I understand that tuition is due on the 1st of each month. I hereby authorize Aspire Dance Arts to charge my credit card or checking account for all services and products related to my family's enrollment in classes and activities at Aspire Dance Arts, including but not limited to tuition, registration fees, late fees, costume orders, and showcase fees. If payments have not been received by the 10th of the month or if my credit card on file is declined more than once, a late fee in the amount of $15.00 will automatically be charged to my account on the 11th for the unpaid tuition. I understand that tuition is due whether or not the studio is open for business during holiday months or inclement weather. Any student with an overdue balance of more than 15 days will not be allowed into class until payment in full is received or payment arrangements have been made.
**If you do not wish to have payments automatically drafted from your account, payments must be received by the office staff at Aspire Dance Arts before the first of the month in order to avoid autobilling. Regardless, we must always have a valid card on file.
Aspire Dance Arts gladly accepts the following methods of payment: Cash, Check, VISA, Mastercard, & Discover. Cash & check payments must be made prior to the tuition due date or our system will automatically charge your credit card on file on the date due if there is a balance. Throughout the season, I can update my card on file under the "My Account" tab in the Parent Portal to ensure the correct card is being charged each month. Payments can be made to Aspire Dance Arts online, through the office, or by mail. Aspire Dance Arts has a returned check fee of $25.
Aspire Dance Arts will not be sending you a statement with your monthly tuition bill from our office. If at any time you would like a printed statement of your account you may request one at the office or login to the portal to view & print.
I have read this entire agreement and understand that I will be held fully responsible for its terms and conditions of service. I agree to notify Aspire Dance Arts immediately of any change in the status of my charge account including but not limited to card expiration, name change, limitation of use, loss or theft or the card, etc. In the event that the amount charged is refused for whatever reason, I accept responsibility for full payment for the amount charged as well as a late fee of $15.00.
*Summer Camps/Intensives: Full payment for the camp/intensive will be charged at the time of registration to the card on file.
As the legal parent or guardian, I agree to assume all risks associated with my child's participation in dance & performing arts instruction, rehearsals, performances, and related activities. I release Aspire Dance Arts' staff, owners, and faculty from any and all liability, claims, demands, medical bills, and causes of action whatsoever occurring in or around the studio premises or at any functions held at other locations in connection with the dance & performing arts classes in which the student is enrolled as well as en route to or from any of said premises. This includes any loss of personal items, damage to property, or injury, including death, that may be sustained by the participant and/or the undersigned. I understand that appropriate physical contact is required during the instruction of dance & tumbling and I give permission for instructors to make appropriate physical contact with me or my child for such instruction.
