I am the legal guardian and hereby consent to Participant's participation in all programs offered by or done in coordination with Gateway Elite Gymnastics LLC, its owners, officers, employees, coaches, agents, teachers, and volunteers ("Gateway Elite"), whether at facilities owned or operated by Gateway Elite's or elsewhere ("Programs"). I recognize that participation in any or all of the Programs will include a variety of activities including without limitation dance, gymnastics, tumbling, trampoline activities, fitness, and/or martial arts, and other activities that such inherently have significant risks of injury as a result of many factors including but not limited to use of equipment, exposure to heights, lights, loud music, fast motions, being inverted, or coming into contact with hard or stationary structures.
Assumption of Risk
I understand that there are certain risks of injury inherent in the Programs, including permanent disability or death, and severe social and economic losses, which might result not only from the Participant's own actions, inactions or negligence, but action, inaction or negligence of others, the rules of the facility, or the condition of the premises or of any equipment used and further, that there may be other unknown risks not reasonably foreseeable at this time, as well as in traveling and other related activities incidental to Participant's participation.
I further acknowledge, understand, appreciate and agree that participating may result in possible exposure to illnesses from infectious diseases, including, but not limited to MRSA, Influenza and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist. I knowingly and freely assume all such risks, both known and unknown, even if arising from negligence of the releases or others and assume full responsibility in participation and exposure.
With knowledge of the aforementioned risks, on behalf of Participant and myself, and all other persons with the ability to make a claim through or on behalf of the Participant, I hereby assume the risks of Participant's participation in the Programs, and hereby release Gateway Elite from all claims and/or liability for damages and/or injuries of any kind or nature suffered by Participant arising from or related to Participant's participation in the Programs. I understand that Gateway Elite is relying on the representations made herein, as well as this assumption of risk and release of liability, in agreeing to allow Participant to participate in the Programs. Accordingly, I further agree on behalf of myself, Participant and anyone claiming by or through me or Participant, that I shall hold harmless and fully indemnify the parties hereby released, including but not limited to Gateway Elite, from any and all claims, damages, costs, including attorney fees and causes of action which may arise from any cause made by me, or by, through, or on behalf of my child, even if the damages, injuries or death are caused in whole or by part by any of the persons or entities I hereby release.
I agree to advise the Participant of the risks, and to encourage the Participant to follow any safety rules and the coaches' instructions. I represent and warrant that there is now in place, and there will continue to be during all times that Participant is participating in the Programs, proper hospitalization, health, and accident insurance coverage, which I consider adequate for Participant considering their participation in the Programs.
This Parental Waiver and Consent Form shall be construed under and in accordance with the laws of the State of Illinois, without regard to its conflicts of law principles. Any claim or cause of action relating to this Agreement shall be brought in the Circuit Court of St. Clair County, Illinois or, if appropriate, in the Federal District Court for the Southern District of Illinois; and the parties hereby agree to submit to such jurisdiction.
I agree that Gateway Elite Gymnastics, LLC ("Gateway Elite") may photograph and record my child/dependent's likeness and activities (Images) during Gateway Elite-related activities. I grant the following rights to Gateway Elite: permission to use and re-use, publish and re-publish, and modify or alter the Image(s) taken during Gateway Elite-related activities. Use of the Images for editorial, commercial, trade, advertising, and any other purpose may be done in any medium now existing or subsequently developed, on Gateway Elite's website and on the Internet, and worldwide in perpetuity for the purposes stated above.
I waive my right to inspect or approve any editorial text or copy that is used in connection with the Images and release and discharge Gateway Elite from any and all claims arising out of use of the Images for the purposes described above, including any claims for libel, invasion of privacy, or other tortuous act.
I have read the foregoing. I fully understand its contents, understand that this agreement does not expire, and confirm my agreement by signing below. I am over the age of 21 and have legal capacity to sign the release.
Gateway Elite Gymnastics, LLC Policy and Procedures
Annual Registration Fee
An annual registration is charged per student. The Preschool, Recreational and Tumbling program registration fee will be $40 for the 1st child. The 2nd sibling will be charged $30. Each additional sibling after that will be charged $25 each. The registration allows students to take classes.
Registration fee for Team Xcel is $75 and Junior Olympic programs are $150 per student.
Annual Registration fee is non-refundable and non-transferable.
Class Session Payment
Payments are due in full upon registration. Class spots are secured only when registration and session fee has been paid. Payments can be made on the Parent Portal.
Team Monthly Tuition Payment
Fee's will be determined by level within the team programs and level is determined by coach's discretion.
The competitive gymnastics team is a year-round program and commitment. Our competitive season is from December through May. By paying monthly tuition, you receive 4 weeks of practices per month, regardless of the number of days in that month. Since there are 12 months in a year, parents pay for a total of 48 weeks of gymnastics instruction per year. This allows for 4 weeks of unpaid vacation days, sick days, or cancelled workouts. It is for this reason that there will be no pro-rating for holidays, vacations, illness or injury.
Should a team/development gymnast decide she will no longer be participating at Gateway Elite Gymnastics, we must receive written notification of the departure 30 days in advance. You will be financially responsible for paying all tuition up until the departure date, as well as any outstanding balances on your account.
Monthly tuition fees will be due on the 1st of each month. Payments can be made on the Parent Portal.
A late fee of $25 will be assessed for monthly tuition payments not received by the 7th of the month. If payment is not received by the 8th of the month, participant may be dropped from team for non-payment.
Team Assessment Fee's
Team assessment fees are determined in July of each year. Assessments are determined based on a tentative meet/camp schedule, estimated number of team athletes, coach's travel and per diem expenses, coach session compensation, individual/team entry fees and administrative support and coordination of all meet/camp activities. Assessment fees are NON-REFUNDABLE & non-transferable.
Class Schedule Changes
Gateway Elite Gymnastics reserves the right to cancel or change class schedules at any time prior to session start date. Participants who are impacted by schedule changes and cannot be reassigned to other classes will be refunded the class fee.
If your child is registered for a class and he or she cannot attend due to a conflict or illness, we will be happy to give you a maximum of 1 make up class per child per 9-week session. PLEASE NOTE: You must notify the office of the absence PRIOR TO the absence to receive a make-up. Should there not be spots available in your child's type of class, you will be offered a make up in a similar class. Make up classes are not guaranteed. There will be no credits applied for missed classes.
Parents may watch class from the viewing areas only. For the safety of all, NO ONE excepts students, coaches and staff are allowed in the work out areas.
If your child is late for class, please go to the front desk to have the office personnel escort them to class. This is for the safety of your child and others. Parents are not allowed in the gym work out areas.
Children in Lobbies
All children in the lobbies must be supervised by his/her parent or someone the parent has deemed responsible. We have provided tables and chairs, in the lobby. There should be no horseplay, running, rolling or throwing of toys or doing gymnastics in the halls or lobby.

Students should wear appropriate attire for the activity they are performing in co-ed environment as follows:
- Female Gymnasts - Leotard. Spandex shorts may be worn over the leotard, but not required. No socks. Hair of shoulder length or longer must be pulled up with a hair tie. No jewelry, gum or candy.
- Tumble, Trampoline, Open Gym - Shorts and a T-shirt with a "sports bra" under the T-shirt. Sport bras may not be worn alone. A shirt must be worn over the sport bra and covering the torso. Students may wear cheer shoes, but they MUST BE CLEAN. Hair of shoulder length or longer must be pulled up with a hair tie. No jewelry, gum or candy.
To provide for the safety and protection of all participants hands-on spotting will be utilized by coaching staff.
