Assumption of Risk
In consideration of participation in gymnastics and tumbling, I recognize that potentially severe injuries, including permanent paralysis or death can occur in sports or activities involving height or motion, including but not limited to gymnastics, tumbling, clinics, camps, private lessons, birthday parties, birthday party guests, open gym, playtime, fundraising events, drop-ins, and overnight events, which may be caused by my own actions, or inactions, those of others participating in the event, the conditions in which the event takes place, or the negligence of the "releases" named. I acknowledge that if I believe conditions are unsafe, I will immediately intervene to discontinue my child's participation in the activity. I understand that it is my responsibility to warn my child about the risks and dangers of gymnastics and injury.
Release of Liability/Medical Emergencies
I fully accept and assume all such risks and all responsibility for losses, costs, and damages I incur as a result of my child's participation in the activity. I hereby release, discharge, and covenant not to sue Calumet Gymnastics Club, LLC, its respective administrators, directors, coaches, agents, officers, volunteers, employees, other participants, sponsors, advertisers and owners and lessors of premises on which the activity takes place (each considered one of the RELEASES herein), from all liability, claims, demands, losses, or damages on my account caused in whole or in part by the negligence of the "releases" or otherwise, including negligent rescue operations and future agree that if, despite this release, waiver of liability, and assumption of risk I, or anyone on my behalf, makes a claim against any of the Releases, I will indemnify, save, and hold harmless each of the Releases from any loss, liability, damage, or cost which may incur as the result of such claim. I will hold Calumet Gymnastics Club harmless of any injuries incurred in and outside of gym areas. I, HOLD HARMLESS each of the Releases from any litigation expenses, attorney fees, loss liability, damage, or cost any Release may incur as the result of such claim.
I have read the Release and Waiver of Liability, Assumption of Risk, and Indemnity Agreement, understand that I have given up substantial rights by signing it and have signed it freely and without any inducement or assurance of any nature and intend it to be a complete and unconditional release of all liability to the greatest extent allowed by law and agree that if any portion of this agreement is held to be invalid the balance, notwithstanding, shall continue in full force and effect. I release Calumet Gymnastics Club to render temporary first aid to my child in the event of an injury or illness and if deemed necessary by Calumet Gymnastics Club staff to seek medical attention including releasing my child to the care of emergency medical services dispatched. Additionally, I agree to assume the responsibility of all medical expenses incurred once the request for emergency medical services has been initiated.
Covid-19
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing. Calumet Gymnastics Club, LLC, (CGC) has put in place preventative measures to reduce the spread of COVID-19; however, we cannot guarantee that you or your child(ren) will not become infected with COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending CGC and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at CGC may result from the actions, omissions, or negligence of myself and others, including, but not limited to, employees, volunteers, and program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)'s attendance at CGC or participation in CGC programs. ("Claims"). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless CGC, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of CGC, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any CGC program. Furthermore, I agree not to bring my child into the facility if he/she is running a fever of 100.4 or above, and will await until my child has been fever free for at least 24 hours unassisted with medication. If my child is positive for Covid-19, I agree not to bring her into the facility unless a documented release from a physician is provided.
Social Media and Photo releasee
I grant consent for my child's picture to be taken or to be filmed while participating in activities at Calumet Gymnastics Club. I authorize Calumet Gymnastics Club to take pictures or record film footage of my child in all forms of media
and in all manner for publication including, but not limited to, advertising and marketing campaigns, press
releases, periodicals, and website use. I hereby waive any right I may have to review, inspect, edit or approve
such publication and I release Calumet Gymnastics Club from any claims I may have against it for use of such
images, photographs, pictures, portraits, and audio, video and/or film footage of me.
Makeup Class policy -
Makeups are allowed for Recreational classes and must be made up within 30 days of the missed class, unless otherwise indicated due to extraordinary circumstances. If a class is missed whether it be an absence or holiday, the student has 30 days to make up that class. If that class is unable to be made up, then a coupon will be issued for one free hour of open gym. Tuition is non-refundable. If circumstances occur deeming the ability to make up a class within 30 days unattainable, then please contact Angie to explore additional options. If you choose to drop your child from the program, any classes you may have left to be made up will be forfeited.
Payment Policies
A $50, non-refundable, annual registration fee is deducted automatically from the card on file on your child's anniversary date. ($85 if more than one child) The account holder must keep the card on file updated and active through the parent portal. Access to the parent portal is granted via email address provided at the time of registration. Tuition is deducted from the card on file on the 25th of each month, the month prior to classes. ( example: June 25th, for July classes) A $10 late fee will be assessed for any payments received after the 1st of each month, and/or for any payments declined by the card on file not rectified within 48 business hours from the declination. Cash is accepted, however, it must be paid before the 25th of each month for the following months tuition. An active, updated card must still be kept updated in the parent portal. We are a year-round program with monthly tuition based on an average of four weeks per month. (If there are five weeks in a month, tuition is not increased. If there are three weeks in a month, credit is not given.) We grant a 10% discount for siblings enrolled in our program. We also grant a 10% discount to children or siblings of active military. As of February 10, 2020, all transactions for open gym, clinics, camps, etc...made by non-members with a credit/debit card, a $5 fee will be assessed per transaction.
Dress Code
Dress Code: A gymnastics leotard is preferred for girls, but footless leggings, with a snug top are acceptable. For boys, shorts, jogging/sweat pants, and a snug shirt are acceptable. Tops should not fall below the chest when upside down. Socks are permitted while warming up, but for the safety of the athlete, they will need to be removed during equipment rotation. With the exception of earring/nose/eyebrow studs, jewelry is not permitted. This is for the safety of the athlete and for the protection of equipment. Clothing with zippers, buckles, buttons, chains, etc...are not permitted. Please ensure that pockets are empty. Shoes are not permitted in the gym area. Hair lengths below the chin will need to be tied back and secured. Eyeglasses are to be attached to a strap, we are not responsible for broken or misplaced eyeglasses. Shoes, coats, etc....are to be stored in the lockers/cubbies provided.
General Safety Rules
General Safety Rules:
1. Use equipment ONLY in the manner you've been instructed. Inappropriate use of the equipment may lead to serious injury.
2. Only one person allowed at a time on the trampolines.
3. Only children enrolled in the class may use the equipment. Siblings and friends must wait with a guardian in the lobby.
4. All participants must have a signed liability waiver before entering the gymnastics area.
5. Injuries/Illness during class should be reported to the instructor immediately.
6. Food and drinks (except water bottles) are not to be brought into the gym area.
7. Children are not to use the warped wall unless approved by the instructor.
8. Children are not to intentionally enter the foam pit head first or in a diving position.
9. Children are to ensure that the area of the pit that they intend to land in is clear of other participants.
10. Children age 3 and under are not to enter the foam pit without adult supervision.
Athlete Behavior
Athlete Behavior: Athletes are expected to exhibit good sportsmanship towards each other at CGC and the other athletes at meets. Any sort of harassment, whether it be sexual harassment or bullying, will not be tolerated. Hitting, pushing, shoving, or any other sort of un-welcomed physical contact will not be tolerated. Bullying, sexual harassment, and/or un-welcomed physical contact may result in the suspension or expulsion of your child without a refund, and if warranted, notification of local authorities.
Parents Role
Parent's role: Unless INVITED by the staff, parents are to remain in the lobby throughout the duration of the class, with the exception of the parent n' tots class. Walking into the gym and interrupting a class will not be tolerated as it is disruptive to the class. This sort of behavior may result in the suspension or expulsion of your child without a refund. Parents are to provide their child with encouragement and support before and after class, they are not to coach their child from the lobby, hence interrupting the class and possibly contradicting the message intended by the coach. CGC holds our coaches to a high standard and we expect the same level of standard from the parents of the participants. If there are any concerns, please address them privately at the end of class or you can email Angie.
Minimum Enrollment Policy/Gym Closeures
Minimum enrollment policy: Each class must maintain a minimum enrollment of at least three (3) students to continue. If the class drops below that minimum, then your child will need to be reassigned to a similar class.
Gym Closures: Any closure due to inclement weather or other reasons beyond our control, notification will be provided via email and Facebook. Please keep your email address updated through the parent-portal, and make sure they are going straight into your inbox, not your spam folder. Cancellation Policy
Cancellation Policy: Intent to cancel is to be provided in writing (via email is acceptable) at least two weeks prior to the billing cycle (25th of each month). Refunds will not be issued if you cancel within two weeks prior to the billing cycle or after. In order for your membership to remain active for the rest of the year of enrollment, written intent to cancel must be provided, or you risk forfeiting the membership for the rest of the year. When you forfeit, you lose the discounted open gym rate and you must pay the $50 enrollment fee if you wish to re-enroll your child. Failure to show up for class is NOT considered a cancellation and your card will be charged the full amount of tuition until the two-week written notice is received.
Drop off/Pick up
Drop-off/Pick-up: Whether you choose to drop your child off at the front door or walk him/her into the lobby, please do not drop your child off any earlier than 10 minutes before their scheduled class time. It is important to us that your child safely connects with an adult upon the completion of class. Upon pick up, please have a designated guardian enter the lobby to pick up the child, or otherwise be visible to staff. Staff are not expected to walk children outside of the lobby. Please notify staff as soon as possible if there are any circumstances causing a delay in the scheduled pick up time, otherwise it is expected that you will be prepared to collect your child upon the scheduled completion of their class. If you arrive later than 5 minutes after their scheduled class time has ended, we reserve the right to assess a babysitting fee of $1 per minute. Children are not permitted to leave the premises without a designated adult.
Class Placement
Class Placement: Safety is our first priority, so coaches will assess the best class level to match your child's ability and age. The recommendation will provide a safe learning environment as well as an appropriate amount of challenge for your gymnast. Each class level has a set list of skills to be mastered before a coach recommends him/her to advance to the next level. If your child is especially eager to advance, you have the option to enroll your child in more than one class per week (with the exception of parent n' tots, and lil' bits). Also, coaches offer private lessons to help gymnasts strengthen a specific skill that might be preventing placement in a more advanced class. If your child has achieved the skills to advance and the coach recommends advancing your child, please make arrangements to have your child transferred into the recommended class within 30 days.
Open Gym
Open Gym: Open gym is provided for participants to freely use the gymnastics facilities for the time allotted. Parents/Guardians are required to remain in the facility if the child is age 4 or under. General safety instructions and dress code apply. Fighting and bullying will not be tolerated. Equipment settings are not to be adjusted unless approved and adjusted by the staff. During this time, staff will not be available to coach or spot participants as their priority is providing supervision for all the participants. It is encouraged for cell phones and other devices to be locked away or left at home. It is strongly encouraged that participants do not enter the foam pit with any items, including cell phones and eyeglasses. CGC is NOT responsible for any lost, broken, or stolen items.
