Assumption of Risk
I understand that while Arise School of Dance has undertaken reasonable steps to lessen the risk of transmission of COVID-19 in connection with their services, Arise School of Dance is not responsible in any manner for any risks related to COVID-19 in connection with their services. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending Arise School of Dance and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I am fully aware that participation in services at Arise School of Dance and being on their premises carries with it certain inherent risks related to COVID-19 transmission that cannot be eliminated regardless of the care taken to avoid such risks.
Medical Emergencies
In the event of an emergency, if the emergency contacts that I have listed for my child cannot be reached, I hereby grant Arise School of Dance the authority to arrange for transport of my child to
the nearest medical care facility.
Release of Liability
I ( or the parent or guardian if student is under 18)years) assume any and all liability and responsibility for the above student, myself, and others under my care with regard to any and all activities directly or indirectly associated with dance instruction given in studio or online through Arise School of Dance.
Privacy Policy
I hereby consent that any photographs and/or videos taken of myself or my son/daughter during participation of Arise School of Dance in studio or online classes and performances may be used by Arise School of Dance in promotional advertising. I understand that all photographs and videos used in promotions will not include any personal identification of the participants.
Payment Policy
This Registration Form constitutes a contract between myself and Arise School of Dance and I understand I am paying for a reserved space in the classes (in studio or online) I am registering for. I understand that I will continue to be responsible for the full amount of tuition until I notify the Arise administration, in writing, of my or my child's withdrawal from classes. At this point I will pay the remainder owing on the tuition or be reimbursed based on Arise School of Dance's current dropout policy. I understand that Arise School of Dance reserves the right to cancel any class that does not have a sufficient number of students enrolled to support the class, at the discretion of the directors. I give Arise School of Dance permission to charge the primary card on my account (credit or debit) on the first of every month for my full tuition owing for that month.
