agree on behalf of myself and my child, to release, hold harmless, and discharge Lakeview Creative Arts from all claims, costs, liabilities, expenses or judgments, including attorneys' fees and court costs for any occurrences in connection with any dance and/or karate instruction. I assume all risks to my child in connection with any instruction and further release Lakeview Creative Arts and its owners and employees from liability for any injury sustained by my child while he or she is enrolled in any dance and/or karate instruction program, including all risks reasonably connected with such activity whether foreseen or unforeseen.
I understand that Lakeview Creative Arts is not responsible for my child or other children under my supervision who are left unsupervised in the common areas and areas surrounding the dance studio and that Lakeview Creative Arts will only be supervising my child when he or she is participating in scheduled dance and/or karate activities, programs or instruction.
I understand that Lakeview Creative Arts is not responsible for personal property that is lost, damaged or stolen while I or my child is at or on Lakeview Creative Arts property.
I acknowledge and agree that it is my responsibility to maintain my own accident and health insurance coverage that provides adequate coverage for myself and my child participating in Lakeview Creative Arts activities and that Lakeview Creative Arts does not provide accident or health insurance for those participating in its instruction, activities or programs.
My signature is proof of my intention to execute a complete and unconditional waiver and release of all liability pursuant to the terms herein, and agreement as to all terms and conditions contained above. I am of lawful age and competent to sign this affirmation.
I HAVE FULLY INFORMED MYSELF AS TO THE CONTENTS OF THIS RELEASE AND HAVE READ THE SAME PRIOR TO SIGNING.
In the event of adverse weather conditions, such as tropical storms, hurricanes, freezes, snow, etc.... Lakeview Creative Arts will follow the Orleans Parish school closures as a guide for closing and canceling classes. If evacuations are suggested/ordered and remain in effect for a prolonged period of time, or if people are unable to return home due to weather conditions, the studio will remain closed until it is deemed safe to return by local authorities. If we are unable to reopen due to damage to our building, and/or the landlord determines we are unable to reopen for any reason, no refunds will be given for tuition already paid to the studio. We will notify families by email and/or social media of any closures when we become aware of them.
We plan ahead for weather days when making our class schedule. Makeups for missed days may be offered if time allows; however, this is not a guarantee.
