Release of Liability
In consideration of my child’s participation in the classes given by On The Move Dance, LLC I do herby for my child release and discharge Kari May and all personnel thereof for all claims or damages, demands, actions, injuries, or whatsoever in any manner arising or growing out of my child’s participation in any of the classes offered by OTM Dance. I attest and verify that I have full knowledge of the risks involved in exercise and my child is physically fit and able to participate without endangering my child’s health.
Image Release
Also, I hereby grant permission to On the Move Dance, LLC to use the image(s) of my child(ren). Such use includes the display, distribution, publication, transmission, or otherwise use of photographs, images, and/or video taken of my child for use in promotional, marketing or educational materials.
I agree that these images may be used by On the Move Dance, LLC without further notification. I do understand that my child(ren)’s last name will not be used in conjunction with video or digital images.
Payment Policies
Tuition is based on 9 months of classes and is the same whether there are 2, 3, 4, or 5 lessons in a month. OTM Dance does not prorate. You have the option to pay all 9 months up front, or pay it out monthly. You will receive a 5% discount if you pay by semester and a 10% discount if you pay for the whole year. Tuition is due the first class of each month. After the 10th, a $10.00 late fee will automatically be added to your account and you will receive a statement. Additional statements will only be sent out if payment is late. There will be a $30.00 fee for all returned checks and cash payment will be required for the rest of the year. There are no refunds on tuition.
Classroom Attire
Classroom Attire Proper dancewear and footwear is required for all classes. Dancers may wear any color of leotard and tights. Dance pants and fitted dance tops are also acceptable. No school clothes please!! The teachers must be able to see a dancer’s body alignment to make necessary corrections.
If you are enrolled in teen or advanced ballet, you must wear a leotard and tights. You may wear dance shorts or a ballet skirt over your leotard. Please have hair pulled back into a bun.
All pre-school through 6th grade non-company classes will wear pink ballet shoes and black tap shoes for combo classes. All 1st grade through 6th grade non-company classes will wear tan jazz shoes. Teen classes will wear tan jazz shoes for jazz, black tap shoes for tap, and pink ballet shoes for ballet. Lyrical shoes may be required for lyrical, but this will be up to the teacher. Company classes may have different shoe requirements.
For hip hop, cheer and musical theater classes, students may wear shorts and a t-shirt along with athletic shoes.
Attendance & Closings
Attendance is very important to ensure a quality dance education. Dance classes often build on skills that were learned the previous week. Please make every effort to be in class each week. If a student misses 3 weeks in a row without notifying the studio, they will automatically be dropped from the class and will be responsible for fees accrued during their absence. Please notify the studio if you wish to remove your child from classes to avoid accrued fees. If a student must miss class, they can do a makeup class if they want to in another class of their same age and level as long as it's not full. Makeup classes are not required.
OTM Dance will observe MISD holidays, breaks, and school closings for weather. The exceptions to this are teacher work days and various Monday holidays such as Columbus Day. We will still have classes on these days. Please check the website (otmdance.com), Facebook, or your email on bad weather days to see if classes have been cancelled. If MISD cancels after school activities because of weather conditions, we will not have class either.
