________ Dress Code: Please see the attire guidelines on our website www.ebonyarrington.com. Students not in compliance with these guidelines may not be allowed to participate. NO REFUNDS will be given for missed classes due to improper attire and/or foot wear.
________ Attendance & Lateness: The studio reserves the right to have students who come late to class, sit out of class. Repeated lateness may result in termination of lessons. A minimum attendance level will be required. If a student misses more than four classes without written notices, the studio reserves the right to terminate lessons. Students missing more than four classes after January will not be allowed to participate in the year-end dance recital.
________ Arrival & Departure: Any Students not driving themselves to class should be signed in and out at the check-in table in the lobby of the studio. Students should arrive no earlier than 10 minutes prior to their scheduled class time. Parents are expected to pick up their child promptly at the end of class. EADP is not responsible for students once their class has been dismissed. After 2 late pick-ups, parents may incur a late fee of up $5 per 15minutes after class dismissal.
________Scheduled / Missed Classes: If the teacher is absent or misses a class, a make-up class will be scheduled or there will be a credit to your account. This decision will be at the discretion of the administrator. If a class time needs to be moved or adjusted, the studio will provide at reasonable notice to each affected student/family.
________ Parental Observation: Parents are not permitted to observe weekly dance classes. Throughout the year specific days will be set aside for parents to observe the classes. Parents will receive advanced notice of these days. Parents are asked to please be respectful of this policy, other students in the class and the teacher. Parents should not enter the classroom during the class time without first notifying and being escorted by and EADP staff member. Please save all questions or comments for the instructor after the classes has ended.
________ Parent's Responsibility to be Aware of Dates & Events: It is the responsibility of the parent or adult student to be aware of all studio activities, such as observation days, closing dates, fundraisers, recital dates, etc. The studio will post all such in your school office and send out notices via e-mail. It is the parent's responsibility to regularly check the bulletin boards, check in the studio, and / or www.ebonyarrington.com to ensure they are informed. It is the responsibility of the parents or adult students to inform the studio of any address or telephone number change.
Ebony Arrington Dance and Performance is committed to conducting its programs and activities in the safest manner possible and holds the safety of participants in the highest possible regard. Parents registering their children in programs and activities must recognize, however, that there is a possibility of injury when choosing to participate.
Ebony Arrington Dance and Performance strives to reduce such risks and insists that all participants follow safety rules and instructions which have been designed to protect the participant's safety. The cost of any medical treatment as a result shall rest with the parent/guardian of participants. Before registering family member for this program parents should review their own health insurance policy for coverage. Your cooperation is greatly appreciated.
Release of Liability & Permission to Secure Treatment
I recognize and acknowledge that there are certain risks of physical injury to participants in the above program(s) and I agree to assume the full risk of any injuries, damages or loss regardless of severity which I or my minor child/ward may sustain as a result of participating in any and all activities connected with or associated with such program(s). I agree to waive and relinquish all claims I or my minor child/ward may have against the Ebony Arrington Dance and Performance and its officers, agents, volunteers and employees as a result of participation in the program.
I do hereby fully release and discharge Ebony Arrington Dance and Performance and its officers, agents, volunteers and employees from any and all claims from injury, damage or loss with the activities of the program(s). I further agree to indemnify and hold harmless and defend Ebony Arrington Dance and Performance and its officers, agents, volunteers, and employees from any and all claims resulting from injuries, damages, and losses sustained by me or my minor child arising out of, connected with, or in any way associated with the activities of the program(s).
In the event of any emergency, I authorize Ebony Arrington Dance and Performance to secure from any licensed hospital, physician and/or medical personnel any treatment deemed necessary for me or my minor child/ward's immediate care and agree that I will be responsible for payment of any and all medical services rendered.
Recital Fee Policies for students attending Roebuck or Dance Foundation Locations:
• The Recital Fee is $135 for one class, and $60 for each additional class (per family). This fee covers a student's recital costume and tickets to the May recital (4 tickets per student, 6 per family).
• The Recital Fee will be charged to your account along with your tuition in September. To ease in this financial obligation, this fee can be divided into equal monthly installments beginning in September with the balance due in November.
• Anyone registering during the month of November or after must pay Recital Fee in full within 30 days of registration.
• Recital Fee balances not paid in full by November 30th (or other specified due date) will be subject to a $10 late charge applied every 30 days until the balance is paid in full.
• Accounts must have a $0 balance before students are permitted to participate in the recital.
Recital Fee Policies for students attending The Learning Center at New Life
o All registered students will participate in the May recital. The Recital Fee is $135 for one class, and $60 for each additional class (per family).This fee covers a student's recital costume and tickets to the May recital (4 tickets per student, 6 per family).
o A 50% costume deposit is due by November 10th. (Full payment will be due in November for those who are not enrolled to be drafted). The balance is due by January 10th. In order to receive costumes in time for the spring show, your full recital fee must be submitted. Costume orders will not be placed for those accounts with outstanding tuition, recital, and/or insufficient check fees. A $5 convenience charge will be added if you choose to break the payment into two payments.
o Recital Fee balances not paid in full by January 10th will be subject to a $10 late charge applied every 15 days until the balance is paid in full.
o Costume companies make costumes for general sizes, thus not always fitting exactly as we would like our regular street clothes to fit. Many times minor adjustments are needed. I understand that all alterations on costumes are the sole responsibility of the Parent.
o Recital fees are non-refundable for any reason. Any student withdrawing after January 30th will be responsible for full costume payments.
o Students must have a $0 balance (including tuition and recital fees) in order to receive costumes and in order to participate in the Spring Recital.
o Any payments made onsite at the recital venue are subject to $25 convenience fee and must be paid by cash, money order, certified check or credit card.
