Release of Liability
RELEASE OF LIABILITY WAIVER
Please understand and agree that in participating in any dance class, workshop, rehearsal or performance, there is a possibility of physical injury. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child during any of The Studio 305 dance classes, rehearsals, performances, or activities. I also exempt, release, and indemnify Studio 305 of it’s owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by Studio 305. I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold Studio 305, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students liable for such damage, loss, or injury. I understand that I should be aware of my physical limitations and agree not to exceed them. If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights. I understand that there are certain risks and dangers associated with the activity and use of the facilities and that these risks have been fully explained to me. I fully understand the dangers that are involved.
Permission is granted Studio 305 to use photographs of students for publicity purposes.

Attendance
Pick up and drop up:
Under no circumstances are children to be dropped off without being unattended during the year or summer. No participant is not allowed in the studio without teacher supervision.
Attendance
Good attendance is important for the student and the rest of the class. Missed classes may be made up by attending a similar level class on the schedule. Missed classes will not be pro-rated and cannot be made up in lieu of tuition. Classes missed, when the studio is closed, according to the school calendar are not included in the tuition and are not made up. Dancers should not be not be late to classes.
All students are expected to attend their regularly scheduled classes. Each class offers a step forward in the educational process. A missed class will leave a child one step behind the other students. If for any reason why a student may need to be excused for a day he or she should let the office know in advance 702-444-0773 or officestudio305nv@gmail.com.
Parents and Dancers
Understanding Dance Education As a parent, you play an important role in supporting your child financially as well as emotionally. Encourage your child to do and be their best, regardless of what others may achieve. Dance is an individual art form and children need to be allowed to achieve at the pace that’s comfortable. No two students will progress at the same rate, even if they experience that exact same training. It’s important to encourage to focus on themselves, give their all, and be satisfied with their own accomplishments.
Your child's growth and success will depend greatly on the strong parent connection formed. Parents may be asked to help with fundraisers, costumes, props, assisting in recitals etc. We understand the success of the studio reflects the support of the parents. We honor out parents and appreciate the teamwork that is displayed which we carry to our dancers. We honor our working relationship and believe that our leadership together will create an amazing season for the dancers.
Parents are responsible for making sure that dancers are always prepared and on time to classes, rehearsals, and scheduled events. We count on our parent support for the positivity in growth and excellence. This should also be put in place at all competitions and recitals events. Parents and dancers must always be mindful of the energy you bring into the studio.
Membership Policy
I have read and understand, and agree to the policies and procedures for Studio 305, Home of the Rolle Project, Membership Contract & Agreement. I understand that by signing this agreement I am committing to the financial responsibility and time commitment for the entire season. I understand that Studio 305, Home of the Rolle Project, holds a no refund policy on any fees paid. You understand that our studio is a performing base company and dancers are required to perform. You understand that your child cannot perform or train with anyone without the studio director permission. By signing this 9 month agreement you understand school work and home behavior must be kept up at all times. If a dancer is on team they are required to attend all mandatory classes during the week. Please make sure your email and number is correct. The registration fee is to be paid upon registration via online parent portal. Payments should always be paid before the 7th of each month, after that it’s a $25 late fee. It is important to know this information. All payments are nonrefundable
Termination of Enrollment
In certain circumstances, when it is in the best interest of one or more students, it may be necessary for Studio 305 to terminate a student’s enrollment. Every effort will be made to correct a problematic situation before terminating enrollment.
Reasons for termination of enrollment including the following: Membership canceled, disruptive or dangerous behavior by students, Unpaid dues, or their parents Abuse of other children, staff, or property or Inability of Studio 305 to meet the child’s needs.
Summer Camp 2020
Summer camp is paid at time of registration
Summer registration fee = $25.00 per student.
Payment Policy
All Fees are due by the First of the month
There is a $25 LATE fee for any fees not paid by the 7th of each month
There will be NSF/Bank fee of $35.00 for any returned Check or Declined Credit Card
Cash can be paid in the office before the LAST business day of the month by 12noon.
All Tuition will be auto drafted by the Credit Card on file on the 1st of Each month
COVID 19 -Waiver
DANCER COVID 19 -WAIVER AND RELEASE OF LIABILITY
READ BEFORE SIGNING
In consideration of being allowed to participate in any way in Studio 305, Home of the Rolle Project program, related events and activities, the undersigned acknowledges, appreciates, and agrees that:
1. The risks of injury and illness (ex: communicable diseases such as influenza, and COVID-19 from the activities involved in this program are significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce these risks, the risks of serious injury and illness do exist;
2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASES or others, and assume full responsibility.
3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately.
4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS (Studio 305,Home of the Rolle Project their directors, teachers and/or choreographers, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("RELEASES"), WITH RESPECT TO ANY AND ALL INJURY, ILLNESS, DISABILITY, DEATH, or loss or damage to person or property.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
FOR PARTICIPANTS OF MINORITY AGE
(UNDER AGE 18 AT THE TIME OF REGISTRATION)
This is to certify that I, as parent/guardian with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of the activity and his/her responsibilities for adhering to the rules and regulations. Furthermore, my child/ward understands and accepts these risks and responsibilities. I for myself, my spouse, and child/ward do consent and agree to his/her release provided above for all the Releases and myself, my spouse, and child/ward do release and agree to indemnify and hold harmless the Releases from any and all liabilities incident to my minor child's/ward's involvement or participation in these activities as provided above.
