Release of Liability
I give permission for my child(ren) to attend Sprouts. My child may also participate in the activities provided by the program and travel in an insured Sprouts vehicle for transportation to/from school and all field trips approved by Sprouts.
I, on behalf of myself, my spouse, and each child designated on the registration form (my “child”), hereby waive and release all rights, causes of action and claims against Sprouts, its Officers, Directors, Agents, and Employees, for any loss, expense, damage or injury suffered by my child during the time my child is visiting Sprouts or while my child rides on the Sprouts bus, excluding gross negligence and intentional misconduct. If there is a need for emergency medical treatment, 911 will be called. Should an ambulance be needed, parents will be responsible for any costs. Parents will be contacted as soon as possible after contacting 911. I understand that the provision of child care contains risk of injury to persons and damage to property, and that by signing this release I engage Sprouts LLC, to provide temporary childcare for my children at my own risk. I further understand that there is an assumption of risk associated with the use of the center. I have been given an opportunity to inspect the premises of Sprouts LLC, and found that it is safe and satisfactory for my child. I also have been given the opportunity to ask questions and obtain answers to my satisfaction regarding any and all aspects of Sprouts LLC, and this Release. I understand this Release will be kept on file at Sprouts and will continue in effect for this and any future visits my child may make to Sprouts.
Illness & Emergency Procedures
If a student becomes ill at Sprouts and is unable to participate in activities, the parent/guardian will be notified. If a student is hurt, a member of the staff or an authorized person will administer immediate first aid. If the situation should require immediate medical attention, we will attempt to contact and inform the parent/guardian as soon as possible. In the event that the parent/guardian cannot be reached, the emergency contact person will be called. We will call the designated physician and/or local emergency unit for treatment and/or transportation to a hospital. A staff member will accompany the student to the hospital and stay until the parent/guardian arrives and signs the student into his/her custody.
Children should be kept at home if they show any of the following symptoms and should not return to Sprouts until they have been symptom free for a full 48 hours:
*temperature over 100 degrees
*any un-diagnosed rash
*sore or discharging eyes or ears
*thick, yellow nasal discharge
*diarrhea or vomiting
*head lice

Administering Medication
We will not administer medicine at Sprouts. Please contact us if you have an extenuating circumstance.
Snacks & Lunch & Allergies
Camp & Full Day Events: We will provide a morning and afternoon snack during camp.
After School/Full Days: Please send additional snacks (2) for full days at Sprouts.
All children enrolled in ANY full day program are required to pack lunch. We are not an allergen-free facility, but strive to make accommodations for our students whenever we can.
Withdrawal Policy
If a parent decides to withdraw a child from the program, a withdrawal form must be completed. If the weekly/bimonthly/monthly draft has already occurred, no refund will be issued.
Personal Items Policy
Please label all of your child's belongings.
Remote Learning: Children are permitted to bring their own tablet/iPad/laptop to complete remote learning if your child is enrolled in remote learning assistance.
Personal toys, games, cell phones, tablets, ipads, etc are not permitted in the program. If a child uses any of these items, they will be taken and returned to the parent at the end of each day.
Camp: Please limit items brought to Sprouts each day, we do not have a lot of storage space. Backpacks, toys, water bottles, etc are not necessary for camp. Each camper will be provided a water bottle labeled with their name. Water bottles will remain at Sprouts to be filled and ready for each day.
Camp: Be sure to dress comfortably to have a blast all day! Sneakers and loose, cool clothes seem to work best!
Discipline Policy
We realize that the single most effective behavior management tool at our disposal is an emphasis on positive behavior. We are trained to be constantly on the lookout for opportunities to praise or otherwise reinforce positive behavior. Children will always be supervised in all activities and any inappropriate behavior will be handled immediately. Respect is expected to be shown by students, teachers, counselors and parents at all times. Any discipline is handled at the time of incident. If necessary, a discipline form will be completed for the parent to read and sing. (After three discipline forms, the child will either be suspended or removed from the program).
Sunscreen
Camp: Please liberally apply sunscreen on your child each morning. We will be outside at least 1-2 hours each day (weather permitting). Sprouts is not responsible for applying sunscreen to your child.
Picture Permission
Sprouts has my permission to use photographs and videos taken at Sprouts for display purposes, slide shows, social media and advertising purposes.
Transportation Policy
Sprouts has my permission to transport my child to and from approved field trips on the Sprouts Bus or Sprouts vehicles.
After School: Children are to go directly to the van/bus pickup location at school once school is dismissed for the day. Failure to get to the bus on time will result in the child being left at school. If a child misses the van/bus, the parent will be contacted by the school and will then be responsible for picking their child up. Sprouts will not be able to return to a school to pick up a child. *We make every effort to avoid this situation. Please encourage your child to arrive to the pick up point promptly.
COVID-19
We fully anticipate to offer you and your children the best experience ever! Due to the recent events of COVID-19, we are continuing our efforts to keep Sprouts as safe as possible.
Steps we are taking in accordance with CDC guidance:
*All students will be dropped off curbside and greeted by a staff member. We will have a hand-sanitizing station set up and will take each child's temperature using a no-contact thermometer before entering the building. Any child who has a fever above 100.4 degrees or above or has other signs of illness will not be admitted into the facility.
*Constant cleaning of classrooms and supplies.
*Hand washing and sanitizing will be required often throughout the day.
*Small groups of children to aid in social distancing.
*Staggered play time outside to limit number of children together.
*All students will be picked up curbside and a staff member will bring your child to the car.
*No adults, other than staff members, will be allowed in the facility while kids are present. If you wish to tour our facility, we will be happy to meet you at a time when there are no students present.
Parent Expectations:
*Take your child's temperature before leaving the house.
*Ensure your child does not have a fever, shortness of breath or cough.
*Do not send your child if they have been given Tylenol or Ibuprofen in the last 8-10 hours.
*Do not send your child if they have been in contact with anyone that has tested positive for the virus or if you have reason to believe you, your family, or your child has been in contact with anyone that tested positive.
*Please keep any child showing any signs or symptoms of illness home (including, but not limited to, cold, cough, sneezing, or any other symptom). DO NOT send your child if they have had a fever within the last 24 hours or are in contact with someone that is in isolation.
I understand the risk associated with allowing my child to participate in summer camp/all day care. Sprouts cannot guarantee that my child will not be exposed to Covid-19. I surrender my rights to litigation, civil or criminal, to any volunteer, employee, owner, or director of Sprouts for any health issues whatsoever related to Covid-19. I further understand that Sprouts, LLC has not made any verbal commitments related to Covid-19 and I understand the risks of sending my child to summer camp.

After School Care: Monday-Friday from 2:30pm-5:30pm
Summer Camp: Monday-Friday from 7:30am-5:30pm
School Child Care (pandemic): 8:00am-5:30pm
Ages:
After School- PreK (public school only)-5th grade (We do offer middle school from Phoenix & NCLA - upon review).
Camp- Our camp is designed to be developmentally appropriate for children in grades PreK-6th grade. All campers MUST be potty-trained and able to use the restroom independently.
We provide After School Care for:

2. Phoenix Academy
3. NCLA
4. Southwest Elementary
5. Florence Elementary
6. Oakview Elementary
We will provide Summer Camp for 2021 from June 1st-August 27th. All weeks are listed under "Classes & Events". You may register for as many or as little weeks you need.
Enrollment Procedures:
1. Online registration must be completed and approved before the child may attend our programs.
2. Parents are responsible for completing/updating all sections of the registration.
3. At the time of enrollment, parents are required to pay a non-refundable registration fee of $65.00.
Full Day Policy
Full day care will be offered on Teacher Workdays, holidays, and inclement weather days (and during pandemics). Operating hours will be from 8:00am-5:30pm. Parents will be able to check the online calendar listing the dates that full days will be offered. Parents are required to enroll their child online in advance for any full day. We must receive 24-hour cancellation notice via email in order for the parent not to be charged. If we do not receive the proper cancellation notice, there will be a $15.00 charge for the day. During periods of inclement weather, we will be open as long as it is safe to travel. We will let parents know via email what are our operating hours will be.
Full Day Enrollment Procedures for DROP IN CUSTOMERS:
1. Log into portal.
2. Select "Classes & Events" tab at the top.
3. Select "Find Events". From here, you can select the days you need.
Payment Policies
Tuition (After School)
Tuition is due every Monday.
All fees will be auto drafted either bimonthly or monthly. (As of now, we will be drafting weekly due to the constant changing to schools in session)
Any Declined Credit Cards or Returned Checks will result in an automatic charge of $30.00
Payment must be received every Monday by 5pm, or a late fee of $15.00 will be applied to your account.
*Tuition is due EVERY WEEK, regardless of absences due to illness, vacations, etc. *
*No sibling discounts are offered at this time*
Registration Fee (All Programs)
A $65.00 (non-refundable) registration fee per family will be due upon enrollment. This fee will be added to your first draft.
Summer Camp is $160.00 ($125 for each additional child) per week for the 2021 summer program. We require a $25.00 deposit, per week, per family, due at the time of registration. The deposit is non-refundable and will be applied to your weekly tuition. The total tuition, less the $25 deposit, will be drafted each Friday prior to the week the child is signed up for from the account on file.
We will close promptly at 5:30pm beginning on August 17th. Please be courteous and arrive on time. There will be a charge of $10 for any pick up after 5:30pm. If you arrive after 5:45pm, there will be an additional $10 charge plus $1 per minute.. Please arrive on time.
