Assumption of Risk and Release of Liability
I, on behalf of myself, my heirs, executors, agents, assigns, and representatives, hereby indemnify, release and forever hold harmless Honey Arts and Sports, LLC, as well as its Manager, employees and instructors, from any and all claims of liability arising from an accident, personal injury, death, or property damage sustained by my child, myself, the minor child for whom I am a legal guardian, while that person is participating in activities connected with Honey Arts and Sports, LLC, including but not limited to dance classes, rehearsals, performances, parties camps or other activities. I understand that dance activities have inherent risks of injury that include, but are not limited to, pulled muscles, sprains, broken bones, viruses and/or sickness, and, being fully aware of all risk, I consent to have my child, myself, the minor child for whom I am the legal guardian, participate in the programs and activities offered by Honey Arts and Sports, LLC., and I accept full responsibility for providing health and accident insurance coverage for the protection of myself, my child, the minor child for whom I am the legal guardian. Participation in class and/or events indicates the acceptance of the risks by attendees and their parents and/or guardians.
By signing this statement, I declare that the Student named above is in good health, with no known physical conditions that might prevent his/her/my participation in dance classes, rehearsals, performances, parties, camps or other activities connected with Honey Arts and Sports, LLC.
Further, I understand and acknowledge that because of the physical nature of dance lessons and the performing arts, there may be physical contact between instructors and students during classes or other activities. I understand that at times, for proper instruction and safety, physical contact is required and necessary.
I understand that I have the responsibility to disclose any medical information that would preclude my child from participating in Honey Arts and Sports, LLCs programs and activities. I agree to hold Honey Arts and Sports, LLC, their agents, and employees harmless if full disclosure of any preexisting medical condition has not been provided.
I agree that I shall hold Honey Arts and Sports, LLC, their agents, and employees harmless from any claims for injuries and/or damage to third parties or their property arising from the negligent or willful misconduct of my child.
I hereby give consent to Honey arts and Sports, LLC to summon and provide emergency medical care, hospitalization, or other treatment which may become necessary in the event of illness or injury.
Lastly, I hereby give consent for my child to participate in the programs and activities offered by Honey Arts and Sports, LLC, and I am fully aware that such participation may result in exposure to the COVID-19 virus. I hereby release Honey Arts and Sports, LLC from any responsibility in connection with such exposure.
I declare that neither my child nor any person in our household has been diagnosed with COVID-19 in the last 14 days, has shown flu and / or cold symptoms, or has been in contact with a person diagnosed positive for the virus. I further declare to be aware that I must report any suspicion or confirmation of COVID-19 in my child or person in my household, or someone that we have been in touche with, as well as that I understand that my child will not be admitted if he / she shows signs of flu or cold or temperature equal to or greater than 100.4º.
I am aware of all the studio policies and I understand that they might change by government decision.
I have carefully read this form and I fully understand its content and therefore I sign below.
Medical Emergencies
I give consent to Honey Arts and Sports, LLC to administer treatment to my child and/or to summon emergency help, as the case may be.
Furthermore, in case of an injury or illness that is life threating or in need of emergency treatment, I authorize Honey Arts and Sports, LLC’s staff to summon any and all professional emergency personnel to attend, transport, and treat the student and to use consent for any X-ray, anesthetic, blood transfusion, medication, or other medical diagnostic, treatment, or hospital care deemed advisable by, and to be rendered under the general supervision of any licensed physician, surgeon, dentist, hospital, or other medical professional or institution duly licensed to participate in the state in which such treatment is to occur.
I hereby grant the staff of Honey Arts and Sports, LLC permission to apply, in accordance with the directions for use as specified on the container, one or more of the following external preparations: baby wipes, mosquito repellent, lotions, sun screen, non-prescription ointments (i.e. antibiotic ointment), topical Benadryl ointment/cream. I will provide Honey arts and Sports, LLC with a detailed list of any and all allergies of the student.
Medical Release Form
I/We agree the undersigned parent(s) or legal guardian(s) of the above-named minor, acknowledge that I/We may not be available to authorize medical care of said minor child and I wish to appoint someone to act in my place in my absence and to give such authorization. This authorization is intended to give Honey Arts and Sports, LLC’s staff the right to give consent to authorize emergency medical care.
It is intended that this document be presented to the physician or appropriate hospital or medical representative at such time as the medical care shall be authorized. It is intended that the authorization relieve the physician, dentist, person rendering such care at the hospital or institution in which such care is given, from any liability resulting from the failure of me, the parent or guardian of the above-named minor, from signing a consent or authorization to render such care. It is the intent that Honey Arts and Sports, LLC shall act in my stead in making such decisions.
I have put the important medical facts, if any, on the form. The medical facts are intended to help the doctor in deciding what treatments to be given but are in no way intended to restrict the giving of authorization or consent by Honey Arts and Sports, LLC. I understand that this form is in effect from the date signed and that is my responsibility to inform Honey Arts and Sports, LLC of any changes to this form.
Authorization for Photography/Video
I hereby authorize and give consent to the staff of Honey Arts and Sports, to take/use still photographs, digital photographs, motion pictures, television transmission, and/or videotaped recordings (hereinafter “recordings”) of me, my children, or my wards for educational, research, documentary, marketing and public relations purposes.
I understand that any such recordings may reveal my identity, or the identity of the student through the image itself without any compensation to me, my child or wards.
I also understand that all recordings taken of me, my children or wards shall be the sole property of Honey Arts and Sports, LLC.
With regard to the use of any recordings taken of me, my children, or wards, I hereby waive any and all present and future claims you may have against Honey Arts and Sports, LLC, their staff, service providers, employees, agents, affiliates and members.
I have carefully read this waiver and authorization and I fully understand its content.
Payment Policy
Tuition will be billed on the 1st of every month for all classes you are enrolled in.
For Summer Camp, tuition is due on the 1st day of the session.
Tuition and Registration fees MUST be paid in full before a student is permitted to attend class.
No months or sessions are prorated, tuition is nonrefundable, nontransferable and no tuition adjustments or credits are given for missed classes.
The tuition is based on lessons received for an entire school year and not on the number of classes received each month. This is standard private school policy. There are 10 installments, in addition to the annual registration fee. Payments begin in August, with the first installment for September and continue through June of the following year. The studio does not send statements, so parents/guardians should take care of payment on time. Tuition each month remains the same regardless of the number of lessons and/or weeks in a month, regardless of holidays and the student’s attendance, and regardless of extended travel by the student.
Late payment fee
Tuition is due on the 1st day of each month and will be automatically charged to the credit card on file.
There is a $20 penalty for payments made after the 3rd day.
Make up Policy
Due to COVID-19, Makeup classes can be taken in any class that is level appropriate for the student within 30 days of the missed class, since the class has spots that allow to keep the social distancing rules observed . Please check with your child’s teacher for the appropriate day and time.
Withdrawal Policy
A thirty (30) day written notice is required to discontinue/withdrawal/drop a class or classes. A withdrawal form must be filled out by the parent/guardian and submitted to the front desk.
Any makeup classes on account must be completed by the end of the 30 day withdrawal period or they will be forfeited
Accounts will remain open and active, with tuition due through the end of those 30 days.
Minor Acknowledgement
The undersigned parent or guardian further warrants and agrees that the above-mentioned minor child will not disaffirm or disavow this consent and permission on the grounds that the participant was a minor on the date of execution or any similar grounds, or endeavor to recovery from Honey Arts and Sports LLC or related entities or parties, shareholders, directors, officers or other associate persons, any sums whatsoever from participating in the activities.
