Release of Liability
As the legal parent/guardian of the listed student(s), I hereby released, waive, discharge and covenant not to sue Rising Stars Dance Academy, LLC and all parties involved with Rising Stars Dance Academy, LLC from any and all liability. Including but not limited to any and all claims, injury, damage, or losses of any kind on account of the student(s), parents, siblings and/or those accompanying the student(s) while in or around the premises or any premises under the supervision of Rising Stars Dance Academy, LLC its owners and operators.
Medical Emergencies
I, the undersigned gives permission to Rising Stars Dance Academy, its owners and operators to seek medical treatment at the parent/guardian’s expense, for the students in the event that Rising Stars Dance Academy is not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health.
Photo & Video Release
I give permission for Rising Stars Dance Academy to take and use photos and videos of my family, including students and parents/guardians. I give permission for pictures and videos of my family to be used in an appropriate manner on the Internet, in media, and in print for the promotion of Rising Stars Dance Academy.
Payment Policy
The initial payment will be charged within 24 hours after submitting this form. After that, all families must be on automatic credit card payments which will auto-run on the first (1st) of each month. If you prefer to pay in cash or check you must pay prior to the 1st or the credit card on file will be charged. Accounts must be settled by the 10th of the month or a $10 late fee will be added. Returned checks will be charged a $30 returned check fee. Tuition is based on a breakdown of the year into monthly payments. Rising Stars Dance Academy does not prorate based on the number of weeks per month, holiday closures, absences, or any other closures.
No Refund Policy
No refunds will ever be given. Once charges have been posted the fee is owed.
Cancellation
I am aware that my child will be auto-enrolled month to month until the end of June 2021. If I want to cancel I understand I must give notice in writing or email at least 2 days prior to the end of the month. You will receive a reply with cancellation confirmation. No verbal cancellations will be accepted. After cancelling your enrollment, you will remain on our email distribution list unless you request to be removed.
Agreement
I have read this agreement, I understand its terms in entirety and I sign it freely.
