PAYMENT POLICIES
A. Tuition is non-refundable and non-transferable.
B. All class program rates are payable in advance.
C. Tuition is due on the 25th of each month for the following month and is considered late on the 2nd. A late fee in the amount of 10% of the outstanding balance will be added on the 2nd of each month for late payments received on or after the 2nd of each month.
D. If the Tuition has been paid, the student has secured a spot in the program. Once the student's spot is secured, the parent/guardian will be held fully responsible for meeting the tuition obligation according to the session(s) enrolled in at the time of registration. This agreement is binding under all circumstances, including, but not limited to: withdrawal from course, moving out of area, vacation, injury or illness, or conflicts that may arise.
E. Tuition payments must be made on time. Students will be denied access to in-person or virtual classes on the 2nd of the month if the account is not paid in full by the 1st of the month.
F. Bills will NOT be sent in the mail.
G. A $25.00 fee will be charged for every returned check and will be added to any existing account balance.
H. Students must be picked up on time. Accounts will be charged a fee if the building has to remain open due to a child who is not picked up on time. The late pick-up fee is $1.00 per minute and will be billed to your account and subject to late fees.
I. Make-up classes are not permitted and monetary credit will not be given for a missed class.
GENERAL POLICIES AND OTHER LIABILITIY WAIVERS
A. Dance and participation in the Academy is physically demanding and can result in personal injury.
B. The Academy of Nevada Ballet Theatre is not responsible for any personal injury sustained by the student in the student's home or other location where the student is participating in a class, or in the Academy or on the building premises, or as the result of the student's participation in class, rehearsal, or performance whether or not at the Academy or on the building premises. Any injuries or illness shall be promptly reported to the School Director and/or front desk.
C. The Academy of Nevada Ballet Theatre is not responsible for loss or theft of any personal property on its premises. Any valuables should be left at home.
D. Staff of Nevada Ballet Theatre will make reasonable effort to immediately contact the parent or guardian in the case of illness or injury which might occur on any premises in which ANBT conducts classes, events, rehearsals or performances. However, in an emergency in which time is of the essence (possibility of broken bone or serious injury, bleeding, seizure, unable to breathe, or similar situations), I hereby grant permission to Nevada Ballet Theatre staff to assess the illness or injury and determine the next step of care, which may include the transport or arrangement for the emergency services transport of the student to a medical doctor or hospital in case of illness or injury, and he/she may receive such treatment as deemed medically necessary by a licensed physician. In the case of situations which are not believed to be of an emergency nature as stated above, the student may be requested to stop participating in class, to rest, apply ice, or follow other reasonable first-aid procedures.
E. After any absence due to illness or injury, Nevada Ballet Theatre reserves the right to request a doctor's statement indicating the illness or injury that prompted the absence and releasing the student to return to participation in class.
F. Coronavirus/COVID-19 has been declared a worldwide pandemic by the World Health Organization. To help the parent/guardian or student to assess the student's health NBT requires the parent/guardian or student to use the Healthcheck App (or alternately, the Healthcheck web portal) each day that the student will be attending class at NBT's facilities. The Healthcheck App will ask the parent/guardian or student a number of health-related questions. The Healthcheck App will not share with NBT the parent/guardian's or student's responses to the questions. The responses given by the parent/guardian or student will not be accessible to NBT. However, the responses will be assessed by the Healthcheck App which will result in a Green, Yellow or Red status to be shown on your screen. The Healthcheck App will provide the result, whether the student received a Green, Yellow or Red status, to NBT. If a Yellow or Red status is shown the student will not be able to attend classes. If a Green status is shown the student may attend classes.
Accordingly, the parent/guardian or student:
1. Agrees to acquire and use the Healthcheck App by the required deadline as determined by NBT before class at the NBT facilities. Alternatively, the parent/guardian or student may also respond to these questions through the Healthcheck portal at www.healthcheck.io
2. Agrees to respond truthfully and accurately all information required by the Healthcheck App each time that the Healthcheck App is used by the parent/guardian or student.
3. Agrees to not have the student not to attend classes if the Healthcheck App results in a Yellow or Red screen.
4. Consents to the result of a Green, Yellow or Red Screen to be provided to NBT.
The Red, Yellow or Green status information is confidential and will be shared only with Nevada Ballet Theatre and the Southern Nevada Health District if disclosure is required by Nevada state statute or regulation.
G. I hereby grant permission to Nevada Ballet Theatre to photograph and/or videotape the student for any public relations, educational, or promotional use for the school or company. The Academy cannot guarantee that any child can be omitted from all educational or promotional photos.
H. I hereby release, remit, acquit and otherwise hold harmless Nevada Ballet Theatre, its employees, volunteers, faculty, staff and independent contractors from and against any and all claims, suits, and causes of action for any injury, illness, property damage or loss sustained or incurred during the student's participation in the activities, classes, or performances of Nevada Ballet Theatre, arising in whole or in part from the negligence or other misconduct, including gross negligence, of any individual or entity, inclusive, but not limited to, Nevada Ballet Theatre.
I. I understand that due to the strenuous curriculum, Pre-Professional students are not allowed to perform with other dance schools or companies without written consent from the School Director.
J. I certify to Nevada Ballet Theatre, its employees, volunteers, faculty, and staff that the named student is physically capable of participating in all activities, classes and performances of Nevada Ballet Theatre.
TERMINATION & WITHDRAWAL POLICIES
A. I understand this enrollment contract may be terminated by the parent/guardian with written notice, and will take effect at the end of a month in which classes have been taken.
B. I understand that this enrollment contract may be terminated by the Academy in the event Student and/or Parent/Guardian fail to meet any of his/her obligations with this enrollment contract and/or the rules and regulations of the Academy of Nevada Ballet Theatre as set forth in the Student Handbook
C. I understand that any advance payments made toward my account may be forfeited in the case of contract termination or withdrawal.
D. I understand that if I withdraw and wish to re-enroll within the same school year, a Re-Enroll Fee of $55 will be charged.
E. I understand that I am obligated and will be held fully responsible for the full tuition amount for the student under the payment terms I have selected. I understand that if the student is withdrawn from the course for any reason, I am still liable for his/her tuition balance remaining for the month in which classes were attended.
VIRTUAL CLASS POLICIES
A. Nevada Ballet Theatre is not responsible for connection or internet issues that may impede a student's ability to take a scheduled class.
B. It is the responsibility of the Parent/Guardian to access the Zoom link provided for virtual classes.
C. The same rules and regulations for students who take in-person classes apply to students taking classes virtually, as outlined on page 4 in the Student Handbook under Virtual Class Procedures/Policies.
SAFETY ENTRANCE/EXIT POLICIES
A. The parent/guardian is responsible for following the ANBT Health and Safety Policies and Procedures which can be found on the blog at www.AcademyofNBT.blogspot.com. These policies are subject to change in order to remain compliant with all local, state and nationwide guidelines and mandates. Nevada Ballet Theatre will make best effort to notify you by email when there are updates/changes however parents are responsible for remaining informed and up to date on these policies.
B. The student must be brought to their assigned drop off location each day and cannot enter in any other location.
C. Students who are more than fifteen minutes late for their first class of the day will not be allowed to attend any classes that day.
D. After exiting the building after class, students may only re-enter if there is an emergency or the parent is late for pickup
E. When dropping off the student, Parent/Guardians must make sure the child gets into the building before leaving.
F. All students, faculty and staff must wear a face covering (cloth mask, surgical mask, etc., that covers both the mouth and nose) at all times, in all common areas, restrooms and during all classes. There are no exceptions to NBT's policy for wearing face coverings. All students are required to have a minimum of two (2) face coverings with them each day they enter the building and must wear one at all times.
G. Students must be picked up on time, at the same location where they entered.
H. Students are not allowed to use the front desk phone to call their parents when they are done. Students must be met at their entrance/exit location when their last class is over.
I. Nevada Ballet Theatre has the right to dismiss any student who does not comply, or whose Parent/Guardian does not comply, with the Building Entrance/Exit Procedures listed in the 2020-2021 Student Handbook.
FINAL AGREEMENT
Nevada Ballet Theatre has the right to dismiss any student who does not comply with its standards, codes, rules and regulations. In the event that a student is expelled from the program, Nevada Ballet Theatre is not obligated to refund tuition or fees paid by parent or guardian.
Nevada Ballet Theatre reserves the right to change these policies at any time without notice and Nevada Ballet Theatre policies will comply with current local, state and national level standards, guidelines and mandates as required.
