I understand that tuition is due on the 1st of each month. I hereby authorize Aspire Dance Arts to charge my credit card or checking account for all services and products related to my family's enrollment in classes and activities at Aspire Dance Arts, including but not limited to tuition, registration fees, late fees, costume orders, and showcase fees. If payments have not been received by the 10th of the month or if my credit card on file is declined more than once, a late fee in the amount of $15.00 will automatically be charged to my account on the 11th for the unpaid tuition. I understand that tuition is due whether or not the studio is open for business during holiday months or inclement weather. Any student with an overdue balance of more than 15 days will not be allowed into class until payment in full is received or payment arrangements have been made.
**If you do not wish to have payments automatically drafted from your account, payments must be received by the office staff at Aspire Dance Arts before the first of the month in order to avoid autobilling. Regardless, we must always have a valid card on file.
Aspire Dance Arts gladly accepts the following methods of payment: Cash, Check, VISA, Mastercard, & Discover. Cash & check payments must be made prior to the tuition due date or our system will automatically charge your credit card on file on the date due if there is a balance. Throughout the season, I can update my card on file under the "My Account" tab in the Parent Portal to ensure the correct card is being charged each month. Payments can be made to Aspire Dance Arts online, through the office, or by mail. Aspire Dance Arts has a returned check fee of $25.
Aspire Dance Arts will not be sending you a statement with your monthly tuition bill from our office. If at any time you would like a printed statement of your account you may request one at the office or login to the portal to view & print.
I have read this entire agreement and understand that I will be held fully responsible for its terms and conditions of service. I agree to notify Aspire Dance Arts immediately of any change in the status of my charge account including but not limited to card expiration, name change, limitation of use, loss or theft or the card, etc. In the event that the amount charged is refused for whatever reason, I accept responsibility for full payment for the amount charged as well as a late fee of $15.00. I understand that all fees including but not limited to Tuition Fees, Registration Fees, Costume Fees , Showcase Fees and Camp/Intensive Fees are nonrefundable.
*Summer Camps/Intensives: Full payment for the camp/intensive will be charged at the time of registration to the card on file.
Aspire Dance Arts' season begins Monday, August 2nd, 2020, and ends Saturday, May 21, 2021. Aspire Dance Arts follows the Cherokee County School Systems schedule with regards to week long closings. We DO NOT CLOSE for Labor Day, MLK Day, or other teacher workdays.
I understand that closings due to inclement weather and other acts of God will not require a make up class. If necessary, the staff at Aspire Dance Arts will schedule make up classes at their discretion. The following are the studio's scheduled closings:
Fall Break: September 21-27th
Thanksgiving Break: November 23rd-29th
Christmas Break, December 21st-January 3rd
Winter Break, February 15th-21st
Spring Break, April 5-11th
I understand that upon registering, my child will be enrolled in classes for the duration of the season (August 2020-May 2021). In order to withdraw from classes, Aspire Dance Arts requires a two week (10 business days) written notice by email or turned into the office prior to the next month's tuition due date. Otherwise, the following month's tuition will be charged in full.
As the legal parent or guardian, I agree to assume all risks associated with my child's participation in dance & performing arts instruction, rehearsals, performances, and related activities. I release Aspire Dance Arts' staff, owners, and faculty from any and all liability, claims, demands, medical bills, and causes of action whatsoever occurring in or around the studio premises or at any functions held at other locations in connection with the dance & performing arts classes in which the student is enrolled as well as en route to or from any of said premises. This includes any loss of personal items, damage to property, or injury, including death, that may be sustained by the participant and/or the undersigned. I understand that appropriate physical contact is required during the instruction of dance & tumbling and I give permission for instructors to make appropriate physical contact with me or my child for such instruction.
I understand that my child may not attend classes at Aspire Dance Arts if he/she has been diagnosed with COVID-19, has exhibited symptoms of COVID-19 or has had contact with a person who has or is suspected to have COVID-19 within the last fourteen days. Also, If your child is exhibiting any signs of illness (cough, cold, etc) please do not bring them to class.
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending classes or camps at Aspire Dance Arts and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed or infected by COVID-19 at Aspire Dance Arts may result from the actions, omissions, or negligence of myself and others, including but not limited to Aspire Dance Arts employees, volunteers, students, and their families.
