Release of Liability
I, the undersigned , recognize and understand the risks of physical injury inherent to dance and dance training and I fully assume those risks. I hereby release North Reading School of Ballet, LLC, its employees and dance teachers from all liability for injuries sustained or illnesses contracted while attending or participating in any dance classes, rehearsals, workshops, or performances. I agree to indemnify, defend, and hold harmless North Reading School of Ballet, LLC, event sponsors, employees and dance teachers for liabilities, costs and judgments arising from acts of omissions committed by me or my child which result in injury or damage to any person or property.
Medical Emergencies
In case of physical injury or medical emergency, I hereby authorize North Reading School of Ballet, LLC to make necessary arrangements to transport myself or my child to a medical treatment facility as necessary. All such transportation and medical treatment will be at my sole cost and expense. In extreme emergency, or if my child is under 18 years of age, I understand that North Reading School of Ballet, LLC will attempt to notify the person(s) I have named as my emergency contact(s) of my condition and how to reach me.
Photo Release/Social Media Policy
North Reading School of Ballet, LLC reserves the right to use photographs and videos taken during classes, workshops, performances, or other affiliated events for the purposes of instruction, advertising and promoting North Reading School of Ballet, LLC and its programs. Students, or parents of students who are minors, who do not wish to comply with this policy must notify North Reading School of Ballet, LLC prior to participation in class.
Student and Parent Use of Social Media: This policy is intended to provide North Reading School of Ballet students and parents with guidelines to eliminate any confusion concerning the use of social media (Facebook, Instagram, etc). Please respect the law, including those laws governing defamation, discrimination, harassment, and copyright and fair use. Do not post video clips of classes or recitals. North Reading School of Ballet, LLC owns the copyright to all choreography taught at the school.
Acknowledment of Waiver
I acknowledge and represent that I have fully informed myself of the content of the waiver and hold harmless agreement by reading it before I sign it, and I understand that I sign this document as my own free act and deed; no oral representations, statements, or inducements, apart from the written statement, have been made. I further state that I am at least eighteen (18) years of age and am fully competent to sign this agreement; and that I execute this release for full, adequate, and complete consideration fully intending to be bound by the same. I further state that there are no health-related reasons or problems which preclude or restrict my or my child's participation in this activity, and that I will pay any medical costs that may be attendant as a result of injury to me or my child.
COVID-19 Waiver
I, the undersigned, knowingly and willingly consent to have my child participate in programs with North Reading School of Ballet during the global COVID-19 pandemic. I understand the COVID-19 virus has a long incubation period during which carriers of the virus may not show signs and symptoms and still be highly contagious. It is impossible to determine who has it and who does not with the current limits in virus testing.
By sending my child to dance, I confirm that my child and members of my household have not in the past 14 days had any of the following symptoms of COVID-19 listed below:
• Fever greater than 100.4 degrees Fahrenheit
• Cough
• Shortness of Breath
• Flu like symptoms including GI upset, fatigue, body aches, or muscle pain
• Chills or repeated shaking with chills
• Sore Throat
• Headache
• Sudden loss of taste or smell
By sending my child to dance I further confirm that he/she has not been exposed to a person under investigation for COVID-19, or a person diagnosed with
COVID-19, in 14 days prior to any dance class attended.
I understand that certain travel may increases risk of contracting and transmitting the COVID-19 virus. In addition, the CDC recommends quarantine of 14 days.T herefore, I verify that my child, nor anyone in my household, have not traveled outside the United States in the past 14-days to countries that have been affected by COVID-19. I will hold harmless and indemnify, North Reading School of Ballet, teachers, associates, and employees against any claims, and actions, in exchange for programs with North Reading School of Ballet during this Covid-19 pandemic.
Please be advised that there may be risks with being in the proximity of other people. We are taking precautions to limit the spread of the disease, and following Massachusetts Reopening: Mandatory Safety Standards for Workplaces, yet there is still possibility for transmission. I make this decision for my child of my own free will relying upon my knowledge and judgement of any injury they may have sustained or possible illness, including the transmission of COVID-19, during participation in programs, and my decision to release has not been affected by any false statements or representations pertaining to those injuries or illnesses. I understand that this action is my decision.
PLEASE do not send your child to the studio if they, or any member of your household, are experiencing any of the signs and symptoms of COVID-19, as outlined above. This COVID-19 screening and consent to participate will be used each day programs are held. This written consent will be in effect for future classes and programs, and by sending your child to the studio you are consenting to continued negative responses to COVID-19 signs and symptoms. If your child or someone in the home has any of the symptoms listed above, they may not attend class at the studio.
By sending your child to the studio, you are consenting to this form, and stating your child and any and all members of the home are not exhibiting any of the signs and symptoms of COVID-19, as outlined above, and any and all members of the home have not been exposed to a person under investigation for COVID-19, or a person diagnosed with COVID-19 in the lst 14 days.
Payment Policy
Payment of Fees
Registration Fee: $35 per student per season (September-June)
Full Season Costume Fees: A costume fee of $70.00 per class will be charged to your credit card or checking account on November 1st. There is an extra $5 fee for XLC and XXLC costumes. Any remaining costume balances will be charged on January 10th. Once costumes are ordered, costume fees are non-refundable.
Full Season Tuition and Expense Policy: Our tuition is based on an annual "Regular-Season" tuition (September to May/June) divided into four quarterly payments for your convenience.
Payments are due on:
First quarter - September 12, 2020
Second quarter - November 7, 2020
Third quarter - January 23, 2021
Fourth Quarter - March 27, 2021
Please note tuition is based on the total classes per season, regardless of how many weeks are in a given month. Tuition does not include the registration fee, dancewear, shoes, costumes, recital tickets, private lessons, special events, etc. Tuition can be paid by credit card, check, or cash. First payment will be debited or charged with registration fee at time of registration. Subsequent tuition payments will be charged to your credit card on the week before each quarter.
If you wish to pay by cash or check, please notify the office as soon as possible so the credit card on file is not charged. Pay the year in full by September 12, 2020 and receive a 5% discount. Please notify the office as soon as possible if you plan to pay in full.
Summer Camps/Classes are payable in full at time of registration.
NSF Fees: Any returned-check charge will be subject to a fee of $35.00.
Decline Fees/Late fee: You will be notified immediately via email of any declined charges. Charges not paid within 5 business days will incur a $25 late fee.
*All charges will appear on your bank statement or credit card statement as being from North Reading School of Ballet.
*Any applicable discounts will be applied by office staff prior to billing.
Withdrawal and Refunds
FULL-Season Dancers Only:
There are no refunds for class withdrawal after the third week of the quarter. Costumes are non-refundable after orders have been placed. There will be a $35 administrative fee for all class cancellations/withdrawals.
