Assumption of Risk - Participation
In consideration of participating in the gymnastics and/or tumbling program at Champions Gymnastics Center, Inc. I represent that I understand the nature of this Activity and that my minor child is qualified, in good health, and in proper physical condition to participate in such Activity. I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue my minor child's participation in the activity. I fully understand that this Activity involves risks of serious bodily injury, including permanent disability, paralysis, and death, which may be caused by my and/or my minor child's actions or inactions, those of others participating in the event, the conditions in which the event takes place, or the negligence of the "releasees" named below in the Release of Liability; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, cost, and damages I incur as a result of my minor child's participation in the Activity.
Release of Liability - Participation
I hereby release, discharge, and covenant not to sue Champions Gymnastics Center, Inc., its respective administrators, directors, agents, officers, volunteers, and employees, other participants, any sponsors, advertisers, and, if applicable, owners and lessors of premises on which the Activity takes place, (each considered one of the "RELEASEES" herein) from all liability, claims, demands, losses, or damages, on my account caused or alleged to be caused in whole or in part by the negligence of the "releasees" or otherwise, including negligent rescue operations and future agree that if, despite this release, waiver of liability, and assumption of risk I, or anyone on my behalf, makes a claim against any of the Releasees, I will indemnify, save, and hold harmless each of the Releasees from any loss, liability, damage, or cost, which any may incur as the result of such claim.
Assumption of Risk - COVID-19
The novel coronavirus, COVID-19, has been declared a viral pandemic by the World Health Organization. COVID-19 is highly contagious and is believed to be spread mainly from person to person contact.
Champions Gymnastics Center, Inc. ("CGC") has put in place preventative measures to reduce the spread of COVID-19; however, CGC cannot guarantee that your or your child(ren) will not become infected with COVID-19. Furthermore attending class at CGC could increase your risk and your child(ren)'s risk of contracting COVID-19.
I, the parent/guardian, acknowledge the contagious nature of COVID-10 and voluntarily assume the risk that I and my child(ren) may be exposed to COVID-19 by attending class at CGC and that such exposure may result in personal injury, illness (infection), permanent disability, and/or death. I understand that the risk of becoming exposed to COVID-19 at CGC may result from the actions, omissions, or negligence of myself and/or others including, but not limited to, the "releasees" named below, volunteers, program participants and their families.
I agree to assume all the aforementioned risks and accept sole responsibility for any illness or injury (including but not limited to personal injury, disability, and/or death), damage, loss, claim, liability, or expense of any kind that I or my child(ren) may experience or incur in connection with my child(ren's) attendance at CGC or participation in CGC programs.
Release of Liability - COVID-19
I hereby release, discharge, and covenant not to sue Champions Gymnastics Center, Inc., its respective administrators, directors, agents, officers, volunteers, and employees, other participants and their families, any sponsors, advertisers, and, if applicable, owners and lessors of premises on which the Activity takes place, (each considered one of the "RELEASEES" herein) from all liability, claims, demands, losses, damages, costs, or expenses of any kind on my account caused or alleged to be caused in whole or in part by the negligence, omissions, or actions of the "releasees" or otherwise whether a COVID-19 infection occurs before, during, or after participation in any CGC program, and future agree that if, despite this release, waiver of liability, and assumption of risk, I, or anyone on my behalf, makes a claim against any of the Releasees, I will indemnify, save, and hold harmless each of the Releasees from any loss, liability, damage, or cost, which any may incur as the result of such claim.
Payment Policies
All tuitions are due the first (1st) of each month.
Any account that is unpaid at the end of the business day on the 17th of the month will incur a $15 late fee.
Any account that is unpaid at the end of the month will result in the student being suspended from class attendance until the account balance is paid in full.
Automatic bank draft is the preferred method of payment. However, if you choose to pay in house instead we accept cash, check, debit card, and credit card for tuition payment. We also process debit card or credit card forms of payment via phone or you may use the ePay service provided via your Parent Portal.
Medical Emergencies
In the event of a medical emergency the PARENT/GUARDIAN listed as CONTACT #1 will be the first person we attempt to contact. Should that person be unavailable we will attempt to contact the PARENT/GUARDIAN listed as CONTACT #2. Should neither of those persons be available we will attempt to contact the person listed as the emergency contact.
