Hold Harmless
I understand that dance is an activity that could cause injury. I hereby assume the risk for any injury that my child may sustain while participating in the activities offered by Studio Dance Centre. "Activities" include all in-house activities, as well as offsite activities, including, but not limited to, classes at Starwood Montessori School, classes at Starwood Academy of Frisco, community performances, rehearsals, recitals, etc. I agree to release and hold harmless and forever discharge Studio Dance Centre, its officials, faculty and staff, agents, other employees, or volunteers from any actions, suits, damages, claims, or judgments that may result from any personal injury that my child may sustain while on the premises of Studio Dance Centre and at any and all offsite activities offered to my child.
Calendar & Classes
I understand that SDC's 2020-2021 calendar year (including holidays) takes place from August 3, 2020 through July 31, 2021 and that weeks marked TENTATIVE are subject to change and dependent on approval from SDC's vendor partners.

Classes
I understand that classes are offered continuously based on the SDC annual calendar and that students may be advanced to a new level at any time recommended by his/her instructor in accordance with SDC's class placement standards. I understand that SDC reserves the right to reschedule/combine/cancel classes at any time due to low enrollment, and that low enrollment is deemed any class with less than six students enrolled. I understand that SDC reserves the right to provide service continuity for classes in-person or online, in accordance with federal, state, and/or local government recommendations.
Tuition & Membership
I understand that tuition rates are nonrefundable and based on the calendar year, not by the number of classes in the month, and that a full season of dance includes 40 weeks (42 weeks are offered, providing 2 complimentary weeks). I understand that tuition does not include non-dance weeks such as break weeks, Photo Week, and/or Performance Week.
I understand that tuition is paid monthly or annually and that monthly tuition must be paid by automatic bank transfer, debit, or credit card payment and will be auto-drafted on the 15th of each month. I understand that late payments may be subject to an accruing 5% monthly finance charge and services discontinued until paid and that NSF payments will incur a $35 fee.
I understand that I can freeze my account a maximum of one time per year. I understand that if my child is performing, my account cannot be frozen in September, March, or April.
I understand and agree that in addition to this agreement, my verbal and/or email consent is sufficient to approve any class adds/drops or other payments throughout the season. I understand that tuition, membership fees, and costume fees are nonrefundable, and that account credit will be issued if I initiate a schedule change that causes a credit. I understand that credits may be applied to any SDC tuition or fees within six months and that no refunds will be given if a similar online class is provided in place of an in-person class.
I understand that annual membership fees are nonrefundable and will be automatically renewed and auto-drafted to my account on May 1 unless a withdrawal form is completed prior to that date.
Attendance & Withdrawal
I understand that regular attendance is vital to student progress and group choreography. I understand that makeup classes are offered but dependent on class availability. I understand that all absences must be reported via SDC's absence form, as found on the Parent Resources website page: https://studiodancecentre.com/parent-resources/ I understand that a doctor's release note is required for my dancer to return to class after an extended injury or illness.
I understand that there is a one-month minimum for all lessons and that a one-month notice from the 15th is required to discontinue any payments. I understand I must complete and sign a withdrawal form and that auto-draft payments will stop after the one-month notice period.
Safer Studio Policy
I understand that SDC requires all staff and students to stay home when they are ill. I understand that SDC follows best practices and recommendations from the local health department and from our industry associations, More Than Just Great Dancing!® and Youth Protection Advocates in Dance. I understand that unlike older children and adults, young children cannot be expected to maintain safe distancing at all times, and therefore SDC focuses on a hierarchy of measures beginning with keeping ill people home, followed by frequent hand cleaning and good hygiene practices, amplified cleaning, and curricular and facility adaptations to minimize contact and mingling where possible. I understand that all studio classes are backed up on an online learning platform and that in the event a class is unable to be held at the studio, classes will convert to an online learning platform until in-studio classes can resume. I understand that SDC operates on a "traffic light" Safety Level system for class schedules, with green meaning "go as usual," yellow meaning "proceed with changes," and pink meaning "learn online with SDC At Home".
Performances
I understand that SDC has two tentative performance opportunities available, the Fall Showcase and the Spring Concert. I understand that eligibility for the Fall Showcase may be limited after September 1st and that eligibility for the Spring Concert may be limited after February 1st.
I understand that for the Spring Concert, there is a Photo Week and a Performance Week where there are no regular classes, and like the school breaks, those weeks are not included in tuition payments.
I understand that for the Spring Concert, costumes are made-to-order for each class, and that costume fees of $90 per costume, plus tax, will be auto-drafted from my account in two installments, on November 1st and December 1st, unless I have opted out by October 10th.
Spot TV Video Agreement
I understand that I have enrolled my child(ren) at Studio Dance Centre, located at 8811 Teel Pkwy, Ste 240, Frisco, TX 75036, also referred to herein as the "Children's Activity Center." I understand that the Children's Activity Center has implemented the use of an electronic program whereby cameras are in use and my children are under streaming video surveillance which is used for secure accessibility from the web, as well as training and management purposes, all in accordance with the terms and conditions associated with the Spot TV website (also referred to herein as "Spot TV"). All material in whatever format relating in any way to this program, including all photographs, web streaming and audio/video recordings, are copyrighted and owned solely by the Children's Activity Center. I understand that I do not have any rights to any of this material for any reason at any time. I also understand and agree that I will not screenshot, copy, reproduce, alter, modify, or create derivative works from classes, rehearsals and other content broadcast through this program. I understand that unauthorized recording, duplication, or distribution of this copyrighted work is illegal. I assume full liability for all the terms outlined in this release regarding any family member's account activity associated with my child or children. I understand that legal action can be taken against me by Spot TV and/or the Children's Activity Center for such copyright infringement. I understand that the term "photograph" as used herein encompasses still photographs, audio, video, and motion picture footage. I hereby consent to recordings and photographs of myself and/or my child(ren) related to this program. I acknowledge that I have been given the opportunity, before executing this release, to review the online security features, privacy policy, and terms of service made available on Spot TV's website. I represent that I have the full legal capacity to execute this release on behalf of all the parties mentioned above.
Additional Policies
Dress Code
I understand that proper attire and hair etiquette are required for all classes and that information may be accessed via email or on SDC's website. I understand that I can also access approved dress code items through http://www.curtaincallforclass.com/my-studio/17109 with the password StudioDance5678. I understand that if my child brings a cell phone to the studio, it must remain in silent mode, in his or her dance bag, during classes.
Photos & Video Marketing
I understand that photos and video clips are commonly taken at SDC and at SDC-related events for marketing purposes, which may include but are not limited to: social media posts, website updates, brochures, flyers, posters, and other marketing materials. Student names are not used on marketing materials. I understand that I must email studiodanceoffice@gmail.com in order to opt my child out of photo/video marketing.
Bad Weather Days
I understand that SDC will make every effort to help you schedule a make-up class within two weeks of a bad weather day. Although SDC will follow Frisco ISD for most bad weather closings, there may be days where the weather improves or gets worse and we will make a decision based on local forecasts and other after school closings. I understand that bad weather closings may be made via our website, voicemail, email, and/or text.
Code of Conduct
I understand that my child and my family are expected to adhere to courteous behavior at SDC and at any other studio-related classes, activities, or events online or offsite. I understand that there is a difference between student conflict and bullying, and that I will encourage my child to navigate conflict age-appropriately if it arises and notify SDC of the situation if I deem it necessary. I understand that bullying or harassment will not be tolerated and the authorities will be contacted if such a case is reported.
Social Media
I understand that the use of social media is commonplace and encouraged, and that this policy is intended to clarify use of social media as related to SDC or at online or studio-related events. I understand that SDC owns the copyright to all choreography and should be tagged in any video snippets taken on the premises, online, or at studio-related events. I understand that I should not post anything that compromises the privacy or integrity of SDC staff, parents, or students, and that SDC does not tolerate any bullying or harassment of any kind, social media included.
Communications
I understand that SDC communicates primarily via email, and I agree to stay informed on all studio news, reminders, dates, and events. I understand that SDC operates on a "traffic light" Safety Level system for class schedules, with green meaning "go as usual," yellow meaning "proceed with changes," and pink meaning "learn online with SDC At Home" and it is my responsibility to know what level SDC is on at any given time.
Agreement
I understand and agree to SDC's policies. I understand I can access SDC's policies through the Parent Portal at any time, or request a PDF summary at any time. I understand that SDC reserves the right to refuse service to anyone not abiding by the policies.
