Getting Started
Welcome to the 2020-2021 School Year Session (August 3rd, 2020 - May 15th, 2021). Once you enroll, we will make a one (1) time charge to the credit card you provided for the Registration Fee of $50 and the First Month's Tuition. Each month after, your monthly tuition will be debited automatically using the credit card on file. If you have any questions about your balance, please contact the Hayiya (478.960.4206). Only enroll in the classes you wish to be charged for.
All participants should come dressed in the required clothing. Please see our studio dress code for details.

Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.
Hayiya Dance Theatre ("the Facility") has put in place preventative measures to reduce the spread of COVID-19; however, the Facility cannot guarantee that you or your guests will not become infected with COVID-19. Further, attending events at the Facility could increase your risk and your student's risk of contracting COVID-19. All students will be required to participate in additional preventive measures while on site to further reduce the spread of COVID-19.
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my student(s) and I may be exposed to or infected by COVID-19 by attending events at the Facility and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at the Facility may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Facility employees, volunteers, and event participants, vendors and their families.
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my students or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my guest may experience or incur in connection with my event at the Facility or participation in activities at the Facility. On my behalf, and on behalf of my student, I hereby release, covenant not to sue, discharge, and hold harmless the Facility, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Facility, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Facility event.
Release of Liability
DISCLAIMER: HAYIYA DANCE THEATRE IS NOT RESPONSIBLE FOR ANY INJURY(OR LOSS OF PROPERTY) TO ANY PERSON WHILE PRACTICING, TAKING CLASS, COMPETING, PARTICIPATING IN PARENT'S NIGHT OUT, OPEN STUDIO, BIRTHDAY PARTIES OR ANY OTHER WAY INVOLVED IN MOVEMENT OR DANCE AT HAYIYA DANCE THEATRE FOR ANY REASON WHATSOEVER INCLUDING ORDINARY NEGLIGENCE ON THE PART OF HAYIYA DANCE THEATRE, ITS OWNERS, OFFICERS, AGENTS OR EMPLOYEES.
In Consideration of participating at Hayiya Dance Theatre, I represent that I understand the nature of this Activity and that I am qualified, in good health and proper physical condition to participate in such Activity. I acknowledge that if I believe conditions are unsafe, I will immediately discontinue participation in the Activity. I fully understand that this Activity involves risks of serious bodily injury, including permanent disability, paralysis and death, which may be caused by my own actions, or inactions, those of others participating in the event, the conditions in which the event takes place, or the negligence of the "RELEASEES" named below; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume such risks and all responsibility for losses, cost, and damages I incur as a result of my participation in this Activity.
I hereby release, discharge, and covenant not to sue Hayiya Dance Theatre, it's respective administrators, directors, agents, officers, volunteers and employees, other participants, any sponsors, advertisers, and, if applicable, owners and lessors of premises on which the Activity takes place, ( each considered one of the "RELEASEES" herein) from all liability, claims, demands, losses, or damages, on my account caused or alleged to be caused in whole or in part by the negligence of the "RELEASEES" or otherwise, including negligent rescue operations and future agree that if despite this release, waiver of liability, and assumption of risk, or anyone on my behalf, makes a claim against any releases, I will indemnify, save, and hold harmless each of the releases from any loss, liability, damage, or cost, which any may incur as the result of such claim.
I have read the RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT, understand that I have given up substantial rights by signing it and have signed it freely and without any inducement or assurance of any nature and intend it to be a complete and unconditional release of liability to the greatest extent allowed by law and agree that if any portion of this agreement is held to be invalid the balance, notwithstanding, shall continue in full force and effect.
Parental Consent
AND I, the minor's parent and/or legal guardian, understand the nature of the above referenced activities and the minor's experience and capabilities and believe the minor to be qualified to participate in such activity. I hereby release, discharge, covenant not to sue and AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS each of the Releasees from all liability, claims, demands, losses or damages on the minor's account caused or alleged to have been caused in whole or in part by the negligence of the Releasees or otherwise, including negligent rescue operations, and further agree that if, despite this release, I, the minor, or anyone on the minor's behalf makes a claim against any of the above Releasees, I WILL NOT INDEMNIFY, SAVE AND HOLD HARMLESS each of the Releasees from any litigation expenses, attorney fees, loss liability, damage, or cost any Releasee may incur as the result of any such claim.

Medical Emergency
The undersigned gives permission to Hayiya Dance Theatre, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian.
Risk Management
I/We, the above participant(s), and parent(s), guardian(s), do hereby consent to participation in the above program including all activities to the program. I/We assume all responsibilities for, and risks and hazards of, participation in the named programs, even though all will be done to keep all children safe, risk or injury may occur. I/We understand that NO REFUNDS will be issued. I have been informed and understand that this program is not a licensed childcare facility. I acknowledge and understand that this program is not required to be licensed by the Georgia Department of Early Care and Learning. I acknowledge and understand that this program is exempt from state license requirements.

Make Up Policy
I understand that the make up policy allows for (1) makeup per month during the School Year. Please call 478.960.4206 to schedule your make up class or if your child(ren) have to miss a class. Make ups will need to be made within two weeks of the missed class. There are no makeups allowed for a missed make up class.
We are closed for the week of Thanksgiving, two weeks at Christmas/New Year and for Spring Break, makeups are not available for these weeks. We are open during fall (October) and winter (February) breaks and on the Monday holidays (except MLK day).
Photo Release
Occasionally pictures are taken of the students for the Hayiya Dance Theatre (HDT) website, wall displays and other potential media. I hereby grant HDT permission to use my daughter's/son's photograph(s)/video(s) in any and all of its publications, such as news releases, newsletters, brochures, Facebook and the HDT web site.
If you do not agree to this, please do not check the box and call 478.960.4206 so that we can place this in your file.
Spectator and Lobby Guidelines
Absolutely no spectators allowed in the studio. Spectators will be invited periodically to view at the end of a class at time designated by the instructor. Parents are welcome to schedule a time to sit in on class for short period to monitor their student's progress. Please note that any visitors in our 3-5 yrs classes create a substantial distraction for the little ones. Please limited those requests in order to maximize your child's and the classmate's instruction time. Spectators must refrain from communicating, disrupting or interfering with dancers and instructors. Please do not interact with your students during view or otherwise distract them (i.e. with photos, videos, "Smile, Sweat Pea", etc. ).
Please monitor your students in the lobby. No children should be left unattended. Dance lessons and rehearsals should be done in the dance classrooms under the supervision of HDT instructors, not in the lobby or green / brown rooms. The green and brown rooms are NOT an extension of the lobby.
Please refrain from sitting behind the front desk in the lobby. This area is designated for HDT staff and volunteers only.
If you need assistance getting your child into their class if you've arrived late, or need to pick-up early, please notify your student's instructor, they will help. HDT is not responsible for fire, theft damage to, or loss of vehicle or any articles left therein while parked on HDT property. Please be considerate of others while at HDT. Management reserves the right to ask those not respecting these guidelines to leave.
Withdrawal Notice
If you wish to withdraw your child(ren), we require you to sign a 30 day withdrawal notice at the front desk. You have to sign the withdrawal form in order to stop the fees from being applied monthly.
Payment Information
SCHOOL YEAR SESSION - Registration will require a $50 registration fee and the first month's tuition. We will run the card on file for this amount at the time of enrollment. Your next payment will be due on September 5, 2020. Payments are due on the 5th of each month. All payments will be automatically debited using the card on file on the 5th of each month. Late fees in the amount of $10 will be applied on the 6th to all unsuccessful monthly debits. Please notify Hayiya before the 5th in the event that you have lost, changed or need to updated your card information to avoid a late fee. Students will be dropped if account balance goes over 30 days.
