Acknowledgment, Waiver & Release
I am at least 18 years of age and of sound mind and body and I have read, and understand, the required policies and agreements. I agree to follow the Policy and will require that my minor children follow the Policy at all times and I take full personal responsibility for any failure on my part or my child's part to follow the Policy.
I hereby acknowledge my receipt and understanding of the Policy and I agree to the terms set forth therein. I acknowledge and accept that there is risk associated with entering the Studio during a pandemic, dancing and teaching dance, generally and during a pandemic, cleaning and otherwise following the policies and procedures of the Studio (the "Activities"). I additionally hereby expressly waive and release any and all claims, now known or hereafter known, against the Studio and it's owners, on account of injury, illness, disability, death or property damage arising out of or attributable to my, or my child's engaging in the Activities, whether arising out of the ordinary negligence of the Studio, the owners or otherwise. I covenant not to make or bring any such claim against the Studio or the owners, and forever release and discharge the Studio and the owners from liability under such claims. This acknowledgement, waiver and release does not extend to claims for gross negligence, willful misconduct or any other liabilities where prohibited by applicable law.
I shall defend, indemnify and hold harmless the Studio and its owners against any and all losses, damages, liabilities, deficiencies, claims, actions, judgments, settlements, interest, awards, penalties, fines, costs, or expenses of whatever kind, including reasonable attorney fees, fees, the costs of enforcing any right to indemnification under this release and the cost of pursuing any insurance providers arising out of, or resulting from, any claim of a third party related to my failure to follow the Policy.
I, hereby willfully and knowingly WAIVE AND RELEASE any and all personal liability claims against Dawn's School of Dance and its staff, which may arise due to my/my child's participation at Dawn's School of Dance. I am fully aware that it is sometimes necessary for the teacher to touch the student to draw awareness and/or make physical corrections to the body that may apply a slight stretch to the body. I assume all responsibility for my/my child's participation in the Dawn's School of Dance
I further agree that any pictures or videos taken of me/my child while I participating in any events or Dawn's School of Dance will become the property of and may be used for advertising in the studio, printed materials and websites belonging to the Dawn's School of Dance with no payment or fees due me/my child.
Class Protocol
In addition to following the basic infection prevention measures (detailed below), dancers and other persons entering the Studio must follow the below protocol.
1. Pre-Arrival Symptom Check: All dancers should have their temperatures checked before arriving at the Studio. If they have a temperature, they cannot come to the Studio. Upon arrival, you must certify that your child is symptom free and are not an Exposed Person.
2. Temperature Check: Prior to arriving at the Studio, all dancers should have their temperature taken. If a dancer has a fever of 100.4 degrees Fahrenheit or above, they cannot come to the Studio. Attached to this policy as Appendix A is a list of common COVID-19 signs and symptoms. If a dancer has any these symptoms, they must stay home.
3. Face Covering: When arriving at the Studio, all people must wear a mask or face covering. Masks or face covering must be worn while dancing. If needed, they should dial down the intensity of their dancing if they are feeling short of breath, lightheaded or dizzy, but please let them know that their body should adjust to exercising with a mask over time. If a mask becomes wet from sweat or heavy breathing, it should be changed out for a clean, dry mask. While dancing, if possible, dancers should remain at least 10 feet apart, but never less than 6 feet apart.
4. Hand Sanitizing: Upon entering the Studio, you should immediately sanitize your hands. Your hands should be sanitized often and as needed throughout your time in the Studio.
5. Pre-Class Wipe Down: Prior to all classes commonly touched areas such as doorknobs, light switches, air purifier buttons, surfaces and stereo equipment will be wiped down with alcohol-based disinfecting wipes.
6. Air Purifier & Open Window and Doors: There will be an air purifier running in each dance room and in the common area. Doors or windows to the outside that can be opened, will be opened and left ajar.
7. Class Arrival Time & Waiting Room: Dancers are not allowed to enter the Studio until the time of their class and must promptly leave after the class. There are no visitors, parents, dancers, faculty or other people allowed in the Studio other than those dancers taking the class and their teacher. Others must wait outside of the Studio in a socially distancing manner.
8. No Dressing Room: Dancers should arrive dance-ready and will not be permitted use of the changing or common areas to prepare for class.
9. Dance Shoes & Street Shoes: All dancers are required to bring a dance bag where they can store their street shoes immediately upon entry to the Studio. Dancers are encouraged to wear full-footed dance shoes when possible. It is understood that dancing barefoot is often the preference. However, for the duration of Phase III, it is highly recommended (but not mandatory) that dancers wear a ballet, lyrical or jazz shoe when they would normally dance barefoot.
10. Dance Grid & Equipment: The dance rooms in the Studio will be divided into "Dance Grids" that provide each dancer with at least a 6-foot by 6-foot dance area where they are to remain during the class. Use of equipment such as the ballet barre, mats and other equipment or props where more than one person will touch something is prohibited. For ballet, floor work and adagio will accomplish much of what was done at the barre. For acrobatics classes, dancers will work on flexibility and less dynamic acrobatics (walkovers, handstands, etc.) from their Dance Grid. As alternative to the Dancer Grid, faculty may place markers throughout the dance room that are at least 6 feet apart.
11. Direct & Indirect Contact: No activities that require direct or indirect contact including partnering, tactile cueing, across the floor work or touching of the same equipment is permitted. Additionally, hands-on corrections will be avoided by all faculty.
12. Post-Class Wipe Down: Following each class, older dancers will be provided with a disinfecting wipe to wipe down their Dance Grid space (similar to how one would wipe down a yoga mat or exercise equipment at the gym). Faculty will again wipe down commonly touched areas throughout the Studio. The entire dance floor will be sprayed down by faculty with the alcohol disinfectant solution following every class.
13. Attendance Policy: During this time, we will not have an attendance policy that penalizes or awards dancers for their attendance.
14. Know that class will be different: In early phases of reopening, it is recommended that we not teach especially dynamic sequences with large movements or combinations that travel across the floor to minimize the movement of droplets throughout the room. While it may seem that the "fun" parts of class are forbidden for the time being, this is a good opportunity to zero in on technique, flexibility, basics, strength and conditioning.
Contact Tracing
Those who are sick and/or have tested positive for COVID-19 or have been exposed to a person who tested positive for COVID-19 are considered "Exposed Persons." When we have been advised that a faculty or staff member, dancer, family member or any other person who has been inside the Studio is an Exposed Person, the Studio will notify those who have been in contact with the Exposed Person as soon as possible.
For example, if we are advised that a dancer is an "Exposed Person," we must notify the families of the other dancers in the Exposed Person's Dance Class and their faculty member as well as the other Dance Classes taught by that faculty member. The Exposed Person, and those who have been in contact with the Exposed Person, should isolate and may not enter the Studio for two weeks from the date they stop having symptoms and/or they test negative for COVID-19. The Virtual Dance Studio can be used at this time.
When notifying people that they have been in contact with an Exposed Person, for legal reasons, we are not permitted to identify the Exposed Person (i.e., we cannot name names and we must just generically advise the family of the exposure without implicating an individual).
Basic Infection Prevention Measures
The practice of good hygiene is imperative during this time. The following practices are mandatory, as applicable.
1. Hand washing: Faculty and dancers should frequently and thoroughly wash their hands with soap and water or alcohol-based hand sanitizers containing at least 60% alcohol.
2. Sick Days: If you are feeling under the weather, have a temperature or are otherwise sick you must stay at home and follow the "Sick Protocol and Virtual Dance Studio."
3. Respiratory Etiquette: Coughs and sneezes should be covered and hands should be subsequently washed.
4. Use of Tissues: When a tissue is used, it should be disposed of properly and hands should be subsequently washed.
5. Face Coverings: All people in the Studio must wear a face masks or other facial coverings while in the Studio.
6. Social Distancing: All persons in the Studio must engage in social distancing. Where they are unable to stay at least 6 feet apart, they must wear a mask or facial covering.
7. Virtual Dance Studio: When, according to this policy, a teacher cannot teach from the Studio or when a dancer cannot be in class, the Studio will utilize the Virtual Dance Studio.
8. Staggered Shifts: Faculty and dancers should not be in the Studio unless they are teaching or taking a class. As a general rule, no person, dancer or faculty member should be "hanging out" in the Studio. Faculty will not comingle Dance Classes to the extent possible.
9. Use of Studio Equipment: All faculty, dancers and other persons in the Studio should refrain, when possible, from using communal equipment in the Studio such as stereo equipment, desks and communal phones.
10. Housekeeping: The Studio will be toughly cleaned at least once a week. Faculty will do a pre-class and post-class wipe downs. Older dancers will also wipe down their Dance Grids following their class. Before every class faculty will spray wash the floor with a bleach or alcohol based cleaner.
11. Travel Restrictions: Faculty or dancers may be required to stay home for a period of two weeks after traveling to high risk COVID-19 areas or otherwise engaging in activities that may result in them becoming an Exposed Person. Travel on behalf of the Studio for conventions, competitions or performances are canceled until further notice.
Appendix A COVID Symptoms
1. Fever > 100.4 degrees Fahrenheit or 38 degrees Celsius
2. Cough, usually dry
3. Shortness of Breath
4. Sore Throat
5. Headache
6. Chills
7. Congestion
8. Muscle and joint plain that are unexplained
9. Nausea or vomiting
10. Loss of sense of smell
11. Diarrhea
12. Oxygen saturation of < 93% with a reliable pulse oximeter taken by a health care provider
Payment Policy
Monthly Tuition:
• The annual registration fee of $25 per dancer with a $50 per family maximum is due at the time of registration.
• Payment is due on the first day of each month.
• Accounts not paid by the 15th of the month are subject to a $10.00 late fee. Arrangements can be made for special circumstances.
• No envelopes or statements will be handed out or mailed, however, tuition email reminders will be sent monthly.
• We accept Cash, Check and Venmo @Dawn-Rand (be sure the picture of Dawn populates) .
• Monthly tuition payments are non-refundable and non-transferable!
*Tuition is based on a monthly course fee and not by the individual class or weeks. Throughout the full dance season there are some months that have 2 classes as well as some that have 3, 4 or 5 classes in a month. All vacations and holidays have been taken into consideration when determining tuition fees for the entire dance season.
Monthly tuition is based on 9 equal monthly payments first monthly payment is due in September and the last payment is due in May.
